Employee alleges he was rolling a piece of screen cloth out on a storage rack and felt a pull in his back. He was diagnosed with a muscle strain, but after no improvement was sent for an MRI which revealed a herniation of a disc.

Illness related to silicosis.

EE recently underwent a medical evaluation for a follow up on a chest x-ray. The results of the medical evaluation show signs of silica exposure or silicosis. This information was received 2/1/08.

Employee performing maintenance on a 980C front-end loader. While removing the loader teeth pins he struck one of the teeth with a hammer, a piece of metal sheared off into the right side of his stomach. the clinic removed the metal, 8 stitches were required.

Employee was working on the 980C loader. While reaching for the open end of the wrench, the wrench stabbed his left hand between the thumb and index finger resulting in a deep cut. Three stitches were required.

Employee was working on the Kaw Loader - tapping ujoint into place when a piece of metal broke off the hammer and went into his pointer finger on the left hand. Metal was removed at the clinic and one stitch was required.

EE was working on the wash tower, holding a screen. While he was holding the screen cloth, it slipped off I beam and fell about a foot and 1/2. When the screen slipped, it struck the EE's right hand resulting in a cut to his ring finger on his right hand. This injury required 6 stitches.

EE was being driven to his machine after break. The driver went over a bump in the road and the injured EE began to hurt in his lower back. He was taken directly to the hospital.

EE WAS GRINDING AT THE BACK OF THE TRUCK WHEN THE GRINDING DISK STRUCK THE SIDE OF THE BED. THE GRINDER KICKED OUT AND CUT HIS LEFT WRIST. * REQUIRING NINE STITCHES THIS IS A REPORTABLE MEDICAL.*

Two employees were setting the scale using a jack to hold up a previously set piece when the crane bumped that piece causing the jack to fall out and our employee smashed three fingers.

TRUCK WAS IN THE PIT. AFTER BEING LOADED IT BACKED UP AND SOMEHOW FELL ON ITS PASSENGER SIDE. EE IS EXPERIENCING DISCOMFORT IN HIS KNEE. DOCTOR WROTE HIM OUT OF WORK EFFECTIVE DEC 22 AND IS GO ING TO DO EXPLORATORY SURGERY 0N DEC 31, 2003.

EE STATES HE WAS UNLOADING TRUCK AT THE QUARRY. THE LOAD SHIFTED AND THE BRACING GAVE WAY, HITTING HIM IN THE FACE, SIDE AND ARM. SPRAINED WRIST, CUT TO THE FACE.

EE WAS ON THE BACK OF THE FUEL-LUBE TRUCK. AS HE WAS STEPPING OVER A SHOVEL HANDLE AND A BUCKET, PART OF THE HOSE REEL JABBED INTO HIS LEG.

WHILE REPLACING A SCREEN IN THE WASH TOWER EE CUT HIS HAND. IT SCRAPED A BOLT THAT WAS PROTRU-DING OFF A SPRAY BAR. HIS GLOVE WAS TORN & HIS LEFT HAND WAS CUT.

EE WAS GETTING OUT OF HIS TRUCK. HIS FOOT GOT STUCK BETWEEN THE LAST STEP AND BUMPER. HE THEN FELL BACKWARDS AND TWISTED HIS KNEE (RIGHT).

EE WAS JARRED WHEN ROCK WAS DUMPED INTO THE BED OF HIS TRUCK. THE DOCTOR SAYS HE HAS STRAINED MUSCLE. THE DOCTOR EXPECTS FULL RECOVERY.

EMPLOYEE WAS OUTSIDE OF THE SHOP LIFTING DRILL STEEL AND STRAINED HIS BICEP MUSCLE.

EE WAS CLOSING DOOR ON 30B CRANE WHEN HIS LEFT MIDDLE FINGER BECAME CAUGHT BETWEEN LEFT DOOR JAM AND DOOR CAUSING FINGER INJURY

EE WAS CLIMBING OFF OF SCREEN. HE STEPPED ON WATER MANIFOLD & STARTED TO TURN WHEN HE LOST HIS BALANCE & FELL ABOUT 47" INTO THE HANDRAIL AROUND THE SCREEN PLATFORM

EE STEPPED DOWN BACKWARDS OFF DRILL STEP WITHOUT LOOKING ONTO UNEVEN AREA OF GROUND, TWISTING HIS LEFT KNEE AS HE PUT HIS WEIGHT ON IT.

EMPLOYEE WAS ON HIS KNEES CUTTING A CONVEYOR BELT WITH A UTILITY KNIFE. HE WAS PULLING THE KNIFE TOWARD HIM, WHEN IT CAME OUT AND CUT HIS LEG.

EMPLOYEE HAD HANDS FULL WHEN HE WAS TRYING TO CLIB DOWN LADDER, AND LOST BALANCE. INSTEAD OF FALLING DOWN LADDER, HE JUMPED OFF LADDER AND HWEN HE LANDED, HIS RIGHT FOOT LANDED ON A ROCK TIWS TED HIS ANKLE.

EMPLOYEE WAS TRYING TO HOOK CHAIN TO TOP OF WATER TANK LOCATED ON FLAT BED TRAILER, AND STEPPED IN HOLE IN FLOOR OF TRAILER CREATED BY MISSING BOARD. EMPLOYEE FELL (LEFT LEG) UNTIL A STEEL ANG LE BROKE HIS FALL INJURING HIS KNEE.

EMPLOYEE WAS ATTEMPTING TO CLEAN SOCKET SEALING RING & HIS WRIST CAME INTO CONTACT WITH THE OUTSIDE EDGE OF RING & WAS CUT, ON HIS RIGHT WRIST.

DUE TO OBSTRUCTION, THE WRENCH SOCKET DID NOT COVER NUT PROPERLY. THIS CAUSED THE WRENCH TO SLIP WHEN EMPLOYEE APPLIED FORCE.
