Employee slipped and fell while moving a rock. The rock landed on ee but per employee it did not cause any harm to self.

While stacking stone from a conveyor belt to a pallet, employee twisted too quickly and felt a pop in the lower back, then felt pain.

Employee pushed stone slab onto loader forks and thought it was stable. After ee turned to look back at the saw, ee placed hand on the next slab and the previous slab (on the loader forks) came sliding off and crushed ee's pinky finger between the two slabs.

Employee was operating a cutoff machine and was cutting stone that was 4 in. EE was holding the stone so that it would not move and was not paying attention to the machine. Saw lacerated right index finger.

Cleaning off paver stone and got stone dust/debris in eye (said was wearing safety glasses that was provided).

While stacking chopped stone onto a pallet, a piece of stone fell off the pallet and landed on EE's right foot causing a small contusion on the top of the foot.

When employee was picking up a rock to move it to the conveyor belt. Employee moved their upper body while their foot stayed in place which caused their knee to sprain.

Lifting a heavy stone while loading a single blade saw and twisted wrist and thumb joint on right hand.

Employee was catching stone and another stone fell on right foot hurting the two smaller toes.

Employee noticed a rash on abdomen on Friday the 29th and reported it to supervisor on Monday, August 1st as a rash or burn from being in the sun. We sent EE to the clinic and it was determined to be hives. We are still waiting for the clinic physician to notify us whether the hives were a result of heat from the sun or another source. EE was given RX topical cream.

Employee felt pain in EE's neck, right shoulder, and lower back from being on the wheel loader all day (during regular work hours).

Employee stepped on rubble from chopped stone and twisted EE's ankle.

Although employee was notified that he is required to wear steel toed boots at all times he removed them because they were bothering him. He hit his toe on a conduit tube embedded in the ground. He did not seek medical attention until 12/15, first aid was done, no RX was dispensed. Returned for further med. attention on 12/24 and RX meds were provided.

Climbing up ladder of KOMATSU LDR WA320-5L - Wheel Loader and felt a pain in his back.

Employee was moving a pallet with a Bobcat when the machine suddenly stopped which cause his body to move foward and he felt pain in his back. Machine safety checklist was performed by employee that morning.

Measuring rocks and fellow employee reversed the rocks and smashed the employees finger.

Strained his back while making pallets. Initially refused medical treatment, so no MSHA report was filed.

While stacking stone on a pallet employee felt pain in right shoulder.

Bending over to pick up stone while stacking pallets and back began to hurt

While catching chopped stone at the end of the chopper, a piece of rock hit his pinky finger on his right hand causing the nail to come off.

While cleaning slab at the Cobra saw with a broom a piece of rock flew into the employee's right eye. Employee states he was wearing his safety glasses at the time.

Tried to lift and flip over pallet and lost footing, fell forward into some rock and hurt right hand (swollen).

He was grabbing a rock from the chopper and another two rocks fell on his right hand finger.

Employee jumped off of forklift and felt pain in right ankle/foot.

Rock hit right hand index finger on table. Initially declined medical treatment offered to him. Did not seek medical treatment until 5/6/14 when finger kept hurting.

While hosing down the floor for the water saw ee slipped off step and fell.

While catching stone coming off the chopper, a piece started to fall and while grabbing it another piece came down and hit his left pinky finger. Caused a contusion to his finger tip.

Employee hit knee on a table while climbing up on it to take measurements. Human Resources and his Supervisor were not notified of the injury until 3/12/2014.

Hammering rock and hit his left thumb with the hammer.

Removing rock from chopper with hand (instead of hammer that should have been used and he was trained to use). Another rock came down and hit his hand as he was turning the rock to pick it up. Hit pinky finger/nail on left hand.

Employee was moving a piece of stone from the chopping machine and the stone broke and a piece of it fell and hit his right foot and leg.

Employee was breaking a piece of stone with a hammer and placing the broken pieces on a pallet. One piece slipped and fell onto his left leg/shin.

Employee was helping a colleague lift a block onto a table and started experiencing acute back pain about 10 minutes later.

Holding rock slab to be palletized and slap fell over and scraped his foot and sprained his back.

Moving stone

No unsafe conditions. Employee was throwing a rock into a metal bin and part of the rock slit off. This cause his ringer to become caught between the rock and the bin when he threw it.

Due to continued lifting of heavy material the employee's back started hurting, then he noticed a hernia on October 28th in his groin area. The condition was reported as workers comp to the insurance co, and was denied. However, as a result of a benefit dispute, the initial decision was reversed on 5/30/13 and is now being considered a work related injury.

While chiseling stone a small piece of rock flew up under his safety glasses and into his left eye. This did not bother him at the time and he was initially unaware until he got home that night and his eye was irritated. He notified the employer the next day, 10/5 and was sent to a clinic where he was given eye drops. Returned to work immediately with no further issues.

Was stacking stone on a pallet and caught his thumb in between two pieces of stone which resulted in a contusion of his left thumb.

He was measuring a rock on the chopper when someone pushed other rocks over causing his right hand to be caught between them. This resulted in a fracture to right wrist and bruised palm.

He was placing a piece of stone on the radial arm platform and the stone broke. One piece hit his knee bruising it and making it sore.

He was putting pieces of rock on top of a conveyor for the chopper and misjudged the placement of his finger and jammed between the rock and the metal siding of the device. He broke his pinky and had a laceration on the top of the finger which required stitches. He has not returned to work.

He was on top of the hyster loader cleaning it out and he slipped and fell landing on his feet. This resulted in a possible fracture in his right heel.

He was hammering a rock and his left thumb got caught between the hammer handle and the rock. This resulted in a contusion of his left thumb.

He was hammering a rock and the head of the hammer fell off which caused his hand to hit against the rock. This resulted in a cut on this right index finger.

He was walking to lunch break & had his lunch in one hand and water in the other and tripped and fell & cut arm on a sharp object. Had to have 8 stitches and returned to work the same day.

WAS UNLOADING STONE FINGER WAS CAUGHT BETWEEN 2 STONES SMASHED END REQUIRED 5 STITCHES.

DURING BREAK TIME HE WAS UP ON TOP OF A STONE. WALKED TO ONE END AND STONE TIPPED OVER, WITH HIM FALLING TO GROUND.

WENT UNDER MACHINE ON BACK SIDE OPPOSITE OPERATION PLACED HAND ON CHAIN, CHAIN WAS ENGAGED. GOT FINGER TIPS [2] CAUGHT IN SPROCKET.

WAS WORKING ON CHAIN CHOPPER ON FEED SIDE, HIS JOB IS TO KEEP STONE OING IN AND KEEP DEBREE OUT OF WAY. A ROD IS THERE TO USE TO MOVE SMALL MATERIAL. HE DID NOT USE IT AND GOT HIS HAND BETWE EN TWO PIECES OF METAL.

STONE FELL ON FINGERS & BROKE INDEX FINGER.

IRON RAIL ON STONE CUTTER CAME DOWN ON TOP OF STEEL TOED BOOT. BROKE BIG TOE AND RECEIVED STITCHES.
