Employee was removing the wings of a sand dewatering screw. He was using a cutting torch to remove the bolts that held the back plate in place. During this process the torch hose got caught on something. While trying to free the hose, he was positioned under the plate. The final bolts broke, causing the plate to land on top of him.

EE WAS CHANGING IDLERS ON A CONVEYOR AND WAS TRYING TO REPOSITION HANDS TO HOLD THE IDLER, WHEN IT SLIPPED CAUSING FINGER TO BE PINCHED BETWEEN THE CAMS ON THE IDLER.
EE & CO-WORKER WERE CARRYING AN ALUMINUM LADDER TO THE SHAKER. WHILE TURNING TO GO UP THE STAIRS EE FELT A PULL IN HIS UPPER LOWER BACK. THE ACCIDENT WAS CAUSED BY THE LENGTH OF THE LADDER & THE SHARP TURN ATOP THE STAIRS.
THE OPERATOR WAS ENTERING OUR STOCKPILE AREA AND CROSSED A DRAINAGE DITCH. HE HAD BEEN THERE EARLIER IN THE DAY TO DRAIN WATER FROM THE STOCKPILE AREA. HE WAS TRAVELING APPROXIMATELY 15 MPH WH EN HE ENTERED THE DITCH. AT THAT TIME THE WEIGHT OF THE LOADER SHIFTED FROM LEFT TO RIGHT WHICH FORCED HIS BACK TO SHIFT LEFT TO RIGHT THEREFORE CAUSING A MUSCLE STRAIN IN THE LOWER BACK.

EE HELPED CARRY SOME CONCRETE BOARD USED TO REPAIR FLOOR. THE BOARD WAS CARRIED APPROX. 40 YDS DOWN A RAMP. EE CONTINUED TO WORK ON FLOOR PROJECT UNTIL COMPLETED... (1/13/04-HAD SURGERY)
EE WAS HELPING HOLD A TAIL PULLEY GUARD WHEN WELDING WAS GOING ON. HE RECEIVED FLASH BURNS TO HIS EYES.
EMPLOYEE WAS CHECKING HYDRAULIC PRESSURE ON CLASSIFIER, WHILE COMING DOWN THE STAIRS HE SAID HIS LEG LOCKED UP AND GAVE HIM A JAR. HE CONTINUED TO WORK, AS THE NIGHT WENT ON HIS BACK STARTED T O HURT. EE LEFT WORK AROUND 3:00 AM AND HE COULDHARDLY WALK.
EE WAS ALIGNING 2 PIECES OF FLANGED PIPE WHICH WERE FLOATING ON PONTOONS ON THE DREDGE POND. THEY WERE USING AN OUTBOARD BOAT TO LINE UP THE PIPE BECAUSE OF HIGH WIND. INJURED WAS ON THE PIPEL INE. THE BOAT WAS AT HIGH THROTTLE TO COMPENSATEFOR THE WIND.INJURED STEPPED ONTO THE BOAT & THEFRONT OF BOAT SLIPPED UNDER PIPELINE. THE BOAT SHOT FORWARD CATCHING LEG BETWEEN BOAT & PIPELIN
EE SLID A HYDRAULIC MOTOR WHICH WAS UNDER A SHELF TO A POSITION WHEN HE COULD LIFT IT TO A TABLE FOR EXAMINATION. THE PUMP WAS ON THE SHOP FLOOR. HE SQUATTED OVER IT AND LIFTED IT TO A WORK TA BLE. AS HE WAS LIFTING HE FELT A TEARING SENSATION & PAIN IN HIS GROIN. HE WENT TO THE DR. AND WAS TOLD THAT HE HAD A HERNIA. HE RETURNED TO WORK WITH NO HEAVY LIFTING FOR 48 HRS. RESTRICTION

O F THE 6TH. HE PRESCRIBED DROPS & COVERED IT FOR THE DAY. NO FOLLOW UP NECESSARY, PRECAUTIONARY DR VISIT ONLY.

EMPLOYEE WAS OPERATING A JOHN DEERE FRONT END LOADER, ALLEDGES THAT THE SEAT DID NOT ADJUST FOR HIM, THUS CAUSING HIM BACK PAIN.
EE WAS LOADING RAIL CAR IN A 980G LOADER HE PULLED OFF OF THE PAVED CROSSING ONTO THE LOADING PAD. THE UNEVEN AREA CAUSED THE LOADER TO JERK IT JARRED EE CAUSING BACK PAIN

WHILE WORKING ON A DREDGE PUMP WITH AN AIR CHISEL, A METAL FRAGMENT STRUCK HIM IN THE FOREHEAD. A DOCTOR REMOVED THE FRAGMENT.
THE EE WAS USING A BOX KNIFE TO CUT SKIRT BOARD RUBBER. THE KNIFE SLIPPED & HE PUNCTURED HIS LEFT THUMB - HEEL OF THE THUMB.

EE HAD A LOWER LUMBAR STRAIN EMPLOYEE STATED HE WAS WASHING 980C WHEEL LOADER WITH A 3000 PS1 WASHER THE FATIGUE FROM HOLDING THE WASHER CAUSED A STRAIN 3 WEEKS LATER IT WAS REPORTED EMPLOYEE SAID HE HAD BEEN TO THE DOC & IT WAS REQUESTED HE TAKE THE REST OF THE WEEK OFF FOR REST (03-14-95) THE DAY HE RETURNED TO WORK WE WERE INFORMED THE STRAIN WAS RELATED
