Silver Springs Quarry Metal/Non-Metal

EE was attempting to rotate a rotor shaft manually to grease the bearing housing. EE placed EE's hands on the belt to move the shaft and when EE pulled EE's pinky finger was pulled into the motor sheave causing a laceration and a fracture to EE's pinky finger.

Employee reported to supervisor EE was experiencing chest pain for a personal medical issue.

EE threw a track on EE's L8 drill rig. While compressing the track tensioner EE struck the tensioner with a sledge hammer. When struck a shard of metal broke off the tensioner and punctured EE's right thigh. EE received sutures and butterfly closures at the Urgent Care Center.

Employee was assisting with setting up a 600 ton mobile crane on the bench of the quarry. EE jumped off the rear of the crane, about 3 ft. EE's right foot landed on a rock and threw EE's ankle and weight off the right. All of EE's weight came down on the right ankle/foot. Employee was diagnosed with an ankle sprain.

Pipe fitting on water pipe was leaking, employee tried to tighten valve and pipe swung against the employee's arm, pinching between pipe and handrail. Employee has bruised and torn right bicep

Hydraulic pump was being lifted by chain, chain slipped and pump fell onto employee foot.

Employee was determined to have a recordable hearing loss discovered during a routine audiometric exam.

Employee was welding stovepipe. He was changing between safety glasses and welding shield when debris went into his right eye.

Employee was removing bucket off skid steer and when he knocked pin loose the bucket fell on his left foot behind his steel toe.

Employee was putting new teeth on a bucket, a piece of a tooth broke off and punctured skin on his left shin.

EE WAS ON LADDER. LADDER WAS NOT ON STABLE GROUND. ONE LEG OF LADDER SANK INTO GROUND. EE FELL FROM LADDER.

EMPLOYEE STATES THAT HE STRAINE DHIS BACK WHILE WORKING BUT NOT SURE HOW.

CLOSING RIGHT SIDE WINDOW ON A CAT 980 FRONT END LOADER. IT WAS RAINING THE HINGES WERE WET. WINDOW CLOSED FASTER THAN THE OPERATOR WAS USED TO. FINGER WAS PINCHED.

MOVING AIR TRACK DRILL UP HILL CONTROLS MALFUNCTIONED UP SETTING DRILL AND STRIKING OPERATOR AS IT ROLLED DOWN HILL

MOVING BOULERS BY HAND IN FEEDER TO THE PRIMARY CRUSHER BOULDERS MOVED CATHCING FINGER BETWEEN THEM.

COME-ALONG FELL ON FINGER.

TWISTED BACK WHILE TWISTING AROUND IN SEAT TO BACK UP FRONT END LOADER.

RIGHT HAND BECAME WEDGED BETWEEN TWO ROCKS WHILE TRYING TO FREE OTHER ROCKS

STEPPED OFF OF BOTTOM RUNG OF PLANT LADDER AND TWISTED ANKLE AND KNEE STEPPED ON LOOSE STONE

USING SLEDGE HAMMER & BAR TO REMOVE PIN FROM TRACK OF DRILL. PIECE OF METAL BROKE OFF TRACK STRIKING EMPL ON EYE LID. FAILURE OF EMPL TO WEAR SAFETY GLASSES.

EMPLOYEE WAS AT THE TOP OF A FIXED LADDER APPROX 7FT OFF GR OUND WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL AS HE WAS FALLING HIS COAT SLEEVE CAUGHT ON THE TOP OF THE STEEL LADDER CAUSING HIM T O HANG BRIEFLY BY THE LEFT ARM
