Individual was getting supplies out of a Job (Knack) box, when the individuals head caught the lid. Causing the lid to close on the back of the head.

Employee suffered a blood clot in his lungs, which rendered him unconscious. He was driving between the mine & the mill. He stopped the truck at the raw ore storage area. Another driver discovered him slumped over the wheel, unconscious. He pulled him out of the truck and administered CPR until the ambulance arrived.

While EE was stepping down from helpers stand on back of drill rig, he stepped on uneven terrain and twisted his knee causing him to fall to the ground. Initial treatment was ice and elevation and then transferred to Barrett hospital for further evaluation.

The injured employee was backing haul truck on an ore stockpile to dump load of material for construction of safety berm. Haul truck travelled over face of stockpile and landed upside down on loadout area. Blunt force trauma resulted in two fractured ribs and a fractured vertebra.
