On Tuesday 7-2-19 employee was diagnosed with a spider bite on left arm.

Employee put the handle of the main disconnect box to the off position resulting in a laceration that required one stitch. No days off, No Restrictions and no prescription medicine.

On 10/10/16, Supervisor noticed that EE's right eye was red. EE stated that EE did not know what had caused the redness in EE's eye. On 10/12/16, EE went to see an Ophthalmologist who removed a piece of metal from EE's right eye. On 10/14/16, EE told Supervisor that EE was using a hammer to loosen material in a hopper when EE felt something in EE's right eye.

Employee reported on May 11. 2015 that on Saturday May 9, 2015 while attempting to unplug a spout, employee felt pain in right shoulder, company management placed employee on restrictive duty/NO LOST DAYS.

Employee fell and slipped off a ladder while taking pictures of the screen media on the secondary side. When she fell down she hit the screen structure and bruised her back, elbow, and wrist when she fell.

MOVING SEALER MACHINE FROM BAGGING LINE. SEALER GOT STOPPED BY AIR HOSE ON GROUND AND FELL ON EMPLOYEE LEFT SHOULDER.

EE employee was running inside of warehouse 1 stepped on material buildup, and twisted his ankle, NO LOST WORK DAYS, OR RESTRICTIVE DUTY

EE was cleaning a finished Pallet when another Employee Was setting up another Pallet and it landed on her Upper Right Foot.

CHISELING ON FAN HUB AND CHISEL BROKE A SLIVER PIECE AND FLEW INTO TOP OF LEFT HAND AND INTO BLOOD VEIN - EMPLOYEE WENT TO CLINIC BECAUSE OF THE MEDICAL CONCERN - XRAYS ONLY WERE TAKEN.

TRIPPED ON FAN CORD INTO PORTABLE 10LB SEALER MACHINE AND BRUISED BACK AND NECK

ROTARY AIR LOCK VALVE WAS CHOKED. EMPLOYEE REMOVED PIPING AND WAS WIPING OFF OUTSIDE MATERIAL AND AIRLOCK SUCKED/VACUUMED FINGER INTO HOLE AND VEIN CHOPPED OFF OF TIP OF LEFT INDEX FINGER".

WHILE EXITING IMPACT CRUSHER TWISTED RIGHT KNEE

PASSED OUT DUE TO DEHYDRATION - CALLED 911 AND AMBULANCE WAS DISPATCHED TO PLANT. TOOK VITALS AND GAVE WATER AND CANDY. EMPLOYEE REFUSED TRANSPORT AND IS OKAY NOW

A piece of dust from the conveyor blew into her left eye. Potassium Permanganate is damaging to eye tissue on contact. Employee had on Company issued safety glasses. Employee utilized the new Eye Wash Station. Employee was taken to the South Texas Regional Hospital. Treatment was continued flushing of the eye and given antibiotics for her eye.

The employee just finished loading a truck at the warehouse and went to another outside production site. She was wearing all her PPE when a gust of wind blew a particle behind her safety glasses into her eye. She went to the ER to have her eye checked, today, she has an appointment with an Ophthalmologist to have the foreign particle removed.

Two Employees were cleaning around a conveyor belt, one on the tail pulley, one on the head pulley. The employee on the tail pulley was leaning on his right hand, his hand slipped and his left arm got caught in the conveyor. The employee sustained a fracture to his left wrist.

Worker was stacking 50# bag, failed to walk around pallet to stack bags, instead walked over pallet. Possible sprain right knee. Worker sent to doctor.

The employee was filling the fork lift propane fuel tank. Upon completion of filling the tank, the employee closed the valve on the bulk tank, closed the valve on the fork lift fuel tank, he did not bleed the fill hose and when he disconnected the hose he was sprayed on left upper arm with liquid propane, causing a cold thermal burn.

This accident occurred in the morning, calm weather, sunny day. The employee was working in the warehouse counting bulk bags of product for inventory. While moving around during the count, the employee caught his foot in one of the lifting loops on the bag and twisted his ankle. The employee was sent to the emergency room for x-rays.

Caught finger under conveyor belt.

