While ascending Cat D10T bulldozer, Asset #DZ032, serviceman lost footing due to muddy boots, slipped and LEFT rib area hit steel push arm causing soreness. Employee was taken to local Urgent Care for observation and diagnosed with LEFT 9th lateral rib fracture.
Miner tripped over rocks and mud and fell on left hip
Employee was performing maintenance on Volvo A40D, asset #TR403, and dropped belly pan. While working to assess for repair, bolt broke that holds clip securing safety chain of belly pan causing metal to impact LEFT index finger. Employee was taken to local Urgent Care and diagnosed with laceration to LEFT index finger and received two sutures.
Employee called off work 3/05/24 stating "last night I had piece of metal removed from eye, I won't be in." On 3/06/24 met w/ employee and employee stated on 3/01/24 while performing maintenance of 390 bucket they felt irritation/soreness to RIGHT eye. Employee was taken to local Urgent Care for observation, sent them to eye doctor and diagnosed with foreign body to RIGHT eye.

Performing pre-operational inspection employee attempted to clean off rear reverse light on asset #TR602 and received a cut to LEFT middle finger, applied bandage and finished shift. Next day employee went to Urgent Care for observation, diagnosed with laceration to LEFT middle finger and received three stitches.

Employee was descending ladder of haul truck at end of shift and EE's right foot slipped causing EE to lose balance. EE reached with EE's right hand to grab hold of handrail and felt pain in right shoulder area.
While performing maintenance pressure washing hydraulic coolers on Cat 6030 employee squeezed trigger, wand pushed back allowing nozzle to brush across RIGHT hand causing pain to RIGHT finger area. Employee removed gloves and noticed bleeding and was taken to local hospital for observation and diagnosed with RIGHT 4th finger laceration and placed on restrictions.
Injury to left foot/ankle using shovel to break up frozen drill cuttings used to stem bore holes.

While talking on phone to another mechanic, employee turned to begin walking back to fuel truck and feet slipped causing employee to fall to ground and felt pain in right foot and ankle area. Employee was taken to local Urgent Care and diagnosed with right ankle sprain.

Employee developed back spasm and tingling in left leg while stemming boreholes.
While changing hydraulic hose on Cat D10T dozer, wrench slipped and left hand hit metal control block. Employee was taken to local Urgent Care for observation and diagnosed pain in left finger.
While performing maintenance pressure washing the 6030 SH002, employee slipped and lost balance causing fall to ground. Employee extended arms attempting to brake fall and felt pain in left wrist after impact with ground. The next day employee was taken to local Urgent Care and diagnosed with possible fracture and scheduled for MRI.

While performing maintenance of JCB 940-4 Forklift, employee leaned forward against machine and right knee pressed against metal frame. Knee hyperextended backwards causing pain and soreness to right knee area. Employee was taken to local Urgent Care for observation and diagnosed with pain in knee and possible tendinopathy of patellar tendon.
While descending ladder on dragline DL039, employee lost footing and took right arm to grab backwards to stabilize self and felt pain in right shoulder. On 2/26/18 employee refused medical attention. On 4/23/18 employee was taken to local Urgent Care for observation and put on restricted duty.

Operator of 980H loader was observed slumped over in machine. Employees removed operator from cab, placed on ground, began CPR after finding no response and contacted local EMS. Paramedics removed employee from mine site, took to local hospital for further observation. Employee was pronounced dead at hospital. Coroner report identified cause of death as massive heart attack.
While placing chain onto metal tooth of loader bucket, employee's right hand was pinched between chain and metal tooth. Employee was taken to local hospital for observation. Employee received thirteen stitches to right middle finger and diagnosed with finger laceration.
While performing maintenance changing steel hydraulic tube, employee loosened bolts and hydraulic oil splashed EE's face and eyes. Employee was taken to Urgent Care for observation and diagnosed with superficial chemical burn of both eyes.
While operating Cat D9L bulldozer in reverse, machine hit rock causing employee's left elbow to bang against metal frame inside cab. On Tuesday March 28, 2017 employee was taken to local Urgent Care for observation.
Employee parked haul truck at end of shift and was descending ladder to dismount machine and placed right foot on ground. While shifting weight onto right leg, employee felt pain and soreness to right knee area and visited local hospital for observation. Employee was diagnosed with a sprained right knee w/ possible MCL damaged.

