HE SLIPPED AND HIT RIGHT ELBOW ON EQUIPMENT HE WAS EXITING. HE WAS UNSURE HOW THIS HAPPENED. THE EQUIPMENT IS EQUIPPED WITH HANDRAILS AND SAFETY STEPS. RIGHT ELBOW WAS BRUISED. HE SAID HE SLIP PED. THERE WERE NO OTHER WITNESSES TO THIS OCCURRANCE.
JUST A SORE BACK CAUSED BY CRAWLING IN AND OUT AND UNDER EQUIPMENT AND LIFTING HEAVY OBJECT.

INJURED WAS CLEANING WINDSHIELD. WHILE DISMOUNTING CAUGHT HIS WEDDING RING AND IT CUT INTO HIS RING FINGER.
INJURED WAS ON A CLEANUP DETAIL AND WHILE CLEANING AROUND THE FEED HOPPER GOT A PIECE OF SAND OR SMALL ROCK INTO HIS LEFT EYE.
HIT HOLE WITH LOADER TIRE. BOUNCED AROUND IN SEAT.
EMPLOYEE DID NOT UNDERSTAND HOW TO REGULATE THE PAI MIXTURE. AIR MIXTURE WAS WRONG AND HEATER STARTED TO EMITT DIESEL SMOKE WHICH EMPLOYEE BREIFLY INHALED AND MADE HIM SICK.
