WHILE CHANGING SCREENS A TOOL FELL FROM AN EMPLOYEE'S GRIP AND HIT ANOTHER EMPLOYEE IN THE MOUTH. THE LIP LACERATION REQUIRED STITCHES.

EE tripped on concrete pad while loading product for a customer and landed on left knee and hand. Swelling and pain in left hand and knee.

EE was cleaning up yard debris when a gust of wind blew particles into eyes that EE was not able to get out with an eyewash. EE went to an eye doctor who was able to remove metal particles.

Employee was in one of the plant's screenhouses in order to change a wearplate. The grating had been removed over the opening to access the wearplate. The employee misjudged a step and grabbed the handrail to prevent from falling into the opening. In so doing, they felt a strain in the left shoulder.

EE was trying to prevent bucket elevator from getting jammed up by opening oversized metal door in plant. The door slammed against their right ring finger causing a deep laceration.

Miner was lifting a 50# gear reducer up onto a shaft that was in front of EE. While lifting EE heard and felt a snap in EE's right forearm in the muscle. Amount of damage is undetermined at this time.

Miner has a standard threshold shift in the right ear. Found when annual autograph. Shift could be deterioration work related for results off duty.

While performing repairs on a conveyor belt, employee injured left knee.

HEARING LOSS AT SOME HIGH FREQUENCIES PER ANNUAL HEARING TESTING REPORT.

Employee injured finger while working on corrugated tin.

Injured worker was removing pipe from the ceiling and he felt sharp pain in right side of neck and upper shoulder.

Employee was doing repairs to 980 loader, lost balance and fell.

Pulled muscles in left hand while moving scissor lift.

While employee was shoveling mud out of the turbulator in the pellet plant, he felt something pull in his shoulder.

The continuous operation of the bucket loader has caused strain to lower back and pain down his right leg. Also, has caused numbness to right foot.

Employee lifted bin vibrator off shelf to place on cart.

Employee was lifting 50 lb. bags of nitrate.

REPETITIVE MOTION USING HANDS TO OPERATE MACHINERY IN OUR PELLETIZING PLANT.

EE LIFTING ROCK OUT OF CRUSHER, STRAINED AND TORE MUSCLES CAUSING HERNIA.

ICY STEPS

EMPLOYEE BANGED LEFT KNEE WHILE REPLACING BOLTS ON THE MAIN FEEDER.

EE WAS SERVICING WATER PUMP HE GOT OFF HIS TRACTOR AND IN THE PROCESS KNOCKED IT OUT OF PARK. THE TRACTOR STARTED TO ROLL & EE DRIPPED GETTING OFF AND AS A RESULT FELL UNDER THE REAR WHEEL

EMPLOYEE WAS DRILLING IN THE QUARRY WHEN THE DRILL COUPLING CAME DOWN AND HIT HIM ON THE LEFT SHOULDER.

DRIVER WAS GETTING ON FORK TRUCK WHEN HE HIT THE SIDE OF HIS HEAD ON THE ROLL CAGE. THIS RESULTED IN PERIODS WHEN HE FELT OK UNTIL 9-27-94. AT THIS POINT HE WENT TO THE DR AND HE GAVE HIM SOME MEDICATION ORDERED X RAYS AND TOLD HIM TO STAY HOME FOR A COUPLE OF WEEKS.

DRILLER WAS EXITING TRACTOR HE WAS USING TO GO TO AND FROM JOB SITE. HE SLIPPED ON THE STEP AND FELL TO THE GROUND, LANDING WITH ARM EXTENDED, ON HIS SHOULDER AND ARM.

INSTALLING TIN ON THE SIDE OF BUILDING, TIN SLIPPED SLIGHTLY CUTTING THE RIGHT INDEX FINGER ON EMPLOYEE.

EMPLOYEE CLAIMS HE SLIPPED WHEN GETTING OUT OF BIN TRUCK AND BANGED HIS NOSE ON THE STEERING WHEEL.

EMPLOYEE CLAIMS WHEN HE WAS UNPLUGGING PLANT HE LIFTED A LARGE STONE AND PULLED SOMETHING IN RIGHT GROIN AREA.

STEEL ROD PUSHED HAND AGAINST STEEL PLATE OF CRUSHER PLATE WHEN STONES LET LOOSE CAUSING HIM TO SMASH LEFT HAND.

EMPL CLAIMS HE SHLIPPED ON LOOSESTONES CAUSING HIM TO FALL & BANG LEFT ELBOW WHILE WALKING TOWARDS DRILL.

DRILLER WAS TRYING TO UNPLUG A STEEL WHILE IT WAS STILL ATTACHED TO HAMMER HE WAS ROTATING AT BACK AND FORTH A HAMMERING IT AT THE SAME TIME. THE STEEL WAS NOT IN THE CENTROLIZER. THE DRILL TOUCHED HIS WET MITTEN TO THE COLD STEEL AND IT FROZE THERE TWISTING HIS ARM INTO THE STEEL WITH HIS FREE ARM HE STOPPED THE ROTATION AND FREED HIMSELF.
