TRUCK DRIVER WAS ATTEMPTING TO DUMP COAL INTO A COAL BIN, WHEN THE DUMP TRAILER SHIFTED WHILE IT WAS BEING RAISE, CAUSING THE TRACTOR AND TRAILER TO ROLL ONTO ITS RIGHT SIDE. THE VEHICLE IS OW NED BY GMP TRUCKING SERVICE.
EE WAS WELDING ON A BOAT RUBDDER. THE RUDDER NEEDED TO BE TURNED OVER. OTHER EE WAS HELPING TO LIFT THE RUDDER WHEN I FELT A PULL IN MY LOWER BACK.

INJURED EE AND SUPERINTENDANT WERE TOSSING HEAVING CHAIN LINKS FROM AN ELEVATED BANK ONTO A TOW BOAT. THE SUPERINTENET LET GOT OF HIS END OF THE CHAIN EARLY CAUSING THE EE TO BE PULLED HEAD FI RST OVER THE EMBANKMENT. THE EE STRUCK HIS HEAD ON THE BOW OF THE TOW BOAT.
EMPLOYEE WAS IN THE BIN CLEANING OFF THE FEADER.AFTER CLEANING THE FEEDER HE WAS ABOUT TO GET OFF WHEN PARTLY FROZEN COAL SLIPPED OFF THE WALLS OF THE BIN AND COVERED HIM TO HIS NECK.

EMPLOYEE WAS CHANGING BUCKETS ON AN 88B CRANE. HE TRIED TO LIFT TOP PLATE TO RELEASE THE TOP BOLT WHILE HE WAS PULLING ON A BAR. HE FELT PAIN IN HIS RIGHT ELBOW, WHICH HAD PREVISOULY BEEN INJU RED IN MAY 1994. HE DID NOT RECEIVE MEDICAL ATTENTION AT THE TIME OF THE INJURY. HE WENT TO A PHYSICIAN ON 12/8/94 WHO ADVISED HIM TO GO TO THERAPHY TREATMENTS FOR 4 WEEKS.
EMPLOYEE WASW OPENING CRANE DOOR, THE DOOR JAMMED. THE CRANE OPERTOR TRIED TO CLOSE THE DOOR RESULTING IN RIGHT ARM(ELBOW AREA) BEING STRAINED (POSSIBLE TENDON DAMAGE). THE CRANE OPERATOR WA S TAKEN TO A LOCAL HOSPITAL FOR TREATMENT BY THE TIPPLE SUPERVISOR.
EMPLOYEE FELL TRYING TO REMOVE A PIECE OF STEEL FROM STORAGE RACK THE EMPLOYEE FELL A SECOND TIME ON THE SAME SHIFT WHEN HIS LEG WENT THROUGH GRATING WHILE WORKING ON REFUSE BELT
AN EMPLOYEE OF ATOM ARC WELDING WAS MOVING A 8' X 10 1/2" X 1/2" METAL PLATE WITH A COME-A-LONG. THE PLATE WAS RESTING ON A HANDRAIL AND SLIPPED WHEN THE CONTRACTOR BEGAN CRANKING THE COME-A-L ONG. THE PLATE SWUNG INTO HIS LEFT SIDE (LOWER STOMACH AREA). A COAL COMPANY EMPLOYEE OFFERED TO TAKE HIM TO THE HOSPITAL BUT HE REFUSED AND LEFT THE JOB SITE.

EMPLOYEE ALSO STATED THAT HE WELDED BRACES ON THE STAKER CONVEYOR. DR'S REPORTT STATED THAT THE CAUSE OF ILLNESS WAS UNKNOWN.
