Employee was operating a stand behind skid steer. While backing up EE trapped elbow and hand between a structural crossbeam and the control support bar causing EE's wrist to bend backwards.
Slurry pump B feeding the wash plant, plugged. After shutting down, the technician opened the pump drain on bottom of the pump housing. Due to the head pressure, water sprayed out of the drain. Since the pump was plugged, the water was extremely hot, burning the technicians arm. The technician was treated for second degree burns on left forearm at the hospital.

Miner entered the mine area in a John Deere Gator and thought the loader operator saw EE enter. Miner got out of the Gator to talk to the operator. EE parked in what EE thought was a safe area but as the loader backed up and turned it struck the Gator.

A rock flung from a conveyor and struck the plant supervisor in the back, no serious injuries were sustained. Medical attention was sought, some bruising. On July 31st, 2013 the employee advised that on his way home from work the day before he experienced some pain. He sought medical opinions and has been instructed to rest at home for two weeks.

While winterizing conveyor system, EE was standing on top of 3100 lb counterweight while a co-worker raised it using a 20 ton crane. The cable from the crane severed causing the weight & EE to fall approx 10'. The weight came to abrupt stop causing EE to fall flat onto the weight & the 150 lb ball that was previously attached to the crane cable fell on his legs.