Employee sprained left wrist changing wire rope on dragline 11/02/16 and declined medical observation. On 11/15/16, employee reported the left wrist is still sore. Employee was taken to local Urgent Care for medical observation and diagnosed with a sprain to left wrist and put on restricted duty.
While performing maintenance on Cat 6030, Asset #SH002, employee tightened hose clamp on left engine coolant hose. Hose separated from metal fitting causing hot antifreeze and steam to hit left wrist area. Employee was taken to local Urgent Care for observation and diagnosed with 2nd degree burn to left wrist area.
While re-mounting haul truck, employee attempted to pull self up while holding onto ladder, EE felt pain and soreness to EE's left shoulder area. Employee was taken for initial observation to local Urgent Care and diagnosed with strained muscle.
Employee was swinging sledge hammer attempting to hit a 4" x 68" piece of steel to break off full sheet. After swinging sledge hammer, employee felt pain and soreness to right rib cage area. Employee was taken to local Urgent Care and diagnosed with a sprained muscle to right rib area.

Employee attempted to flip a steel fender into a loader bucket. After letting go, the steel fender fell backwards hitting employee in the right knee area causing a laceration. Employee was transported to local hospital, diagnosed with laceration, and received 15 stitches.
While operating empty haul truck, employee drove over bump in road entering shovel cut that jarred lower back section causing soreness and pain to lower back area. Two days later on 9/10/15, employee was still sore and taken for observation and diagnosed with lumbar strain.

Employee attempted to descend the stairway on 4600 dragline, lost his balance and slipped, causing a fall forward and his chest area impacted the steel hand railing causing pain to chest/left side area. Employee refused medical attention. However, three days later on 12/13/14 employee was still sore, went to local hospital for observation and diagnosed with one fractured rib.
Employee was making repairs, grinding on shovel bucket SH002, to install new wear bars. At home later that evening, employee noticed a piece of metal in his left eye. On 9/22/14, the next workday, employee was taken to hospital for observation, diagnosed as having foreign body in left eye, sent to eye doctor. Eye doctor removed a metallic foreign body from the left eye.
Employee was walking, carrying fuel nozzle and dragging hose when his feet got stuck in mud causing him to lose balance. Employee fell forward and his right knee impacted the ground causing pain and soreness. He was observed at the local hospital and diagnosed with a knee contusion.
Employee was walking around the back of his personal truck to retrieve a tool, slipped on ice causing a slip/fall to the ground causing injury and bruise to right shoulder area.

While attempting to lift and secure the crane load block on the service vehicle, I felt a strain in my left stomach area.
While attempting to descend the steps on the RH120E excavator, employee slipped and fell then landed on the metal platform outside the engine room. Employee suffered a slight concussion and a bruise to his upper back area.
While standing on wooden ladder and performing maintenance on the steel hopper at the tail section of the McCloskey wheeled stacking conveyor, the ladder slid causing employee to twist, and while grabbing hold of the steel hopper this allowed his left leg to strike the steel hopper, causing a contusion to his left leg in the area of his thigh.

While attempting to lift metal screens, employee felt a pain in stomach area. Not sure what it was, employee went to see Doctor on his own. After reading cat scan, employee was diagnosed with a hernia. Employee will have operation at a future date.
While attempting to mount the D9H bulldozer, employee turned around on the ground and felt a pain in his right leg. Employee was taken to the hospital for observation.
Employee grabbed hold of metal stand and attempted to pull towards him. While lifting the stand he felt soreness and strain in his Left shoulder area.
While handling and unloading metal screens without PPE (gloves), employee reached to grab the next screen and received a laceration to the palm area of his right hand. Employee received 4 sutures.
While performing maintenance employee suffered an insect bite to his left forearm. After wound became red and swollen, employee went to hospital for observation. He was diagnosed with MRSA staph infection and kept in hospital to treat infection. Employee has not yet returned to work.
A 15 year old juvenile fell and injured his left ankle while tresspassing on an inactive part of the mine site.
While performing maintenance employee suffered an insect bite to his right bicep area.
While pulling down on the refueling arm to prepare for refueling, employee felt sudden pain and pop in his right shoulder.
Employee was operating haul truck TR604 and started experiencing chest pains. After parking haul truck employee descended haul truck, called foreman, and then taken to local hospital for observation.
While attempting to close the operator's cab door on the H285S Hydraulic Shovel employee pinched his left index finger on the door hinge.
While exiting the patrol vehicle near the fuel rack employee slipped on snow and landed on back.

While working on changing left front tire of Cat 992D loader employee was attempting to place hydraulic bead breaker on wheel when the tool slipped from hands and fell on employees left foot.
While operating unit SH001 employee developed a pain in his left side, just above the hip. Called foreman and for precaution was escorted via company personnel to local hospital for evaluation. Was diagnosed with kidney stone.
A trespasser riding an Atv on the in-active part of mine property became sick with chest pains summoned for help and was removed from the mine property via emergency personnel.

Individual was operating an ATV on mine property and unfamiliar with terrain, lost control and wrecked sustaining an injury to his right leg.
Male individual was tresspassing on mine property and wrecked his ATV. Person suffered broken collarbone, broken shoulder and broken ribs.
Engineer was climbing on waste pile while surveying and when he went to turn around to look at the last set point, he felt a tear or stretch on the outer part of his left knee, causing discomfort.

