Hired Hauler driver was in his truck on the scale. He retrieved his ticket from the weigh master and returned to his truck. When the vehicle did not move, someone approached the truck and found the driver slumped over the seat. He was unresponsive. CPR was begun and an ambulance was called.

EMPLOYEE WAS HELPING CUT AND FASTEN 3 SECTIONS OF 2 PLY BELTING TO A 72' LONG PIECE OF ANGLE IRON. THE TWO EMPLOYEES CARRIED THE BELTING AND ANGLE IRON UP THE PLANT. FIRST EMPLOYEE PROCEEDED INJURED UP THE STAIRS. INJURED THEN WENT TO THE PLANT OFFICE AND STATED THAT HE HAD HURT HIS BACK.

EE WAS ON THE DREDGE PREPARING TO DO SOME BASIC START UP MAINTENANCE HE OPENED THE HATCH DOOR TO GET TOOLS. THE DOOR IS VERY HEAVY AND AS HE WAS CLOSING IT THE DOOR SLIPPED FROM HIS GROSP AS I T FELL CLOSED IT STRUCK AND FRACTURE HIS RIGHT INDEX FINGER.
