The employee was transitioning from the top of a silo to the 135' manlift. He was tied off to the manlift climbing over the rail of the silo, his foot slipped and he caught himself with his right hand and his weight injured his right hand. He was taken to the clinic where he was x-rayed and referred to an orthopedic who determined he just bruised his hand.

EE was running wire when energized motor leads laying on the floor of the wire way were apparently sawed through wire insulation and caused a short and caused a arc flash. EE received burns to hands, arms, and face. EE was treated at hospital for burns release from hospital on 3/16/15.
Employee had lowered the wheel chock on the 980M loader and was placing it under the front of the back wheel when the momentum to place the chock in front of the tire when he felt a snap in his shoulder. This injury did not improve with first aid.

Employee was running the 980 Cat loader in the plant and came across a concrete slab in the yard. The slab halted the equipment immediately and caused a back injury in the operator. On 10/29/14 the employee requested to go to the doctor since the pain in his back was not improving. At this point the dr prescribed medicine and rehab to improve his condition.
Employee was removing a bolt from a plate without the proper PPE. He was not wearing protective gloves. The plate struck his hand when the bolt was removing causing a laceration between the index finger and thumb. Stitches were required and is estimated to take 5-10 days for skin to heal.
Intense rain events prior to the accident caused the ground to become saturated and extremely slick
While jack hammering a chunk through a side opening a chunk from above fell on the jackhammer bit causing the handle of the jackhammer to thrust up hitting the operator above the left eye causing a cut that required 4 stitches.
EE's foot slipped getting into the skid steer. The downward momentum caused him to lose his balance due to not having a full three points of contact. He reached for the roof of the equipment but was unable to grab firmly to the roof. He twisted around and fell to ground while his foot was behind the bucket. He broke his tibia and fibia in the fall.

Employee walked behind the loader without radio communication assuming the loader would take a path that would not put him in danger. Accident resulted in lower extremities suffering soft tissue trauma and sprained ankles.
