PINNER HAD PUT UP ONE ROW OF BOLTS WHEN A PIECE OF ROCK FELL OUT BETWEEN THE ROW OF BOLTS & RIB PINNING HIM AGAINST THE PINNER.
THE VICTIM WAS STANDING ON A 1 TON FLAT BED TRUCK WHICH HAD A PALLET OF BELT STRUCTURE ON IT. 3 MEN WERE TRYING TO PUSH THE PALLET OFF THE TRUCK. WHEN IT ROLLED OVER THE EDGE OF THE TRUCK, TH E VICTIM'S COAT WAS CAUGHT BY THE STRUCTURE AND PULLED HIM OFF THE TRUCK. HE FELL ON HIS LEFT ARM.

THE VICTIM WAS PERFORMING MAINTENANCE ON A MANTRIP IN AN INTERSECTION IN THE #7 ENTRY. THE SCOOP & OPERATOR WAS IN #8 ENTRY, COMING TOWARD #7, AND THE BUCKET STRUCK THE VICTIM ON THE RIGHT LEG . THE VICTIM WAS ON HIS KNEES WITH HIS BACK TO THE SCOOP.
THE VICTIM WAS TRYING TO PULL A 35 LB. BUCKET OF MINE SEALANT OUT OF THE MUD. HE SAID HE STRAINED HIS BACK. NO WITNESS!

CAME AROUND LT. TURN #7 ENTRY HIT RT. SIDE ENTRY WALL. THROWED EE INTO CONTROL LEVERS INSIDE DECK CATCHING HIM ON LT. SIDE RIBS.
THIS MECHANIC WAS CHANGING A HING PIN ON THE HEAD OF A CONTINUOUS MINER. HE WAS HOLDING A PUNCH WITH HIS LEFT HAND AND A HAMMER IN HIS RIGHT. HE MISSED THE PUNCGH AND HIT HIS HAND WITH THE HAM MER.

TWO BREAKS OUTBY SPAD 1349, DOWN ENTRY #8 OF THEMAINS. THE INTERSECTION FELL ABOVE ANCHORAGE. THE PLACE WAS BRAKERED AND DANGERED WHEN IT WAS FOUND.

MOVER WAS CARRYING BELT STRUCTURE AND DROPPED STRUCTURE ONTO HIS LITTLE FINGER.
MINERS WERE PULLING A 21SC SHUTTLE CAR WITH A 270 TRACTOR. THE CHAIN WAS UNHOOKED FROM SHUTTLE CAR AND THE TRACTOR TRAMMED AWAY. THE CHAIN ACCIDENTLY BECAME HUNG ON SHUTTLE CAR AND CAUSED THE SHUTTLE CAR TO PIVOT AND CATCH HIM BETWEEN THE SHUTTLE CAR AND THE COAL RIB.
