While bolting a rib with a jackleg drill the miner twisted his knee while pulling the drill out of drill hole. Third Dr visit showed a torn ACL on 11/11/14

Miner was helping another miner bolting a rib with a jack leg drill and when pulling drill out of hole he placed his finger in pinch point between leg and drill

Miner was lifting half of a 150 lb. deck grate and suffered a hernia on right side

Miner was drilling a hole in the rib and after completion of the hole leaned the drill against the rib and had his hand in a pinch point between the dog nut of the steel and the chuck of the drill.

Miner was drilling a raise round when his chuck tender broke on his jackleg drill causing the drill to come off the drill steel and the steel fell out of the hole striking his finger breaking it.

This is an injury or illness from repetition of work duties, not related to one instance, symptoms are numbness in hands and tingling of neck.

Miner was hanging utilities and slipped on wet ground, falling and landing on a jagged rock with his right forearm, causing a cut near the elbow, requiring 4 stitches.

Driller slipped on a small bit of grease and fell straining his knee.

While drilling with a jackleg, the jackleg was extended up faster than its penetration rate and broke off the steel, the hammer drill then struck the face of the Miner

EMPL TIGHTENING SIDE RODS ON 123 DRILL AND WRENCH SLIPPED OFF NUT HITTING HIM IN MOUTH CHIPPING SEVERAL TEETH.

EMPLOYEE WAS STAGING UP IN RAISE WHEN A ROCK APPROX. 150# FELL FROM HANGING WALL HITTING HIM IN LEFT ARM. LACERATION WAS DEEP, REQUIRING 10 PLUS STITCHES.

WHILE DRILLING RAISE ROUND ROCK CHIPPED FROM FACE & CUT ARM

ROCK HIT BUCKET WHILE MUCKING AND CHIP FLEW OFF HITTING MINER IN EYE.

EMPLOYEE WAS BARRING DOWN GETTING READY TO BOLT UP WHEN ROCK FELL FROM BACK HITTING HIM ON THUMB OF RIGHT HAND CAUSING CHIP FRACTURE.

WHILE DRILLING A RAISE ROUND EMP HIT IN CHIN AND SHOULDER WITH FALLING ROCK

MINER WAS DRILL3NG BOLT HOLES IN BACK AT THE 3820 LEVEL WHEN A SLAB HIT HIM IN THE LEFT FOREARM.

EMPLOYEE WAS LI0TING 5 GALLON BUCKET OF PARTS,FEET SLIPPED OUT FROM UNDER HIM, CAUSING PAIN TO LOWER BACK.

MAN WAS FASTENI3G SPADS TO RIB THE FASTENING DENSE CAUSED A PIECE OF ROCK TO RICOCHET

MINER WAS WATER3NG DOWN MUCK PILE AT BOTTOM OF SHAFT A ROCK FROM UNKNOWN ORIGIN HIT HIM IN BACK OF HEAD RESULTING LACERATION REQUIRED 5 STITCHES

EMPLOYEE A TOPL5NDER WAS LIFTING TIMBER AND RUPTURED DISC IN BACK.
