EE was adjusting the tension on a conveyor belt. This is done by using a large wrench to turn an adjusting nut. There was an accumulation of water and mud at the end of the belt. The worker stood to the side so he would be out of the mud and adjusted the belt. He had to apply more force than usual, probably due to the awkward position. He felt a burning sensation in his back.

EE NOTICED RED MARK ON LEG C PRESSURED TO BE BUG BITE EE WAS GIVEN BENDARY BY HOSPITAL END OF SHIFT. SWELLING INGREDED AT WHICH POINT EE RETURNE TO HOPSITAL AND WAS ADMITTER.
OPERATOR WAS UNLOADING TRUCK. HE RAISED BOX TO FULLE HEIGHT. PART OF LOAD STUCK IN FRONT OF BOX. HE PULLED FORWARD WITH BOX FULLY RAISED. WEIGHT IN FRONT OF BOX PULLE TRUCK OVER. WET GROUND, A ND FAILURE TO BACK IN SO WHEELS WERE LEVEL FROM SIDE TO SIDE ON SLIGHT SLOPE WERE CONTRIBUTING FACTORS.
EMPLOYEE HAD PROPPED STEP LADDER AGAINST BOOM OF EXCAVATOR THAT WAS AT GARAGE FOR REPAIRS. LADDER SLIPPED ON MUDDY GROUND AND EMPLOYEE STRUCK SHOULDER ON BOOM AS HE FELL. DIAGNOSIS IS CONTUSIO N AND SWELLING. PLACED ON ANTI-INFLAMMATORY AND LIGHT DUTY.

EMPLOYEE REPORTED A RASH ON HIS ARMS. UPON SEEING HEALTH CARE FACILITY THE NEXT DAY, THE DIAGNOSIS WAS SIMPLE DERMATITIS. ALTHOUGH UNPROVEN, SUPPOSITION IS THAT EXPOSURE TO HEAT AND DIRT AT WO RK CAUSED CONDITION. EMPLOYEE RETURNED TO WORK ON HIS NEXT SCHEDULED WORK DAY.
INJURED PERSON SLIPPED & FELL ON ICE ON GROUND WHILE WALKING IN VEHICLE PARKING AREA.
EMPLOYEE WAS REMOVING OIL PAN FROM PAYHAULER WHEN PAN DROPPED AND PINCHED FINGER BETWEEN PAN AND OPEN TOP OF 55 GALLON DRUM BEING USED TO CATCH OIL.

WHILE CHECKING FLUID LEVELS OF PARKED DOZER, EMPLOYEE SLIPPED ON MUD ON TRACK AND FELL STRIKING RIGHT SHOULDER ON GROUND. X-RAYS SHOWED NO FRACTURE.
CLIMBING UP ON CONVEYOR TO CHANGE BELT [EQUIPMENT WAS SHUT DOWN] AND SLIPPED. FELL 3 - 4 FT AND HIT BACK ON I-BEAM NEAR FLOOR. PERSON WENT TO DOCTOR NEXT DAY AND WAS DIAGNOSED TO HAVE BRUISED BACK MUSCLE AND PLACED ON RESTRICTED DUTY.
