EE was attempting to rotate a rotor shaft manually to grease the bearing housing. EE placed EE's hands on the belt to move the shaft and when EE pulled EE's pinky finger was pulled into the motor sheave causing a laceration and a fracture to EE's pinky finger.
Employee reported to supervisor EE was experiencing chest pain for a personal medical issue.

EE threw a track on EE's L8 drill rig. While compressing the track tensioner EE struck the tensioner with a sledge hammer. When struck a shard of metal broke off the tensioner and punctured EE's right thigh. EE received sutures and butterfly closures at the Urgent Care Center.
Employee was assisting with setting up a 600 ton mobile crane on the bench of the quarry. EE jumped off the rear of the crane, about 3 ft. EE's right foot landed on a rock and threw EE's ankle and weight off the right. All of EE's weight came down on the right ankle/foot. Employee was diagnosed with an ankle sprain.
