Smoke was reported in the developed underground space at 3:21am. The fire department responded but was unable to locate the source, although it was thought to be in the developed underground area. No miners were working underground. Mine management and Buckley Powder discovered the burned truck at approximately 8:30am and MSHA was notified at 8:36am.

Was working at the surface plant on rollers and conveyers with a wrench and pulled right shoulder causing tingling in index, thumb and middle finger.
Employee pulled away from crusher with his 35 ton truck bed in the air. The bed of the truck hit the ceiling of the mine and although the employee states that he was wearing his lap belt, he was thrown into the steering wheel and suffered lacerations and a broken nose.

Plant Operator was moving base stacker conveyor out of way for the Loader Operator. During this maneuver hydraulic hose broke. While searching for leak, either in hose or connections, he asked that it be raised up and pump turned on. In doing so pressure was reinstated in hose. Hand was positioned right over hole causing penetration to middle finger on right hand.

Changeling a punch plate on the second deck of scalping screen. EE stated he felt a pull in his lower back. Declined med attention at this time. Called supervisor at 5:30pm on 10/31/11 and stated his back was stiff and sore, but he would wait until morning and see how it felt. Called supervisor at 5:30 am on 11/1/11 and requested to see a doctor because his back still hurt.

Prying rock out of motor and guard on scalping screen. Pry bar slipped and mashed finger between bar and motor. Went to doctor, gave anti-inflamatory and finger splint.

EE was in a lift basket scaling the roof of the mine. He stated that he twisted while trying to pull down a piece of rock with a pry bar, and pulled a muscle in his neck/back area. EE has been prescribed muscle relaxers, anti-inflammatories and pain aid. He will not work until the Dr. has released him. Dr. will reevaluate in one week.
EE was in a lift basket with another EE when the lift basket was raised too high & the employee bumped his hard hat on the ceiling. Employee did not report discomfort until 5 days later & was sent to a physician. Minor diagnostic & medical attention was given & a follow up visit was scheduled. A 3rd visit was scheduled 3/31/09, thus making the injury reportable(a "Series")

Employee was scaling the roof of the mine when the mechanical scaler that he was operating came off of its' turntable and toppled over. He suffered bruises and contusions. He has been fully released as of today.

EE WAS REPAIRING A EUCLID R-22 DUMP BODY. THE DUMP BODY WAS RAISED AND SECURED. HE HAD FINISHED WELDING AND FLIPPED UP HIS WELDING MASK TO CHIP SLAG OFF OF HIS WELD WHEN HE FELT SOMETHING HIT HIS EYE. HE WAS WEARING SAFETY GLASSES. AT ABOUT 9:20 HE WENT TO THE RESTROOM AND COULD SEE SOMETHING IN HIS EYE, SO HE TOLD HIS PLANT MANAGER.
EE GHAD FUELED HIS LOADER AND WAS HANGING UP THE HOSE WHEN HE TWISTED HS BACK.

EE WAS STRAIGHTENING A COTTER PIN ON THE TAMROCK TRACK DRILL. HE WAS HOLDING THE PIN AGAISNT HE DRILL WITH HIS RIGHT HAND AND USING A HAMEMR WITH HIS LEFT. AS HE WA POUNDING ON THE PIN HIS F LASHLIGHT FELL TO THE GROUND CAUSING TO STRIKE HIS THUMB ON HIS RIGHT HAND WITH THE HAMMER.
EMPLOYEE WAS ATTEMTPING TO REMOVE ROCK FROM THE CRUSHER WHEN HE SPRAINED HIS LOWER BACK.
MAN WAS SUPERVISING CLEARING OFLARGE BOULDERS FROM PRIMARY CRUSHER USING A STEEL CABLE TO PULL THEM OUT OF THE CRUSHER HE WAS SIGNALLING THE TRUCK DRIVER TO PULL FORWARD WHEN THE CABLE PULLED THE STEEL SPILL PROTECTOR OFF OF THE ROCK BOX DUMP HOPPER & PINNED HIS RT ANKLE AGAINST THE GROUND

AN EQUIPMENT OPERATOR WAS SCALING WIATH THE GRADALL SCALER IN THE SOUTHWEST CORNER AREA OF THE MINE WHEN A ROCK THAT HE SCALED LOOSE ROLLED TO THE GRADALL & BECAME LODGED BETWEEN THE ACCESS LA DDER & FRONT WHEEL. THE OPERATOR ASKED A NEARBY EQUIPMENT OPERATOR TO HELP HIM DISLODGE THE ROCK & WHILE DOING SO THE MAN HELPING THE GRADALL OPERATOR STRAINED HIS BACK.

HE SAID HE HAD REMOVED A COUPLING FROM AJUMBO FRILL AND HAD BACKED UP FROM THE DRILL WHEN THE COUPLING CAME OFF AND OIL AND SLIVER OF METAL CAME FROM THE DRILL.SMALL METAL STEEL SHAVING CAUSED PUNCTURE WOUND TO THE RIGHT EYE.
HE WAS DOING MAINTENANCE ON COMB CURSHER HE JUMPED OFF FRAME OF COMB CRUSHER WHEN HE LANDED ON GROUND, HE SPRAINED RIGHT ANKLE.
HE WAS REPLACING WEAR PLATE IN CHUTE & DROPPED RATCHET & IT FELL UNDERNEATH GUARD. AFTER GRABBING IT HE PULLED HAND OUT LEFT HAND & CUT IT.
