The employee was using a shovel to try and get a rock out of the #1 conveyor belt scraper when EE hit hand on the conveyor. The next day the hand started swelling up so EE went to see a doctor. They told EE EE fractured the thumb and put EE on restricted duty for two weeks.

Employee was changing a screen panel on the #2 screen, while picking up the screen panel, EE was cut by a sharp piece of metal on EE's right hand middle finger.

Employee was using a come along to change head pulley bearings when their right index finger got cut by getting caught between the come along and the conveyor transmission/gearbox.

Employee notified yard foreman on 01/12/2024, that on 01/04/2024, EE was digging a rock out of a cone crusher, when EE hurt EE's hip or back. EE made an appointment with a doctor for 01/17/2024. The doctor prescribed EE medication, and gave EE a note on 01/18/2024 that EE was able to return to work regular duty.

Employee was using a razor knife to cut drive belts that were hanging from HP400 Crusher when employee cut self with the knife on left arm.

The miner was grinding a steel plate when the grinding wheel broke apart. The piece that came off cut their right leg requiring going to the emergency room for stitches.

The miner was grinding a steel plate and a small metal chip got into EE's eye. EE was wearing eye protection.

The employee was moving/relocating a portable LT130 Metso jaw crusher. The employee was wearing a hard hat. A rock fell off of the top of the machine around the feeder area, which they saw the shadow of and looked up and the rock struck them in the head under hard hat.

While doing plumbing work, the employee caught left hand thumb between a pipe wrench and a jaw wrench. This caused the finger nail on EE's thumb to tear, EE went to the hospital where they gave EE stitches to repair it.

While operating an end dump, the employee started experiencing soreness in the left knee, as the day went along it got worse.

The employee was carrying a bucket shroud, climbing over shot rock to get to the excavator on its perch. EE lost balance, fell, and thumb was crushed between a rock and the shroud. EE's thumb split open and required eleven stitches.

Employee was attempting to connect a 3" hose with a cam lock fitting to a pipe. EE jammed hand against the screen while pulling on the hose and broke finger.

Employees were cutting a bolt out of a screen. EE was attempting to knock the bolt out and a piece of hot slag landed on EE's face near eye. When EE tried to brush it off, brushed it into eye. EE was wearing safety glasses at the time of the injury.

Employee was assisting cleaning out a jammed chute when they cut their wrist on a sharp rock. Cut was bleeding so employee was transported by company to the local emergency room where they received (2) stitches to close cut.

The employee was pulling on a breaker bar (ratchet) which disconnected from the socket causing the employee to slip and twist employee's left knee.

Employee was working reinstalling a repaired feeder. EE jammed EE's hard hat against the feeder, which caused the hard hat suspension to knock a stud that holds EE's hearing aid into EE's ear out. The employee is scheduled for surgery in January to repair this problem and will require a two week recovery period out of work.

Employee felt a pull in back while working on replacing a conveyor belt. EE was working overhead putting the belt on. EE notified supervisor at the end of the day but did not think it was a big problem. EE worked regular shifts until Friday, March 4th, when EE called in and said they needed to have it checked, that it wasn't getting better.

Employee cut his knee while cutting a piece of rubber belt.

We were in the process of pulling the moveable die from the C125 crusher when the die moved forward and pinched employee's foot between both jaw dies.

Employee was standing on a catwalk lifting a screen panel to hand up to another employee when he felt his left elbow "pop."

Employee was working on the jaw's discharge conveyor when another employee energized the conveyor. The conveyor started, the employee who was on the belt fell, rolled around and hit his head before the conveyor was shutdown.

Employee was cutting a piece of conveyor belt with a plastic utility knife. The knife broke and the employee cut his finger.

Employee wiped his eye while wearing cotton, string knit gloves and got something in his eye. He thinks whatever it was must have been on the glove.

A tool was tossed to employee who was working on a screen. He caught the tool, but the electrical cord whipped around and knocked off his safety glasses and cut him above his left eye.

Employee tripped over concrete footings on scale. He twisted and sprained his ankle.

While working with an air tool to tighten a bolt the employee cut his hand.

While performing maintenance work on a jaw crusher, the employee activated a 1/2" drive air gun which blew dust into his face and eye. The employee was wearing safety glasses at the time.

Employee was shoveling under the crusher when "he felt something give" in his shoulder.

Moving rock in road, slipped and sprained ankle.

Removing bolt using impact gun. Finger crushed between truck and gun.

Working with wrench on worn out bolt. Bolt had sharp edges. Spun around and cut finger.

Driving loader - vibration started pain in lower back and leg.

Climbed down from excavator to help on the portable plant. While descending the 3" x 5" pile where he was working, he twisted his knee and fell to the ground.
