EE was working from extension ladder, when base of ladder slid back, causing EE and ladder to fall. EE received medical treatment on 2/22/11. this will be a lost time injury.

A cap from an oxygen cylinder fell off top floor of the plant fell 45' below and landed on the EE's right hand causing a laceration that required two stitches. The EE missed one day of work and returned to regular duty on his next sch. shift.
Employee was unstopping oversized rock chute. A big rock was lodge in chute, it dislodged, fell down the chute, bounced and struck employee in leg. Employee received medical treatment and lost 5 days.
