THE ROOF IN #5 RETURN AN COURSE FELL. THERE WER NO PERSONEL INJURED. THE CONDITIONS CONTRIBUTING TO THE ACCDIENTAL FALL OF THE ROOF WERE SMALL STREAKS OF COAL AND WATER IN THE SLIDE TOP.
I,NAME WAS HELPING LOAD RAIL ON THE MOTOR OUTSIDE THE MINE AND HAD LOADED & WHEN ON THE LAST RAIL HE CLAIMED HE HURT HIS BACK. I,NAME WAS BEING HEP ED BY THE WITNESSES.
ROOF FELL IN #4 ENTRY.DIDNT HINDER VENTILATION.NO PERSONAL INJURY SUSTAINED.CAUSE OF FALL WAS SMALL COAL SEAMS RUNNING THROUGH SHALE TOP.
I,NAME WAS TRAMMING HIS OFF STANDARD SHUTTLE CAR FROM THE FEEDER BACK TO THE FACE WHEN A SLAB OF DRAW ROCK FELL FROM IN BETWEEN THE BOLTS STRIKING HIM ON THE LOWER LEFT LEG BREAKING HIS ANKLE IN TWO PLACES.

MR. HINCHER WAS DRILLING A HOLE IN THE ROOF WITH A 36" DRILL ROD. THE DRILL ROD FELL FROM THE HOLE CATCHING, MR. HINCHER RING FINGER ON RIGHT HAND AND RESULTED IN BROKEN BONE. THE FINGER WAS C AUGHT BETWEEN THE ROOF DRILL HEAD AND THE ROOF DRILL ROD.
MR BRIDGES WHILE MOVING A FLGHT PUMP 1 1/2 HP PICKED THE PUMP UP AND STRAINED BACK NO LOST TIME ACCIDENT
MR PERRY HAD WENT UP THE LADDER ON TOP OF THE OFFICE TO GET A BOARD WHEN HE STARTED BACK DOWN THE LADDER SLIPPED OUT FROM UNDER HIM HE FELL TO THE FLOOR ON HIS LEFT SHOULDER
E FORWARD TO GIVE HIM EXTRA CABLE...WEIGHT FORCED EMPL DOWN.RECEIVED RUPTURE IN GROIN.
WHILE TRAVELING OUTSIDE FOR SHIFT CHANGE ON MANTRIP RAILMOTOR MR BEGLEY PUT HIS RIGHT LEG UP IN THE AIR OUT OF THE MANTRIP AND CAUGHT THE ROOF PULLING LIGTMANS AND MUSCLES IN HIS RIGHT L EG SAID HE WAS TRYING TO KNOCK A ICE CYCLE DOWN
GILLIAM WAS SHOVELING COAL FROM AROUND THE #3 BELT DRIVE. HE WAS SHOVELING AT THE PULLING HIS SHOVEL BETWEEN THE SPLICE IN THE BELT CAUGHT HIS SHOVEL THEREFORE PULLING HIS SHOVEL BETWEEN THE BRACE & BELT ROLLER & FORCING HIS LEFT HAND & ARM WITH IT. PULLING THE LEGIMENTS & MUSCLES IN HIS LEFT ARM.
