The Middle left finger was pinched between half-inch piece of steel and a quarter-inch piece of steel. While putting a new guard together for the HP500 crusher. The injury resulted in needing stitches.

Miner was lifting one end of a 8" x 20' long piece of channel while the other end was on the ground. EE was handing the end to another miner in the man lift as to upright it and install as a conveyor leg. After lift EE felt a strain in EE's lower abdomen.

While walking to speak to the manager, slipped on the ice and fell on left knee.

EE and another employee were carrying a PVC pipe. When the pipe slipped it cut EE's left index finger requiring 7 stitches. EE missed no time from work.

Miner was wearing a welding helmet, walked under a steel structure and hit his head. Resulting in staples to the head. No lost time.

the miner operating a Genie S-85 manlift. While he was moving it from one location to another the front wheel fell into a washout in the roadway. Which caused him to fall in the basket injuring his left shin.

While cutting the head of a bolt with a grinder, when the bolt caught and was sent flying into his face/tooth

While operating loader, he came across some drill bits that he moved by hand, while moving them he felt a pain in his left hip.

Employee was welding, building a chute for the crusher when the metal started to get away from him, in his attempt to save it he pulled a muscle in his lower abdomen.

Employee was dragging a length of 2" sch40 steel pipe into the DOT shop to be welded. While employee was dragging the pipe into the shop he lost his footing and fell backwards. While falling backwards he put his right hand out to catch himself and sprained his wrist.

Employee states he was holding onto a piece of steel while another employee was welding it into place and although he turned his head, he reports "flash burns" to both eyes.

While placing a drill pipe through a drill table hole, right ring finger got caught between the drill & the pipe, crushed right ring finger.

EE WAS HOISTING UP A COUNTER WEIGHT WHEN HIS RIGHT HAND LITTLE FINGER GOT CAUGHT BETWEEN THE CHAIN.

EE WAS TIGHTENING A BOLT ON THE CRUSHER CHUTE, THE RATCHET SLIPPED AND HIT HIM ON THE FOREHEAD, CUTTING HIM AND REQUIRING 11 STITCHES.

WITNESS WAS CUTTING IN SHAKER WHILE INJURED WAS WIRE BRUCHING SIDE PLATES OUT SDIE OF SHAKER. A SPARK FLEW INTO INJURED'S LEFT.

EE WAS CHANGING A CONVEYOR BELT ON THE PLANT THE HELP OF A CRANE BETWEEN THE CABLES OF THE CRANE WHERE WAS A SPREADER. WHILE MOVING HE BELT THE SPREADER CAME OUT FROM BETWEEN THE CABLES FALLIN G C HITTING HIS RIGHT HAND.

EMPLOYEE WAS GRINDING ON AN OVERHEAD STRUCTURAL MEMBER WHEN AN AIRBORN PARTICLE FLEW UNDER HIS GRINDING SHIELD & BECAME LODGED IN HIS LEFT EYE. GRINDING SHIELD & SAFETY GLASSES WERE BEING USED AT THE TIME OF THE ACCIDENT.
