On December 11, 2023, EE from Sureco Construction was instructed to install the cover cap. EE began cleaning the dust from the tin and after finishing a section, EE moved to another section. That's when EE fell through the roof and landed on concrete. EE's left leg was angulated in an abnormal way. Paramedics arrived and took EE to the hospital.
Miner found the contractor truck driver walking with their cell on hands and noticed that the driver was bleeding from the nose and helped them walk back to the office, they asked the driver what happened. Driver stated that they were trying to lift up the handle lock of the door to open the trailer and it hit them on the nose.

On 10/12/16, EE went to see an Ophthalmologist who removed a piece of metal from EE's right eye. On 10/14/16, EE told Supervisor that EE was using a hammer to loosen material in a hopper when EE felt something in EE's right eye.

A piece of dust from the conveyor blew into her left eye. Potassium Permanganate is damaging to eye tissue on contact. Employee had on Company issued safety glasses. Employee utilized the new Eye Wash Station. Employee was taken to the South Texas Regional Hospital. Treatment was continued flushing of the eye and given antibiotics for her eye.

She was wearing all her PPE when a gust of wind blew a particle behind her safety glasses into her eye. She went to the ER to have her eye checked, today, she has an appointment with an Ophthalmologist to have the foreign particle removed.
Two Employees were cleaning around a conveyor belt, one on the tail pulley, one on the head pulley. The employee on the tail pulley was leaning on his right hand, his hand slipped and his left arm got caught in the conveyor. The employee sustained a fracture to his left wrist.

The employee was filling the fork lift propane fuel tank. Upon completion of filling the tank, the employee closed the valve on the bulk tank, closed the valve on the fork lift fuel tank, he did not bleed the fill hose and when he disconnected the hose he was sprayed on left upper arm with liquid propane, causing a cold thermal burn.

This accident occurred in the morning, calm weather, sunny day. The employee was working in the warehouse counting bulk bags of product for inventory. While moving around during the count, the employee caught his foot in one of the lifting loops on the bag and twisted his ankle. The employee was sent to the emergency room for x-rays.

OPERATOR WAS DISMOUNTING THE LOADER AND ONLY HOLDING THE HANDRAIL WITH ONE HAND - HE SLIPPED AND FELL TO THE GROUND. HE HIT THE GROUND WITH HIS BACK.