Employee was installing 12 x 12 blocks of wood under 4600 shovel. He pushed a block under body of shovel and caught right index finger.
EE was cleaning up spilled material and twisted lower back. EE was released to return to work, however is taking 2 days off to rest.
Employee reported a small rash on arm on 9/6/09. Set up an appointment with dr after rash spread entiretrm took 1 day off to go to dr's
EE was cleaning area of tail wheel when he went to move, foot got stuck and twisted knee.
EE was moving a 55 gallon drum of anti-freeze during clean-up and splashed liquid on head and then dripped into eye.

Slaker filled with lime due to frozen water lines. Slaker needed to de cleaned by hand Injury was in process of cleaning slaker, wearing two sets of protective gloves. slake had just been installed and had been working for 24 hours.
Felt pain and tingle in wrist when moving levers. Went to doctor for exam and found out to be carpal tunnel. First day EE missed work will be 2/25/09 for operation.
Employee was cleaning up work area at the end of shift and steped on rock and twisted ankle.
Employee complained of pain in wrist. Medical exam reported it as carpal tunnel. First day missing work was 11/13/08.
Employee was air arcing & grinding in a confined space when metal flew behind helmet and into eye.
Day shift foreman found an ATV Saturday morning on job site wrecked. Informed Tamaqua police. MSHA supervisor requested a 7000-1 form.
Employee noticed anti-freeze around the radiator cap when he went to tighten the cap it popped off and sprayed anti-freeze on his back and arm.
Employee went to remove fire extingusher and release for bracket was not open fully jamming left wrist and thumb.
Employee was removing a log from screening plant when a large rock slid down belt striking leg.
EE was replacing cable on 2400 shovel, EE was forcing cable into a loop when it slipped and flew back, struck EE on right side of his face.
Employee came into shop area, asking for help said he felt light headed and had chest pains. also felt numbness in arm.
Inspecting job site got bit by bug later in day area of bites looked infected.
EE was conducting his pre-shift work-around and twisted his knee, he stepped on starting fluid can.
While fueling a truck, fuel nozzle stuck splashing employee in eyes.

Employee was climbing on to excavator, slipped on 2nd step from top and fell to the ground.

COLD WEATHER, FROST. EMPLOYEE WAS HANDING FUEL HOSE UP TO THE 4600 DRAGLINE OPERATOR TO COMMENCE FUELING. THE FROST COVERED NOZZLE SLIPPED OUT OF THE OPERATOR'S HAND. THE NOZZLE KNOCKED OFF HA RD HAT AND NOZZLE HIT EMPLOYEE IN THE HEAD.

EMPLOYEE WAS PUTTING DOWN LADDER. THE LADDER STARTED TO FALL & EMPLOYEE GRABBED IT AND PULLED MUSCLES IN HIS SHOULDER AND NECK.
WHILE EMPLOYEE WAS EXITING OPERATIONS CAB OF #34 DRAGLINE HE SLIPPED ON THE STEPS AND HIT HIS RIGHT ELBOW. NO MEDICAL TREATMENT REQUIRED. WE WERE INFORMED ON 6/19/03 THAT FLUID STARTED TO ACCU MULATE AND ON 6/24/03 EMPLOYEE WENT FOR MEDICAL TREATMENT.
OPERATING D8R #1 DOZER CONTROLS. NOTE: RECEIVED A DOCTOR'S DIAGNOSIS ON 4-22-03 THAT SAID THE EMPLOYEE HAS CARPEL TUNNEL SYNDROME OF BOTH HANDS.

EE WAS REPOSITIONING HIS BODY TO WELD A NOSE BUSHING ON AN R-100 EUCLID TRUCK, WHEN HE SLIPPED ON THE FRAME WHICH CAUSED HIS BODY TO DROP DOWNWARDS. EE CAUGHT HIS LEFT SHOULDER ON THE FRAMEWOR K AND HUNG BY THE SAME.
THE EE WAS APPROACHING THE DMH DRILL TO PERFORM SOME REPAIRS WHEN HE STEPPED UPON SOME UNEVEN GROUND, CAUSING HIS LEFT FOOT TO MOVE UNEXPECTIVLY.
EMPLOYEE WAS FUELING A DOZER AT THE DUMP AREA WITH THE FUEL TRUCK. WHEN EMPLOYEE PULLED FUEL HOSE FROM TRUCK HE FELT A PAIN IN LEFT FOREARM/ELBOW AREA.
EMPLOYEE WAS ATTEMPTING TO RETURN TO HER DUMP SHANT AFTER SPOTTING A ROCK TRUCK AT THE DUMP ARA WHEN SHE FELL OVER SOME VERY DEEP RUTS OF MED. EMPLOYEE HAS FALLEN SEVERAL TIMES IN THE PAST.

