Individual was getting supplies out of a Job (Knack) box, when the individuals head caught the lid. Causing the lid to close on the back of the head.

Employee suffered a blood clot in his lungs, which rendered him unconscious. He was driving between the mine & the mill. He stopped the truck at the raw ore storage area. Another driver discovered him slumped over the wheel, unconscious. He pulled him out of the truck and administered CPR until the ambulance arrived.

While EE was stepping down from helpers stand on back of drill rig, he stepped on uneven terrain and twisted his knee causing him to fall to the ground. Initial treatment was ice and elevation and then transferred to Barrett hospital for further evaluation.

While removing O2 cylinder from pickup finger was caught between cylinder and latch on tailgate resulting in cut finger.
The injured employee was backing haul truck on an ore stockpile to dump load of material for construction of safety berm. Haul truck travelled over face of stockpile and landed upside down on loadout area. Blunt force trauma resulted in two fractured ribs and a fractured vertebra.

WHILE DOING A SURVEY, HE WENT TO STEP UP ON A ROCK AND HIS KNEE GAVE OUT. HE WAS SEENT AT THE HOSPITAL AT APPROX. 1500 ON 6-24-99. DIAGNOSED A HAVING A MENISCAL TEAR RIGHT KNEE AND WAS PUT IN A BRACE FOR 2-6 WEEKS.

WHILE WORKING ON A 235 HOE WENT TO GRAB TWO GEARS AND CUT HIS RIGHT THUMB DEEP ENOUGH TO REQUIRESTITCHES.

EE WAS ALIGNING BOLT HOLES ON A LIFT CYLINDER WHILE INSTALLING A HOIST CYLINDER SUPPORTED BY A PRY BAR. THE PRYBAR SLIPPED WHILE EE WAS CHECKING HOLE FOR ALIGNMENT WITH FINGERS.

EMPLOYEE WAS BACKING UP TO THE #1 HIGH GRADE ORE DUMP. EMPLOYEE DID BACK UP AND DUMP THE LOAD, BUT IMMEDIATELY UPON THE MATERIAL LEAVING THE DUMP BOX, THE DUMP FAILED CAUSING THE TRUCK TO GO O VER THE DUMP. THE DUMP SITE WAS APPROXIMATELY 100' HIGH AND THE TRUCK WENT DOWN THE DUMP 55'.
EE SLIPPED AND FELL WHILE CLIMBING OFF 225 BACHOE, STRIKING BACK OF HEAD ON ROCK, LEAVING APPROX. A 2 INCH CUT ALSO SCRAPED RIGHT FOREARM.
EMPLOYEE WAS OPERATING A HAUL TRUCK.HE BACKED IN UNDER THE SHOVEL TO BE LOADED.WHILE BEING LOADED,A LARGE BOULDER DROPPED OUT OF THE BUCKET HITTING THE HEADACHE RACK RESULTING IN THIS EMPLOYEE JARRING HIS BACK.
EMPLOYEE WAS LIFTING A 62.5 LB BOX OF BOOSTERS INTO THE BACK OF THE BLASTING TRUCK UPON LIFTINGTHE THIRD BOX INTO THE PICKUP HE TWISTED & HIS LOWER BACK GAVE OUT CAUSING HIM TO FALL TO THE GR OUND THE FALL DID NOT CAUSE THE INJURY BUT THE LIFTING DID
