Employee was using a hydraulic wrench working on crusher when wrench slipped and pinched a finger against metal flange.

Employee stepped backwards and twisted left knee.

Employee was performing regular job duties with bolting and scaling and claims back spasms.

Employee was lifting machine liners with forklift and sling, liner spun around and struck top of foot.

Employee stepped out of ticket booth and turned ankle on steps. EE twisted ankle and hard hat cause a head laceration.

Employee tried to remove drill bit from a hydraulic drill. EE accidentally hit the trigger causing the bit to go through the glove and cut EE's thumb.

Employee's hard hat fell off while leaning over rail. As EE was walking down steps to meet Supervisor who brought EE a new hard had, rock flew from crusher and struck employee on back of head.

Stepped on beam to install rigging and foot slipped. Injured left ankle. Went to care facility for evaluation. Referred to Orthopedist. Was able to return to full duty.

Rail locomotive stopped suddenly and employee struck grab bar on door of cab

Employee claims he felt back strain while lifitng sheet metal

Employee pulled on plate and felt pain in forearm. Was not made aware it was reportable till 6/23/14.

Working in Pumping Station. Started to install 18" pipe with 90 on end, 7' long. Put pipe into position, started to leave it down, when hook on lug was noticed; the chain was under the hook. Chain came around hook, pipe jolted down, sprung back up and then down again, busting the rigging. Employee grabbed the pipe and smashed his thumb.

DRIVER ARRIVED @3:45PM, DROPPED EMPTY CONTAINER, HOOKED UP FULL CONTAINER, @4PM FELL WHILE WALKING BACK TO THE TRUCK. @4:14PM DRIVER USED CELL PHONE TO CONTACT COMPANY OFFICE REGARDING ACCIDENT. INJURY TO HEAD, NECK, SHOULDER, ARMS, UPPER BODY. TAKEN TO HERSHEY MEDICAL BY AMBULANCE FOR TREATMENT. RELEASED FROM E/R AFTER CT SCAN AND STITCHES. FOLLOW UP WITH FAMILY DOCTOR.

Employee slipped on snow covered ice and sprained left knee.

Employee was shoveling stone onto conveyor belt and felt discomfort in his back. He did not wish to seek treatment at that time.

Employee claims neck pain and stiffness while operating scaler machine.

Employee had hood of truck shut on thumb

Employee was helping to change head light on skid steer. Operator of skid steer moved forward to position light and pushed throttle forward with his body and ran over employees foot.

Employee worked 6/14 and 6/15 with no report of any injury. Was a no call/no show for work on Monday 6/17/13. Reported to work on 6/18/13 and said he might have hurt his back on Friday while lifting drill bits into pick up truck with another employee.

Driller was hammering on drill bit and metal shard flew off and hit employees hand

Employee failed to release pressure in hydraulic hose prior to removing. Pressure caused hose to break loose and struck employee on the right thigh.

Employee pinched 2nd finger on left hand between roller foot and upper rail of man basket. Required 3 stitches.

While cutting tree down the chainsaw came through the tree and hit the left leg.

Employee was sitting in truck while being loaded and complained of chest pain. 911 was called and employee was taken from site.

Employee did not follow procedure and got out of truck violating posted rule. He twisted left knee. Employer became aware of recordable status on 06/15/12.

While working at PI-G Pump, setting up a level detector the EE stepped down on a pipe then stepped on to a flat base and when stepping off the base onto the ground he lost balance and twisted his ankle and knee.

Employee was splicing conveyor belt and blade on utility knife broke off and cut finger.

Employee was cleaning the Resevior Pumphouse. He was moving 2 gallon buckets partially filled with stone. After moving 3rd bucket, he experienced intense abdominal pain. Had co-worker contact supervisor.

EE slipped and fell on icy walkway, fell and twisted his knee.

He was wearing protection but metal from grinding got in his right eye. He was given a prescription by the doctor.

Employee pinched his left hand between underside of conveyor and a bucket.

He was cleaning out the feeder pan and his legs felt like they were asleep.

He was digging with a loader, hit a rock ridge jerking him forward and he jammed his left arm.

He was pulling a smart roller up onto a D-6 conveyor with a rope and felt sharp, shooting pains in his left forearm.

He was pulling a hose from fuel truck, tripped backwards over rocks and fell on buttocks.

He was shoveling at the Sinter Flux and his right shoulder started hurting.

Employee was using a pry bar and a spade to remove build up from underneath the primary motor base.

Track drill operator was using a hammer to help a drill bit into place. A piece of metal broke free from the hammer and struck him in the leg.

Employee was shoveling build up from under a live conveyor. The shovel made contact with the moving belt and was pulled into a return idler. The handle pinched his thumb against the conveyor frame.

While washing out an area of the plant with a water hose, a rock from some unknown area fell and struck him in the right shoulder.

WAS ATTEMPTING TO REMOVE WHEEL CHOCKS FROM HAUL TRUCK TIRES. ONE CHOCK WAS STUCK. PULLED HARD ON CHOCK. LOST BALANCE AND STRUCK HAND/FINGERS ON THE TRUCK FRAME CAUSING INJURY TO FINGER.

THE EE WAS MOVING A SMALL 44" DIAMETER STEEL PLATE WEIGHING ABOUT 60 LBS. WHEN HE PICKED UP THE PLATE HE FELT A POPPING SENSATION IN HIS BACK, BUT CONTINUED TO CARRY THE PLATE APPROX 50 FT. TH E INJURY WAS A RESULT OF IMPROPER BODY MECHANICSWHILE LIFTING. THE PLATE SHOULD HAVE BEEN CARRIED IN FRONT OF THE BODY RATHER THAN TO THE BACKSIDE.

EE WAS WASHING THE WINDSHIELD OF A HAUL TRUCK. HE LOST HIS BALANCE AND FELL TO THE GROUND, SUFFERING A COMPOUND FRACTURE OF THE RIGHT ANKLE AND EVULSION TYPE INJURY TO HIS RIGHT INDEX FINGER AND LACERATIONS ON THE LEFT INDEX FINGER.
