Changing roller on belt when finger was pinched between roller and beam that the guard bolts to. *This injury was originally deemed as a first aid injury and not reportable but has evolved into a reportable injury with medical treatment involved.
Employee was walking in a brush pile and slipped off of a log injuring EE's left knee.

EE was in Wash Plant Lab starting up the plant. EE blacked out and fell on the floor. Co-worker walked into Lab and tried to do chest compressions on EE. EE opened eyes. Co-worker called 911. EE is on several medications. EE was in the hospital for a couple days for observation. Cause was issue with medications.
Excavator was hoisting an electric motor in place to be bolted by injured employee. Excavator operator shirt sleeve accidentally bumped swing joy stick after EE released the hydraulic lockout lever causing motor to swing and hit employee against side of screen. Employee was conscious entire time. EMS was called and employee was conscious when taken by EMS to hospital.