Moving screen. When lowering to ground, screen started to slide off forks. Caught one fork and pulled fork lift over onto screen. No damage to fork lift until we pulled it back upright, bent roll cage. Will replace and recertify.

He was bagging on the #25 bagline when he sprained a muscle.

He was lifting a #50 bag. Bag slipped, he attempted to catch it.

EMPLOYEE SLIPPED ON LADDER, LADDER WAS ON WET SURFACE.

SMASHED HIS FINGER ON JUG HANDLE.

HE TRIPPED ON A ROLL OF STRETCH WRAP AND FELL.

HIT A TIRE RIM, HAMMER SLIPPED AND HIT HIS FOOT.

CAUGHT HAND BETWEEN STRAPS AND THE FORKLIFT.

HE WAS LIFTING 25 LB BAGS WHICH RESULTED IN HAVING A SORE WRIST.

FELL ONTO SIDE OF PALLET AND CUT HIS LEFT LEG.

HURT HIS BACK. BAGGING CAT LITTER.

HURT HIS BACK WHILE LIFTING 50 LB BAGS. HE WAS OUT 2 DAYS. DR EXCUSE SAYS HE CAN LIFT NO MORE THAN 20 LBS.

EMPLOYEE WAS PUSHING WHEELBARROW AND SLIPPED IN MUD.

SLIPPED WHILE CLIMBING DOWN LADDER, HIT HIS RIGHT KNEE ON A CROSS PIECE.

HURT RIGHT SHOULDER WHILE SHOVELING DEBRIS AROUND BIN.

HURT LEFT SHOULDER WHILE LIFTING 25 LB BAGS.

EMPLOYEE WAS RESTACKING PALLETS, FELT PAIN IN LOWER BACK.

HURT LEFT SHOULDER BAGGING CAT LITTER.

SLIP ON LOADER AND HURT LEFT THUMB.

SLIPPED WHEN CLIMBING OFF LOADER. WORKED TWO WEEKS BEFORE GOING TO DOCTOR. SORE RIGHT SHOULDER.

BULK BAG STRAP TORE AND BAG FELL ON PALLET AGAINST LEG.

STACKING 25 LB BAGS SAID SHE CAUGHT ONE WRONG AND IT HURT HER SHOULDER.

EE WAS CLIMBING OFF DOZER WHILE IT WAS RAINING AND SLIPPED HURTING GROIN AREA

EMPLOYEE WAS CONSTRUCTING NEW CONVEYOR, WAS WALKING BY IT AND HIT HIS HEAD ON METAL BRACE SUPPORT.

OPERATOR SAID HE HURT HIS WRIST WHILE BAGGING CAT LITTER 11-28-94. HE WORKED 11-29-94 HE WAS OFF 11-30-94 & 12-1-94 WITH PAY. HE DID NTO REPORT THIS ACCIDENT TO ME UNTIL 1-30-94. HE WORKED 12-2-94 NO COMPLAINTS, MONDAY 12-5-94 HE WEN TOTGE DR (1 WEEK LATER). HE TOLD HIM NOT TO WORK UNTL 12-14-94. LEFT WRIST SPRAIN..

OPERATOR WAS DISMOUNTING THE LOADER AND ONLY HOLDING THE HANDRAIL WITH ONE HAND - HE SLIPPED AND FELL TO THE GROUND. HE HIT THE GROUND WITH HIS BACK.

BUILD UP ON TAIL PULLEY CAUSED CONVEYOR PRIMARY DRYER DISCHARGE TO STOP OPERATOR PULLED ON BELT TO GET IT GOING WHEN IT STARTED IT PULLED HIS ARM INTO THE TAKE UP PULLEY INJURIES WERE ABRASION S CUT LEFT HAND AND A CRACKED BONE LEFT FOREARM.

THE INJURED HAS BEEN GOING TO VARIOUS DOCTORS FOR ALONG TIME ON 03-26-88.HE WENT TO DOC ENGLISH-WHO SAID HE WAS SUFFERING FROM OSTEOCHONDROSIS OF T EH THORACIC SPINE.HE WAS PLACED ON LEAVE OF ABSENCE SINCE WE HAVE LIGHT DUTY JOBS.
