EMPLOYEE GOT OFF THE MINER AND DOWN ON THE RIB. WHEN A ROCK FELL CUTTING HIM OVER LEFT EYE.
PULLING A ROCK OUT OF THE FEEDER. MASHED THE RIGHT HAND.
EMPLOYEE WAS WATCHING THE MINER CABLE WHEN A PIECE OF THE ROOF FELL AND HIT HIS RIGHT HAND.

EE WAS GOING TO PULL ROCK OFF OF THE MINER WHEN HE PULLED HE HURT HIS BACK
EE WAS RUNNING THE LEFT DRIVE CAR AND HE RAN OVER A LOOSE ROOF BOLT. IT CAME UP AND HIT HIM IN THE EYE.
EE WAS RUNNING THE MINER WITH THE REMOTE BAY. WHEN HE FELL PUSHING THE LEVER AND THE MINER TURN AND HIT HIM.
WHILE CHANGING HOSE ON HEAD JACK PULLING OLD HOSE THROUGH, IT FLIPPED UP PUTTING HOT HYDRAULIC OIL IN LEFT EYE. THE END OF HOSE STRIKING HIM INLEFT EYE.
EE WAS RUNNING THE RIGHT DOVER CAR WHEN HE GOT HIS HEAD BETWEEN THE CAR AND THE TOP PULLING THE MUSCLE IN HIS BACK.

HE WAS GOING TO HANG THE MINER CABLE. WHEN HE PICKED UP THE CABLE, HE HURT HIS BACK.
THE MANTRIP WAS GOING UP #7 ENTRY AND WAS GOING TO CROSS OVER IN TO #6 ENTRY IN THE BREAK. THE MAN TRIP HIT HIT A ROCK PUTTING THE EE'S HEAD INTO THE TOP PULLING HIM OVER THE TRIP.
WHILE PUTTING 2" VENTRALIC WATER LINE TOGETHER STRUCK RIGHT THUMB WITH HAMMER.
WHILE REPAIRING CONVEYOR CHAIN CAUGHT LEFT RIGN FINGER IN BETWEEN FLIGHT AND FOOT SHAFT BUSTING THE END OF FINGER OPEN AND POSSIBLY NAIL CAME OFF.

WHEN PREPARING TO PLASTER A BRATTICE, EMPLOYEE SOPNED A BUCKET OF TNT PROSEAL PLASTER. AFTER OPENING THE BUCKET HE WENT TO SIT IT DOWN AND WHEN HE DID THE PLASTER SPLASHED UP OUT OF BUCKET COV ERING EMPLOYEE'S FACE AND GETTING INTO BOTH EYES
THE ROOF BOLTER PULLED UP IN #3 HEADING. EMPLOYEE SCALED A PIECE OF DRAW ROCK ON OPERATOR SIDE. HE STEPPED BACK TO RAISE ATRS AND A PIECE OF DRAW ROCK FELL OUT BETWEEN LAST 2 ROWS OF BOLTS STR IKING INJURED (OFFSIDE) PUSHING HIM BACK TURNINGHIS LOWER LEFT LEG.
A PIECE OF DRAW ROCK FELL OUT BETWEEN ROOF BOLTS STRIKING HIM ON RIGHT KNEE HE FININSHED THE SHIFT OUT WENT TO DOCTOR NEXT MORNING.
WHILE USING THE HAND HELD GRINDER THE GRINDING DISC BROKE CAUSING THE GRINDER TO KICK BACK INTO INJURED PERSON'S HAND.
WHILE USING THE HAND-HELD HYDRAULIC DRILL TO DRILL HOLES TO SET ANCHOR PINS, THE SEAL ON THE DRILL BUSTED, SHOOTING HYDRAULIC OIL ONTO EE'S LEFT SIDE OF HIS JAW AND FACE.
EE TOOK SCOOP TO SECTION FROM OUTSIDE WHEN HE GOT TO SECTION HE SAID HIS NECK WAS HURTING HIM. I ASKED HIM IF HE HIT HIS HEAD ANDHE SAID NO.
EE WAS GRINDING ON A PIECE OF METAL WHEN METAL WHEN IS LEFT EYE.
SUBJECT WAS BACKING A GOLF CART BACKWARDS UNDER LOW TOP. CAUGHT HIS HEAD ON ROOF PUSHING HIM FORWARD OVER THE CART STEERING WHEEL.
WE WORK LOW COAL. YOU HAVE TO KEEP YOU HEAD [...] OVER SO THE WOULD LET COAL FALL IN EAR.

ALLEGED ACCIDENT WAS NOT REPORTED TO ME. ONLY FOUND OUT SEVERAL DAYS LATER THAT THE EMP HAD HURTHIS BACK BUT NOT AS HOW IT HAPPENED.
UNKOWN BECAUSE IT WAS 6 DAYS LATER BEFORE THE EE NOTIFIED ME OF ANY ACCIDENT. REASON REPORT WAS NOT FILE EARLY WAS BECAUSE THE INVESTIGATION WAS STILL GOING ON.
WHILE AIRING UP A SCOOP TIRE, THE RIM EXPLODED AND IMBEDDED DIRT IN THE SIDE OF THE EYE BALL.
