Employee was standing on a temporary platform just below a jaw crusher attempting to remove two toggle seat wedges from the pitman. Shortly after removing the two bolts holding the wedges in place, the pitman unexpectedly moved fatally crushing the employee.

EE was operating a 2004 Volvo 220 front end loader clearing snow off the haul road when he heard a loud pop. EE noted smoke & flames were coming from under the engine cowling. EE dropped bucket, set brake, turned machine off & exited the vehicle. After briefly trying to extinguish flames with hand held extinguisher, the fire dept was called. They extinguished fire. No injuries.
Employee was grinding in a manlift. Protective eye wear slipped down and he got a small splinter in the eye.
Employee had sand in eye resulting in scratched cornea. Employee unsure when sand got in eye, it may have been during clean up around the wash plant or having rubbed his eye with a dirty glove.

Employee was swinging a 12 pound hammer to remove the main shaft from a water pump when he recieved a partial tear of his right biceps tendon.
While using a hole punch to punch holes in a piece of rubber, employee hit finger with hammer, requiring stiches.
Employee strained tendon in his left leg as he was working on changing a conveyor belt.
Worker was lifting steel door into the syntron of the pit run tunnel when he strained his back. Worker is receiving therapy and on prescription medication.

WHILE CHECKING UNDER THE HOPPER FEED BELT, HE HIT HIS LEFT SHIN ON A SHAPE CORNER OF A GUARD.
WHILE WELDING A PIECE OF ANGLE IRON HIS LEFT ARM SHIRT SLEEVE CAUGHT ON FIRE AND BURNED HIS ARM. THE BURN IS PROBABLY AND INCH IN DIAMETER.

EE WAS REMOVING METAL BANDING ON HAND RAIL OF TUNNEL CONVEYOR WITH A CRESENT WRENCH. THE WRENCH SLIPPED AND THE BANDING CUT HIS LEFT HAND INDEX FINGER REQUIRING 4 STITCHES AND A TETNUS SHOT. A PROPER CUTTING TOOL AND GLOVES WOULD HAVE PREVENTED THE INJURY.
THE EE JUMPED OFF OF A FLAT BED WORK TRUCK WHEN HIS FEET HAD THE GROUND IT SPRAINED HIS LEFT KNEE.

WHILE LEAVING THE OPERATORS CAB INJURED STEPPED ON THE OUTSIDE TRACK AND SLIPPED AND FELL TO THEGROUND.

HE WAS WALKING ON A CATWALK NEXT TO A SHAKER SCRREN. THE WALKWAY MUST OF MOVED BECAUSE OF THE VIBRATION OF THE SHAKER SCREEN. HE STEPPED ON THE CORNER AND IT GAVE WAY. HE FELL ON THE SAME WALK WAY SCRATCHING HIS LEG.

EMPLOYEE HAD COMPLTED A SMOOTHING OPERATION WITH A HAND HELD GRINDER. HIS TARK WAS TO REMOVE THE EXCESS SLAG FROM A WELD IN THE FABRICATION OF A CONVEYOR SYSTEMS. IN RESTING HIS TOOL (9" GRIND ER) HE SIMPLY LET THE TOOL HIT HIS LEFT KNEE RESULTING IN A LACERATION OF WHICH HE RECEIVED STITCHES TO CLOSE THE WOUND.
