Drilling hole for roof bolt, drill steel broke and swung around hitting bolter behind ear causing a cut.
Installing roof bolt, aligning bolt head to chuck, spun motor to align and bolt head caught glove and tore end of small finger on left hand off to first knuckle.

Was bending 6 foot roof bolt.

EE was driving a johnson personal carrier out of the mine, when driving past a parked scoop he came in contact with the scoop and his left arm was injured between the two machines.
He was handling a piece of corrigated sheet metal in the muddy conditions when it cut him.
Was throwing cribs under miner to get unstuck when the crib block hit side of machine with his finger between the block and machine.
Hooking up a bolt trailer to the scoop, he pinched his hand between scoop & trailer tongue.
Miner's knee. I was not told of any injury date. I was just told knee was swollen and he was seeing his family physician, and not a panel doctor until 1/4/08 for one visit then back to his family physician. Didn't consider it to be work-related at this time

Handling low belt structure when thumb got smashed between a rail and bottom structure.
Gradual oncoming knee problem from crawling on knee surgery 11-14-2007.
A piece of rock fell out between bolts.
Backing miner out of right X-cut, stopped and was swinging miner over when left tram lever on remote stuck, causing miner to swing downhill. The auger of the miner rolled over EE's leg, causing a break.