EE WAS TALKING WITH HIS FOREMAN FROM THE CAB OF HIS GRADER. WHEN THE EE ATTEMPTED TO CLOSE THE DOOR THE WIND BLEW THE DOOR OPEN CAUSING THE EE TO STRAIN HIS RIGHT ELBOW. THE EE DID NOT SEEK ME DICAL ATTENTION UNTIL 2-7-96.
EE WAS DUMPING TRUCKS AT NIGHT TIME. AFTER DUMPING A TRUCK THE EE WAS WALKING BACK TO HER SHANTYWHEN SHE LOST HER VISION IN SOME SHADOWS & FELL OVER SOME MATERIAL, STRIKING THE GROUND.
EMPLOYEE WAS OPERATING HIS DOZER IN REVERSE WHEN HE HIT A BOUNCE, CAUSING HIM TO STRAIN HIS RIB/BACK AREA.
EE WAS SERVICING HIS LOADER. WHILE DESCENDING FROM THE MACHINE, HE SLIPPED FROM THE LADDER, TWISTING HIS LOWER BACK. THE EE ALSO JAMMED HIS LEFT KNEE UPON LANDING ON THE GROUND,
ON 9-21-95 AT APPROX 23:15 HRS, EMPLOYEE PARKED COMPANNY TRUCK #652 ON THE STEAM PAD AREA OF SHOP 111. SOMETIME AFTER 00:10 HRS AND BEFORE 03:55 HRS ON 9-22-95 THE TRUCK LEFT THE AREA AND TRA VELED APPROX 300' STRIKING A PARKED UTILITY TRUCK. THE WHEELS WERE NOT CHECKED. IT IS UNKNOWN HOW THE TRUCK MOVED.
EMPLOYEE SLIPPED OFF A STEP AFTER CHECKING FLUID LEVELS IN A TRUCK EMPLOYEE FELL TO THE GROUND & WHILE STANDING UPRIGHT STRUCK HIS UPPER BACK WITH THE TRUCK FRAME
EMPLOYEE WAS CHANGING A CUTTING EDGE ON A DOZER. WHEN THE EMPLOYEE LIFTED THE EDGE TO PUT IT INTO POSITION, HE FELT A PAIN IN HIS BACK.
EMPLOYEE WAS CLEANING SPILLE DDEBRIS FROM THE LOADING SITE WITH A D8 CAT DOZER. SOME MATERIAL SLID FROM A LOADED TRUCK BODY, STRIKING THE DOZER ON THE RIGTH SIDE JOLTING THE OPERATOR. EMPLOY EE STATED HE WAS THROWN AGAINST THE SEAT. EMPLOYEE ALSO STATED HE WAS NOT WEARING A SEAT BELT AT THE TIME OF THE ACCIDENT.
EE WAS ASCENDING THE LADDER TO BOARD HIS TRUCK, WHEN HIS FOOT SLIPPED, CAUSING HIS KNEE TO BUCKLE. EE FELL FORWARD INTO THE LADDER, STRIKING HIS RT KNEE ON A STEP. EE STATES HE IS UNSURE WHAT CAUSED HIS FOOT TO SLIP BECAUSE IT HAPPENED TOO QUICKLY.
EMPLOYEE WAS WELDING A DRAGLINE BUCKET FROM A SCAFFOLD.EMPLOYEE STOOD UP TO REPOSITION HIS BODY AND FELL TO THE GROUND BELOW APPROX 4 1/2 FT STRIKING HIS LEFT SHOULDER/WING BONE AGAINST THE SC AFFOLD.
WHILE THE EMPLOYEE WAS INSTALLING BATTERIES INTO THE DEMAG HE FELT A PAIN IN HIS LOWER BACK WHEN HE TRIED TO STAND UPRIGHT
EMPLOYEE WAS HAULING A LOAD OF COAL FROM THE PIT TO THE BREAKER WHEN HE PASSES OUT. THE EMPLOYEE DOES NOT KNOW HOW LONG HE WAS UNCONSCIOUS. EMPLOYEE STATES HE IS ON NEW MEDICATION FOR SCIATICA .
EE WAS ASCENDING A SET OF PORTABLE STEPS TO BEGING WELDING ON A SHOVEL BUCKET WHEN THE STEPS MOVED, CAUSING HIM TO FALL TOWARDS THE BUCKET, STRIKING HIS LEFT ELBOW AGAINST THE BUCKET.
EE WAS SERVICING HIS GRADER AT THE 111 FUEL STATION. WHEN HE ATTEMPTED TO REPOSITION HIS BODY ON THE GRADER TIRE, HE LOST HIS BALANCE, GRABBED THE DOOR TO STOP HIS FALLING MOTION, & FELT A PUL LING TYPE TWINGE IN HIS NECK AREA. [NOTE] THE LATCH WAS BROKEN.
