On 3/18/08 the leadman drove onto the quarry ledge to pickup the ee who had been operating a drill. When the ee exited the drill he stepped in a large crack falling up to his waist. The ee stated he was okay. On 5/09/08 the ee called his supervisor at 2:45PM stating that when he woke up that morning his knee was swollen.

EE WAS HAULING ROCK TO PLANT WHEN HIS TRUCK BLEW A HOSE AND THREW OIL ALL OVER HIS WALKING DECK, IT ALSO CAUGHT FIRE AND THE EE WAS PUTTING THE FIRE OUT WITH AN EXTINGUISHER WHEN HE SLIPPED IN THE OIL AND FELL ON HIS KNEES.

EE WAS INSTALLING CABLE UNDER OFFICE TRAILER-SLIPPED OFF ICEY STEPS OF TRAILER & FELL ON HARD FROZEN GROUND-DIRECTLY ON LEFT SHOULDER TEARING ROTATOR CUFF REQUIRING PHYSICAL THERAPY.
