St Joseph Plant #8 Metal/Non-Metal

Affected employee was standing in the middle of an empty barge preparing to tie it up when the barge bumped the dock barge causing the employee to lose his footing and fall onto his right shoulder.

Employee was replacing a guard on the conveyor after repairs were made. The guard slipped and smashed the thumb on the left hand. Sutures were required to close a cut, however, there are no fractures.

Employee was exiting his vehicle when he slipped on the ice and fell. He injured his right elbow and right lower back. Contusion of the right elbow.

This accident was not reportable until now. Employee strained his shoulder in November and he continued on regular duty as his shoulder got better. File was closed without medical, however, now he is complaining of it hurting again and they suspect nerve impingement. An MRI is scheduled and therapy. This is now a reportable accident.

Employee was working on jig, he slipped on the ladder going into the jig and sprained his back. this was not MSHA reportable at first, however, they have determined he needs therapy. He is still doing his regular job.

Employee suffered heat exposure on his last day of work. He was hired as summer help before he went to college. Employee was dehydrated and hospital had to rehydrate with IV's. He is now fine but will not be coming back to work as it was his last day.

Employee was tying a barge, he slipped and hit his right leg causing a bruise. this just now became reportable, because on 4/28/06, the bruise became infected. He is taking antibiotics and has a wrap on his leg. He is doing his regular job and has had no lost time.

Fell backwards while descending loader. Employee caught himself and strained his left shoulder and lower back. This just became MSHA reportable because the clinic decided to send employee to therapy for two weeks. He is on regular duty and was never on restricted duty.

Employee was coming down from his highloader missed the last step and fell to the ground. He landed on his right hip and immediately started having back pain.

EE CLAIMS RUNNING LOADER HAS HURT HIS NECK.

EMPLOYEE WAS PULLING CABLES AND STRAINED HIS RIGHT SHOULDER.

EMPLOYEE WAS CHANGING A RETURN ROLLER. HE WAS STRETCHING AND LEANING AGAINST A PIECE OF ANGLE IRON. HE FELT A PINCH. THIS RESULTED IN A CONTUSION OF THE VEIN.

EE WAS WALKING FROM THE DREDGE. HE STEPPED IN A HOLE AND TWISTED HIS LEFT KNEE.

EE SAID HIS ARM AND NECK HURT. HIS ARM HAS BEEN GOING NUMB. HE CLAIMS IT IS BECAUSE HE RUNS A HIGHLOADER.

PUTTING GUARD ON BIG PUMP. GUARD SLIPPED AND FELL, PINCHED LITTLE FINGER ON RIGHT HAND IN BETWEEN OTHER GUARD

SHOT LEFT INDEX FINGER WITH AIR NAIL GUN.

WELDING BOX TUBING AND PIECE OF METAL FELL INTO BOOT BURNING HIS ANKLE.

EMP WAS CLIMBING OUT OF THE HULL ON THE BOAT AND HE SLIPPED ON THE DECK AND FELL HITTING HIS SHIN ON ANGLE IRON.

HE WAS JUMP STARTING THE WENCH & THE BATTERY EXPLODED

WHILE RETRACTING FUEL HOSE FROM FUEL BARGE, HE LOST HIS FOOTING HALF WAY FROM BANK DUE TO LOOSE GRAVEL AND WET CONDITIONS. SLIPPED BACKWARD 10 TO 15 FEET TO EDGE OF WATER.

WHILE MOVING DOCK BARGE THE CABLE HIT HANDRAIL UPRIGHT ON DOCKBARGE THE UPRIGHT BROKE WHEN CABLE HIT IT AND THE CABLE HIT EE.

WALKING ON BARGE CHECKING EQUIPMENT-SLIPPED AND FELL AND HIT HEAD.

CARRYING A BEARING WHEN BACK DISLOCATED

ON 1-20 PATIENT WENT TO DR AND WAS DIAGNOSED AS FLASH TO EYE FOLLOWUP ON 1-27 THE DR DISCOVERED A PIECE OF METAL IN EYE

CHEATER PIPE SLIPPED ON RACHET PINCHING END OF RIGHT THUMB WITH LACERATION OF RIGHT THUMB.

CLIMBING OUT OF BARGE AND HIT HEAD ON HATCH.

PULLED ON EXTENSION CORD AND THE END FLIPPED BACK AND HIT HI ON FOREHEAD AND CAUSING A LACERATION.

GRINDING WELD O3 DECK AND TRIPPED OVER A CHAIN WHILE WALKING ON THE DECK OF BARGE.
