Mostrando los 24The conditions at the plant were all clean and in good shape. The problem happened when the employee slipped while putting in a bolt on the motor mount at the cone and was not wearing his fall protection equipment.
Approximately 4 hours of welding in 100+ degree weather with leathers on & in semi-confined area. Was drinking water but became weak & started cramping up leading to impaired ability to control self for 15 minutes.

WHILE PUTTING RETURN ROLLER IN PLACE ON A/B STACKER FEED CONVEYOR, THE RETURN ROLLER CAME OUT OF MOUNT & STRUCK RT. HAND INDEX FINGER. FINGER WAS HIT BY THE RETURN ROLLER & FINGER WAS CAUGHT BETWEEN FRAME OF CONVEYOR & ROLLER.
JOSTLING IN HAUL TRUCK IN MINE PIT RESULTED IN CHATTERING OF TEETH WHICH CAUSED TOOTH TO FRACTURE
While attempting to reverse the rotation of an electrical motor, employee received flash burns to both eyes & minor burns to his rt. wrist. EE opened what he believed to be the correct circuit breaker. However, the wrong circuit breaker was in fact opened thus the power was not shut off to the motor. He was using a pair of needle nose pyiers to move the wires, explosion occurred.
STEPPED OFF LADDER ON LOADER & FELT BACK POP. PULLED LOWER BACK MUSCLE.

RESTRICTED USE OF RING & PINKY FINGER, LIGHT DUTY ON MIDDLE INDEX, NO LIFTING W/LEFT HAND.
Ee stated he was swinging sledge hammer to break up rocks. On one swing he missed hitting the rock causing his body to twist oddly causing strain.
DURING REPAIRS TO YELLOW 100 FT BELT, FORKLIFT & OPERATOR WERE HANGING LAST PIECE OF SKIRTING STEEL WHEN FORKLIFT TURNED OVER ON ITS SIDE. OPERATOR WAS THROWN AGAINST INSIDE OF ROPES & CAB CAG E CAUSING GASH & BRUISES.

NEITHER OPERATOR NOR LABORER FOLLOWED PROPER LOCK-OUT PROCEDURE WHICH LEAD TO UNIT ACTIVATING. INJURED EE WENT IN TO FEEDER TO CLEAR ROCKS. OPERATOR SAW LABORER CLIMB OUT & LEANED OVER TO YEL L AT LOADER OPERATOR ON RADIO NOT TO DROP BUCKET OF MATERIALS. AS HE LEANED OVER, OPERATOR HIT FEEDER BUTTON & LABORER FELL ONTO BELT.
EE WAS STANDING BEHIND BERMED OFF AREA OF BACK FILLED MATERIAL WHEN THE GROUND STARTED TO MOVE. AS HE TURNED TO MOVE FROM THE AREA HE LOST HIS FOOTING AND FELL BACKWARD ON HIS LT LEG.
EE STATES "COULD ONLY USE RIGHT HAND TO PULL HIMSELF UP ONTO THE TRUCK. PUT RIGHT FOOT IN STEP ON FUEL TANK, REACHED UP AND GRABBED BAR. AS HE WAS PULLING HIMSELF UP, HIS FOOT SLIPPED & HIS WHOLE BODY SHIFTEDTO THE RIGHT & HE FELL ACROSS THE FUEL TANK.
LOADING OUT PIT RUN, BANK COLLAPSED, NO ONE AT FAULT.
EE WAS GRADING ROADS. HE WAS LETTING THE BLADE DOWN & WHEN THE BLADE TOUCHED THE ROAD, THE CONTROL LEVER KICKED BACK CAUSING WRIST TO BE JAMMED.
A/C BLOWER FAN ON 988 LOADER WAS STUCK. EE PUT HAND IN TO SPIN THE FAN AND IT STARTED UP AND UPLLED HIS FINGERS INTO IT. THE SWITCH HAD BEEN LEFT IN THE "ON" POSITION BY MISTAKE.
EMPLOYEE WAS ADJUSTING TAIL ROLL AT CRUSHING SITE WHEN WRENCH GOT CAUGHT CAUSING FINGERS TO BE SMASHED.

CAUGHT HIS HAND ATTEMPTING TO FREE STUCK CONVEYER BELT IDLER ROLLER.
