Operator of a 777F haul truck drifted into haul road berm, momentum and inertia of truck carried over into an impoundment. Operator sustained minor injuries, but taken to hospital to error on the side of caution and released from hospital after observation.

Miner was in a boom lift with another miner who was operating it, miner was holding on the outside railing with right hand when the lift basket bumped into the the crusher they were working on and pinched right thumb.

Employee was using a porta-power to spread the arms on a 966F loader to replace the bucket attachment. After releasing the pressure, the porta-power became wedged beween the dog bone and the lift arms. Employee used a sledge hammer to try and dislodge the porta-power. Upon striking the porta-power, it dislodged, traveled upward and struck him in the chest

While driving on the 60 freeway west in Riverside Cty a Robertson's Transport truck driver began to feel chest pains and continued to drive to a familiar location to seek aid. The driver pulled into the Banning quarry and requested medical attention. 911 was called and the driver was transported to an area hospital.

Loader rolled down an embankment about 40' several minutes after the operator set the hydraulic brakes, the loader had the forklift assembly attached and it pinned the employee between the forks and the guard assembly surrounding the primary incline belt gravity take-up, EE sustained cuts to his right middle finger and right leg with a possible broken left ankle.
Inj employee was working with an electrician they were having trouble shutting the power off a particular electrical service, so inj ee continued to try and disconnect the power and in that process there was an arch of electricity that caught ee's left arm and jacket on fire, he received a serious flash burn to the face and his left arm.
Injured employee received smoke inhalation from the incident when another employee caught fire from an electrical arch.

WHILE EE AND ANOTHER EE WERE BUILDING A HOPPER THE 2 SIDE PANELS WERE BEING HELD BY TACK WELDS WHICH BROKE WHEN THE PANELS WERE BEING ALIGNED CAUSING ONE PANEL TO FALL ONTO EE.KICKERS WERE ALS O IN PLACE.

EMPLOYEE WAS TRYING TO REMOVE A ROCK THAT WAS LAYING BETWEEN A HEADPULLEY AND DEFLECTOR CHUTE WHILE THE CONVEYOR WAS RUNNING WITH A PICE OF METAL AND A ROCK CAME OFF THE CONVEYOR AND PUSHED HI S FINGER BETWEEN THE PULLEY AND DEFLECTOR PLATE.
EMPLOYEE WAS CLOSING HTE GATE ON THE 3/4 ROCK, BUNKER AND A SMALL ROCK CHIP BOUNCED OFF THE TRUCK AND GOT IN HIS EYE.

EMPLOYEE WAS DISMOUNTING FROM A BUNKER TRUCK, SLIPPED ON RUNNING BOARD AND FELL AGAINST RUNNING BOARD. EMPLOYEE FELT SHE MAY HAVE HAD SOME OIL OR GREASE ON HER SHOES FROM EARLIER IN THE DAY.
EMPLOYEE WAS OPERATING A LOADER. HE WAS OBSERVED BY FOREMAN TO BE OPERATING EQUIPMENT ERRATICALLY, WAS RELIEVED BECAUSE OF EXTREME FATIGUE AND TOLD TO CLOCK OUT. EMPLOYEE BECAME INCREASINGLY D ISORIENTED, WAS DETAINED AND MEDICAL ASSISTANCE SUMMONED. EMPLOYEE DIAGNOSED AS HAVING A STROKE, WAS COMATOSE AT HOSPITAL. INJURY NOT CLEARLY JOB-CONNECTED.
EMPLOYEE WAS USING LEVER TO OPERATE BUNCKER GATE. WHILE PULLING BACK ON LEVER, SLIPPED ON LOOSE GRAVE, FALLING BACK AGAINST PROTRUDING BOLT. INJURY WAS DIAGNOSED AS CONTUSION TO LOWER BACK, SP RAIN TO LEFT SHOULDER.

THE LOCKOUT WAS TAKEN OFF 5ND THE CONVEYOR WAS ACTIVATED. HE CLIMBED BACK UP AND WAS ON THE BELT WHEN IT STARTED. HE JUMPED TO THE PLATFORM AND STRAINED HIS LOWER LIMBAR.
