Employee was retrieving model number on a gear box. In order to do this they used a manlift to elevate 24'. EE removed the guarding, retrieved information, and proceeded to remove the motor assembly. The motor was not properly secured, it fell striking the employees leg. EMS was called to the site employee was taken to DVMC for treatment. Employee was released on 6/13/2019

While pulling a clump of polymer out of a clogged chemical mixing tank. Clump was solid on outside, center was powder and it blew into employees face and eyes.
Driving haul truck, hit a bump and jarred his back.

Employee was tightening a bolt when his glove was caught in between the washer and the frame.
Employee was using a pry bar and strained his shoulder.

Employee was lifting a one inch impact above his head, twisted wrong and strained his chest.
The conditions at the plant were all clean and in good shape. The problem happened when the employee slipped while putting in a bolt on the motor mount at the cone and was not wearing his fall protection equipment.
Employee was exiting crawl space area and stood up going forward striking his head on beam jamming his neck. Employee broke his hard hat when he struck beam. He worked for 3 weeks after accident until pain was only getting worse and he decided he better see a doctor.

Ee stated he was swinging sledge hammer to break up rocks. On one swing he missed hitting the rock causing his body to twist oddly causing strain.

NEITHER OPERATOR NOR LABORER FOLLOWED PROPER LOCK-OUT PROCEDURE WHICH LEAD TO UNIT ACTIVATING. INJURED EE WENT IN TO FEEDER TO CLEAR ROCKS. OPERATOR SAW LABORER CLIMB OUT & LEANED OVER TO YEL L AT LOADER OPERATOR ON RADIO NOT TO DROP BUCKET OF MATERIALS. AS HE LEANED OVER, OPERATOR HIT FEEDER BUTTON & LABORER FELL ONTO BELT.
EE STATES "COULD ONLY USE RIGHT HAND TO PULL HIMSELF UP ONTO THE TRUCK. PUT RIGHT FOOT IN STEP ON FUEL TANK, REACHED UP AND GRABBED BAR. AS HE WAS PULLING HIMSELF UP, HIS FOOT SLIPPED & HIS WHOLE BODY SHIFTEDTO THE RIGHT & HE FELL ACROSS THE FUEL TANK.
LOADING OUT PIT RUN, BANK COLLAPSED, NO ONE AT FAULT.
EE WAS GRADING ROADS. HE WAS LETTING THE BLADE DOWN & WHEN THE BLADE TOUCHED THE ROAD, THE CONTROL LEVER KICKED BACK CAUSING WRIST TO BE JAMMED.
