EMPLOYEE WAS IN THE PROCESS OF TARPING HIS TRUCK WHEN HE FOUND THAT THE AUTOMATIC TARPER WAS FOULED WITH A BLOCK OF COAL. EMPLOYEE CLIMBED UPON THE SIDE OF THE COAL TRAILER (APPROX. 6 FT. OFF THE GROUND) IN AN EFFORT TO SHAKE THE TARP LOOSE. HE LOST HIS FOOTING & FELL TO THE GROUND. HE WS TRANSPORTED VIA AMBULANCE TO HAZARD ARH AT 12:45 PM & DIAGNOSED AS HAVING BROKEN LEFT ANKLE.
DURING THE NIGHT SHIFT DINNER BREAK THE FUEL FILTERS ON THE 992C LOADER HAD TO BE CHANGED AFTER FINISHING THIS TASK THE SLIDING DOOR ON THE REAR OF THE GREASE TRUCK STUCK CAUSING EE & SEVERAL OTHERS TO ASSIST IN PULLING THE DOOR DOWN WHEN THE DOOR CAME LOOSE IT STRUCK EE A GLANCING BLOW TO THE HEAD THERE WAS NO MENTION OF AN INJURY THEN OR AT QUIT TIME (3:30PM)
HE HAD BEEN WORKING ON THE TRUCKS BRAKE SYSTEM. AS HE CLIMBED UP THE TRUCKS LADDER TO THE CAB, HE STATED THAT HE LOST HIS FOOTING DUE TO THE SNOW ON HIS BOOTS. HE FELL APPROX. 8 FT CATCHING HI MSELF ON THE WAY DOWN. HE REPORTED TO HAZARD ARH (SAT MORNING ) WHERE HE WAS DIAGNOSED AS HAVING A BADLY BRUISED RIGHT SHOULDER, HIP AND ANKLE.
FLYROCK TRAVELED 500' AND STRUCK FOUR PARKED VEHICLES IN THE PARKING LOT. NO ONE WAS NEAR THE VEHICLES. THERE WAS APPROX. $3500.00 DAMAGE TO THREE VEHICLES.

WAS CLIMBING DOWN FROM LOADER. RATHER THAN USE THE LADDER, HE CAME DOWN OVER THE LEFT REAR TIRE, ON TO THE PLATFORM WHERE THE BATTERIES ARE LOCATED, THEN DOWN THE STEP TO THE GROUND. BANKS STA TED HE HAD RUN THRU SOME WATER ON HIS WAY TO THEFUEL TANK. HE STEPPED DOWN FROM CAB LEVER ONTO TIRE AND LOST BALANCE AND FELL. BANKS INFORMED THE NIGHT FOREMAN, FALLEN BUT DID NOT THINK HE WA
INJURED WAS IN THE PROCESS OF LOADING A COAL TRUCK. AS HE STARTED TO BACK AWAY FROM THE COAL PILE, HE HALF TURNED TO LOOKE OVE RHIS RIGHT SHOULDER TO INSURE THAT IT WAS CLEAR TO BACK UP. IT WAS AT THIS TIME STATED THAT THE LOADER JERKED AND THAT HE FELT A SHARP PAIN IN HIS BACK. HE NOTIFIED FOREMN. INJURED WAS TRANSPORTED TO HAZARD A.R.H. WHER HE WAS DIAGNOSED AS HAVING A BRUIS
EE HAD BEEN WORKING ON THE FINAL DRIVE OF A D96 DOZER. HE WIPED HIS FACE WITH A RAG WHEN AN UNSCEEN PIECE OF METAL GOT IN HIS EYE EE STATED THAT AT THE TIME, HE DID NOT REALIZE WHAT IT WAS. HE FINISHED THE SHIFT. HE WORKED SUN. HE WENT TO HAZARD ARC. WHERE IT WAS DECIDED THAT HE SHOULD GO TO U.K MED CENTER IN LEXINGTON KY MONDAY TWO SMALL PIECE OF METER WERE REMOVED AT THIS TIME.
WHILE USING AN IMPACT WRENCH, THE SOCKET SLIPPED, CAUSING EE TO BE STRUCK IN THE FACE, CUTTING HIS LIP & CRACKING HIS TOOTH. 40 STITCHES WERE REQUIRED TO SEW UP THE CUT. HE REUTRNED TO WORK TH E FOLLOWING MORNING AT 7:00 AM.
AFTER FUELING HIS DRILL EE JUMPED FROM THE TRACK TO THE GROUND WHILE HOLDING THE FUEL HOSE IN HIS HAND A DISTANCE OF APPROX. 2 1/2 FT HE WORKED THE REMAINDER OF FRI. 22ND SAT AND SUNDAY 24TH H E REPORTED TO THE DR OFFICE MON 4-25-94 WHERE HEDIAGNOSED AS HAVING A LOWER BACK SPRAIN. HE WAS TREATED AND RELEASED TO RETURN TO WORK AT 600 AM 5-2-94.
EE WAS IN THE PROCESS OF STEAM CLEANING THE DRILL WHEN HE LOST HIS FOOTING AND FELL TO THE GROUND. (APPROX. 4'). HE WAS TAKEN TO THE HOSPITAL (BY THE WATCHMAN ON DUTY) WHERE HE WAS X-RAYED & R ELEASED. HE WAS CHECKED AGAIN ON MONDAY (4-4-94)IT WAS DETERMINED AT THIS TIME THAT IT WAS A SPRAIN.
EE WAS STEPING FROM THE ROCKTRUCK TO THE GROUND WHEN HE STEPED INTO A CRACK WITH HIS RIGHT FOOT. DR ADVISED OUR OFFICE THAT HE HAD A ANKLE SPRAIN.
EE WAS STANDING ON LOADER PREPARING TO TAKE ENGINE OUT. HE HAD REMOVED THE FLOOR PLATE TO BEGIN WORK WHEN HE SLIPPED CAUSING HIM TO DROP THE FLOOR PLATE ON HIS RIGHT FOOT. HE WAS WORKING IN TH E RAIN WHICH MAD THE FOOTING SLIPPERY.
