It was raining, ground wet, employee was walking into mud room at sand plant when they slipped in mud and polymer causing them to fall. States they have back and knee pain.

This is not an accident or injury. Employee was operating a loader when EE needed to use the restroom, EE went to stand and could not feel EE's leg, ambulance took to hospital where they found EE had low potassium, EE was released and told to return to work Monday morning.

Employee was opening our entrance gates, the gate has a wheel on it to where you roll gate open, wheel got turned employee kicked wheel with foot when EE heard and felt a pop in knee causing a strain to knee area.

Employee was exiting loader, when ee reached the 3rd rung hands slipped on handrails causing ee to fall to ground. At the time it was raining hard and handrails were wet. Took employee to ER to get checked out and they found nothing other then some bruising. Dr's excuse said to return in 2 days but employee was not scheduled to work except for half a day the following day.

Employee was removing a metal strainer from a pvc line at our Sand plant, when he tried to remove the pvc line from the metal strainer he cut top of left thumb.
Employee was standing on crusher platform using a digging bar to try and dislodge a wedged rock in jaw crusher feeder when bar slipped catching his finger between the bar and handrail. This caused a laceration to tip of ring finger on right hand requiring 8 stitches.

EE was on a scaffold 4' off ground welding hooks to hold electrical cables, he went to move and a board had been moved causing him to fall. Lanyard caught him but he tit his rt hand on scaffold causing injury. He did not report to management. Co-workers asked if he needed to go to Dr, he stated he was okay. Reported 11/12/12. He has surgery for joint locked in flexion.

Employee was walking to #1 tailpulley when he stepped across about a 2 foot puddle of water, he felt a pop in his leg. Employee pulled a muscle in upper right leg, causing him to use crutches with limited walking.
EE was putting new brake pads on dump truck when he felt a strain in lower back. Employee went to doctor and was put on light duty work for 4 days.We are looking into this accident. We had a helper with him, he did not report any injury friday before he left work but came in monday morning first thing and reported back injury.

EE WAS SHOVELING MATERIAL OUT FROM UNDER BINS FOR APPROX. 10 MINUTES WHEN HE FELT PAIN IN HIS LOWER BACK AND DOWN HIS LEG.
EE WAS DRILLING ON TOP OF HILL. IT WAS A WINDY DYA, WHEN A GUST OF WIND BLEW SOME DUST INTO EE'S EYES UP UNDER HIS SAFETY GLASSES, CAUSING IRRITATION TO EYES.

EE HAD BUCKET UP IN AIR WENT TO SHAKE BUCKET, CASE LOADER STARTED TO ROCK BACK & FORTH, EE PUT HIS RIGHT FOOT UP ON THE FRONT OF THE CAB WHEN HE ACCIDENTLY HIT THE ARM LOWERING LEVER CAUSING THE ARMS TO COME DOWN CATCHING RIGHT FOOT BETWEENFRAME & METAL BAR BETWEEN ARMS, CAUSING BRUISED RIGHT FOOT
EMPLOYEE NOTICED A WATER LEAK IN A SMALL WATER PIPE. EMPLOYEE DECIDED TO PUT COUPLING ON TO STOP LEAK. HE WAS TIGHTENING SCREWS WHEN HE STATED THAT HE FELT WRIST POP.
