Miner injured middle finger of right hand between tailgate and bed of a dump truck.

Employee developed redness on skin in the area where their respirator fits around the employee's nose. The employee went to the doctor, and was diagnosed as having "unspecified contact dermatitis, unspecified cause". The employee was prescribed a topical steroid cream and returned to work without restrictions.
Employee was removing a broken bag from the air bagger turntable when the bag broke further causing the employee to hit EE's right hand off the guard. The guard is covered with foam and rubber. The medical paperwork from the doctor was received by management on 9/11/2023. The employee is on light duty due to a strain to right hand and thumb. Medical follow up scheduled for 9/19/23.
A truck driver from a trucking co that's a subsidiary of R.W. Sidley was loading sand at RW Sidley's dry plant when the driver tripped on a 2x4 that was partially covered in sand and landed on knee. The incident was not reported until 2/21/23. The mine was notified on 2/27/23 that the driver had sought medical attention and this had become a reportable incident.

During a period of heavy rainfall the drains within the bagging room became overwhelmed with water. A miner working in this area was walking through the water when they stepped on some loose gravel causing the miner to slip and fall. The miner suffered right rib contusions and contusions on their right arm and right leg.

Plowed snow turned into ice. When the operator walked out of the plant the operator slipped on the ice and sprained EE's ankle.

Employee was walking away from the fuel pump when they slipped on ice and landed on a shoulder. Employee went to the doctor in December and began physical therapy at the end of January (18 visits). Employee went for a second opinion and was put on restricted duty (4/28/21).

Employee was removing a pipe from the manifold. EE felt a pop in shoulder. Employee continued to work under normal conditions with no restrictions from doctor exam on 12/10/18. On 12/26, employee had an MRI conducted. Results came in on 1/10/19 that has now diagnosed a shoulder injury and restrictions have been implemented by doctor.
One of the sand chutes was becoming clogged with material in the north crusher. The employee was using the clearing tool to clear the material. As the employee was clearing the chute, EE struck finger between the tool and the chute frame.
EE reviewed customers paperwork for a pickup. EE let the driver know to enter the center of the building. As the EE was stepping back onto the lift truck, the truck driver entered the building at an angle, allowing the trailer to catch the lift truck dragging it into and pinning lift truck at the building and the trailer. EE hit shoulder and knee on the frame of the lift truck.
At the end of the day, the employee stepped out of the loader carrying lunch box. When EE stepped out, EE slipped on the icy platform on the loader. EE was able to grab the handle but struck left side on the machine, fracturing two ribs. Cold temperatures, rain and ice was forming throughout the day.
Contractor turned portable belt sander on as they turned the sander on its side, their finger became caught in between the rotating belt guide on the sander, creating an avulsion to left hand index finger and tip.

Removing a cover plate on carryall trailer when it slipped and came down on left hand causing a deep cut on third finger. Onsite first aid rendered and then transported to clinic where he received 3 stitches.
Employee was climbing up onto his truck and strained the calf muscle on his right leg.

EE WAS LIFTING (WITH ASSISTANCE) A LARGE BAG OF SAND OF A TRAILER WITH A RAMP. EE ATTEMPTED TO CLOSE DOOR ON TRAILER WHEN RAMP SWUNG AROUND HITTING HIS LEFT HEEL AND KNOCKING HIM DOWN.
