Miner was descending down the stairs of 992 front end loader when they slipped on steps slid down 2 steps but remained upright never falling. The steps were wet from the rain and the miner strained their left lower back.

Miner was shoveling a deck hit when the shovel hit an uneven surface causing his right arm to be pushed back. Miner has right forearm and elbow strain.
The haul road was slick due to rain. The haul truck lost control and rolled over on right side. The operator was knocked out for 2 to 3 minutes at time of incident. After further exam at clinic is right wrist was sprained.

Employee failed to follow Company procedures for safe access when entering belt press area. Employee removed a removable catwalk (only used when replacing the gear box is necessary. The gear box is lowered through the opening using a crane) and proceeded to climb down framework to catwalk 5 ft below. While climbing down, employee slipped and fell approx 3 feet.

Injury was standing approximately 6' from a 400 horsepower motor to a cone crusher when a mechanical failure caused the fan to come apart inside the fan housing which in turn exploded and one of the many pieces of the fan assembly struck him in his right leg 6" above his ankle thus breaking his leg

While pulling wire through a conduit pipe on a 500' run with a p/u truck, the rope we used to pull the electrical wire with was tied in a square knot around a 6" C-clamp which was attached through one of the loops on the trailer hitch assembly. The clamp twisted off the hitch assembly & hit the employee above the right knee on the side of his leg leaving a very small cut.

EE was removing a return roller and when he rolled it out of the bracket he got his middle finger on his left hand slightly smashed between the roller and the roller bracket. We weren't even aware he went to the clinic until he turned paperwork in 3 weeks later.

The employee was working with another EE off a man-lift without a safety harness. The man-lift needed to be moved & he elected to stay on the belt without being strapped in, and at some point got impatient & he took it upon himself to walk down the belt leading to the belt press and lost his balance and falling approximately 16' breaking a bone above his right ankle.
