Employee was working in the open pit sorting stone and a large stone fell on top his right foot. Employee was wearing steel toe boots at the time of incident. The employee was sent to the doctor immediately after he reported to the office.

Employee was pulling a water pump up the hill and it slipped out of his hands. When he tried to grab it, he fell on his side and hurt his left shoulder. Employee was able to get up and walk into the office, his complaint was pain in left shoulder. He was then sent for medical exam.

Employee was at the Chris Stone Cutter machine and was pushing stone through the cutter, when his right hand slipped and his right forearm braised against the stone. This resulted in a 2" long cut to his right forarm.

Employee was picking up stones from stock pile when larger stone rolled down and impacted employee's leg. He suffered a bruise above his right knee.
EE was reclaiming stones from stock pile when a larger stone rolled down and smashed his finger.

Employee was tending to water pump in quarry pit when a side of the pit gave way (heavy rain storm). A piece of rock slit his right hand in the thumb area cutting a deep gash.
The operator of the forklift was approaching a pallet of stone. The injured EE was still in the process of wrapping a pallet of stone when the forks of the forklift pinned the injured employee's leg.

THE OPERATOR OF THE TELESCOPIC FORKLIFT WAS APPROACHING A METAL PAN THAT WAS FULL OF WASTE MATERIAL. THIS WORKER WAS ASSISTING IN CHANGING THE PAN TO THE FORK LIFT WHEN HIS RIGHT INDEX FINGER WAS CAUGHT BETWEEN THE FORKS AND THE PAN.
