Employee was standing on the blade arm mount on the D8 bulldozer fueling the dozer. While trying to fuel the dozer employee was holding the bracket on the side of the dozer to balance with one hand and the fuel nozzle with the other they slipped off and fell 30". Employee landed on their feet however they hurt their left shoulder when they slipped off.

Employee was cleaning off stone on crusher deck. While bent over shoveling, a piece of stone fell from above and hit EE in the lower back of their head near the top of their neck. This impact caused EE to pass out and fall to the ground.
