Miner tripped and fell, striking head on a handrail.

EE was cleaning up the haul roads and opened an existing weep hole in the berm. When EE opened the weep hole, hit a 16" natural gas line. When EE hit the gas line it stalled the engine on the dozer, and the pressure broke the glass out of the machine. The pressure was also throwing sand and gravel into the cab of the machine.

The miner was installing flight on a screw. EE was turning the screw by hand, when they pinched their finger between the v-belts and the pulley. Left index finger was cut and smashed.

A company mechanic was replacing the starter on a Cat 730 articulated haul truck. When he removed the bolts from the belly pan it fell smashed his left index finger. He had a laceration about 3 inches long on his finger. He was taken to urgent care where he received 9 stiches.

Miner was climbing down a ladder and twisted his knee on dismount.

Employee was hitting a Track Idler with a hammer to free it up. A piece of metal chipped off and went into his right Index Finger.

Employee was pounding a bolt thru plastic liner. A piece of plastic went into his eye. He was wearing Safety Glasses.

EE WAS CHANGING CUTTING EDGE ON 966G LOADER USING 3/4" IMPACT GUN. PUSHED BOLT UP INTO SLOT TO HOLD AND STARTED GUN. BOLT CAME OUT OF SLOT AND THE SHARP EDGE TURNED INTO EE'S RIGHT MIDDLE FINGER.

RAIN, WET, LOOSE GRAVEL, WHEN REMOVING ROCKS FROM GRIZZLY. SLIPPED ON GRAVEL & FELL ON GRIZZLY RAILS.

WHILE REPLACING A BELT ON HAZMAG CRUSHER, LEFT HAND WAS CAUGHT BETWEEN THE MAGNETIC SHIELD AND THE METAL SMASHING FOUR FINGERS.

A WASH HOSE HAD NOT BEEN PROPERLY DRAINED, ICE FORMED IN HOSE. WORKER WAS ATTEMPTING TO REMOVE ICE, HAD NOT CHECKED VALVE POSITION OR TURNED OFF. HE PICKED UP HOSE OFF GROUND, WAS LOOKING INTO HOSE, WHEN ICE CAME OUT HITTING HIM IN FACE.

EE WAS INSIDE MAINTENANCE SHOP. OPENED SLIDING DOOR WITH INSIDE HANDLE, WENT THROUGH OPEN DOOR. PULLED DOOR CLOSED WITH INSIDE HANDLE, WHILE STANDING OUTSIDE OF DOOR. FINGERS CAUGHT IN HANDLE, BREAKING 2 FINGERS.

EE WAS PUTTING AIR IN A FLAT TIRE ON WATER TRUCKHE HAD JUST CHECK THE PSI AND IT WAS 35# AND BEGAN PUTTINGMORE AIR IN TIRE WHEN IT BLEW HIS HARDHAT OFF HIS HEAD & IT WENT ABOUT 40-45 FT FROM H IM

MID TO LOW BACK PAIN, DO TO BAD SEAT IN TRUCK #296.

WHILE TAKING BOLTS OFF SKIRT BOARD ON EE WAS USING BOTH HANDS ON WRENCH WHEN IT SLIPPED COMING DOWN ON LEFT HAND & SMASHING LITTLE FINGER

WHEN RETURNING GREASE GUN TO SHOP AFTER GREASING DODGER TRIPPED OVER A LEG OF A METAL SAW HORSE AND WHEN HE TURNE DTO SEE WHAT HE TRIPPE DOVER FELL AGAINST A WELDER PARKED IN SHOP COMING DOWN ON LEFT KNEE HITTIN GHEAD ON WELDER AND SCRAPPING ARM.

OPERATOR OF CATEPILLAR D-25C OFF ROAD DUMP TRUCK FAILED TO MAINTAIN CONTROL AND SPEED, LEFT AHUYHAUL ROAD ROADWAY ON THE RIGHT SIDE OF ROAD. VEHICLE ENCOUNTERED ROUGH TERRAIN AND 3 INCH DITCH VEHICLE TRAVELD 200 FEET AFTER LEAVING ROADWAY BEFORE COMING TO REST. DRIVER SUSTAINED FACIAL LACERATIONS FROM CONTACT L SIDE DOOR WINDOWS AND BACK INJURIES.
