EE was using an air tool with a flapper wheel installed on it. He was working on a hoist cylinder for a haul truck. He was cleaning the packing seats to replace the packing. While cleaning the seat a particle of foreign material got in his left eye. He was wearing safety glasses, dust mask and ear muffs but admitted he should have worn a face shield or goggles.

Employee was greasing inside the house of the dragline. Employee had to squat down to grease a fitting when he felt something pull in his right leg (calf area). First day off work 1/27/09.
Employee was loading engine onto truck. He needed blocking. EE and another employee were lifting a 12x12x48 wooden block onto the truck. At mid lift EE's back snapped.

EMPLOYEE WAS CLIMBING OUT OF TEREX HAUL TRUCK. HE WAS COMING DOWN THE LADDER WHEN HIS FOOT SLIPPED CAUSING HIM TO SPIN AROUND AND HIT HIS RIB CAGE ON THE CORNER OF THE BUMPER. THE LADDER HAD M UD ON IT.

EMPLOYEE WAS GRADING HAUL ROADS WITH ROAD GRADER WHEN HE WAS STUNG ABOVE THE KNEE ON HIS LEFT LEG BY BEES WHO HAD A NEST IN THE GRADER SEAT. HE WAS TAKEN TO THE HOSPITAL AND RECEIVED TWO SHOTS . ONE FOR INFECTION AND ONE FOR BEE STING.
EMPLOYEE WAS LAYING ON BOTTOM DECK SCREEN, CUTTING BELOW TOP DECK OF SCREEN IN TIGHT CORNERS AND A PIECE OF HOT IRON FELL INTO HIS EAR.
EMPLOYEE WAS REACHING BACK TO GREASE MAIN DRIVE AND HIS BACK POPPED OUT OF PLACE.

THE COMPANY HAD RENTED 2 CRANES TO DISMANTLE A CONVEYOR. THE CRANES WERE SUPPORTING GANTRY AND CONVEYOR FRAME. ONE MAN WAS STANDING ON CATWALK AND ONE MAN WAS INSIDE FROAME OF CONVEYOR CUTING LAST BOLT FROM GANTRY WITH A CUTTING TORCH. WHEN LAST BOLT WAS CUTG LOOSE THE 70 FT RUSTED CONVEY FRAME COLLAPSED IN THE MIDDLE. CUTTING TORCH WAS SWAYING AROUND, AND THE MEN WERE BEING SHAKE
EMPLOYEE WAS LI0TING A CHAIN HOUSE INTO POSITION. HE MOVED TO HAVE A BETTER LIFT AND INJURED HIS BACK.

EMP WAS WORKIN1 ON A DRAGLINE. IN ORDER TO OBTAIN A TOOL HE PULLED HIMSELF UP ON THE DRAGLINE, USING THE TRACKS AND HAND RAIL FOR SUPPORT. THIS IS A NORMAL MEANS OF ENTRY WHEN THE DRAGLINE IS IN A RESTING PO8ITION. HOWEVER, IF THE TRACKS ARE NOT IN AN IDEAL POSITION IT CAN CAUSE A T R EMENDOUS AMOUNT OF STRAIN TO A LARGE/BULKY TYPE INDIVIDUALS LOWER BACK. HE FELT PAIN IN HIS LOWER
EE WAS POSITION3NG SLUSHER CABLES TO RUN WHITE SAND THRU PLANT. HE COMMITTED AN UNSAFE ACT. NORMAL PROCEDURE CALLS FOR RELEASING TENSION ON LIFTING CABLE WHICH IS HOLDING DRAG CABLE UP. EE D ID NOT RELEASE 7ENSION & IN TURN HANDLE RELEASED WITH TREMENDOUS FORCE CATCHING BOTH EES HANDS FRACTURING HIS RIGHT WRIST.
