Emp was stuck by a angle iron brace that was being used to hold up a roll of belting. The angle moved and the piece struck them in the left shoulder causing them to fall to the ground. They worked the rest of the shift and went to the ER after the shift. They treated them for left shoulder and elbow pain. Employee was placed off work until next Tuesday, 11/18/25.

Mostrando los 25Miner picked up can of break cleaner out of service truck walking to back of truck and slipped on some rock causing miner to start falling. Miner reached out with right hand to stop fall causing break cleaner can to hit rear bumper of truck. The can exploded in miners hand causing lacerations to EE's index and middle fingers.
Employee was sticking barge for draft and stern wire broke hitting employee in the lower right leg. Employee's lower right leg and ankle were broken as a result. Employee was given first aid and transported by ambulance to Hospital.

Employee was pulling a chain out from under a parts box and tore left bicep.
Employee was helping in hooking up a plastic 8" pipe to pump. Pipe was being pulled with strap and equipment to pump. The pipe broke friction from windrow and struck employee pushing employee into stationary pump. This caused a contusion on employee's back.
Employee was pulling barge rope and strained a shoulder. Employee didn't feel like they needed to go to doctor. On Sunday June 7th employee called supervisor and said they needed to go to doctor because shoulder was hurting worse. Supervisor sent employee to ER because they were the only place open on Sunday, ER released employee to light duty.
Miner was lifting 2 sample buckets into the back of pickup truck and felt a pull in EE's back. Employee strained EE's back. Was taken to clinic and Doctor put employee on light duty for strained back.
Employee was throwing a 1" steel cable and felt a pain in right elbow.
Clevis holding cable broke and struck EE on left leg. At the time of accident it was a first aid DR visit only all x-rays were negative. Two follow up DR visits because of light pain, Dr ordered MRI and referred to Orthopedic. Results torn ACL and surgery was scheduled to repair on 11/20/14. Employee had outpatient surgery and was released to LD the same day.
Employee was descending a ladder into a hopper barge when the ladder slipped on the barge floor. He lost balance and fell approximately 12' to the bottom of the barge fracturing his left ankle, fracturing his left elbow and compressing the L2 disk in his back.
While replacing a 38" return roller on conveyor C6 the roller slipped out of the bracket on one end striking the employee in the mouth knocking one tooth out and breaking two more off at the gum.
Employee tripped and fell, did not want to go to dr. Feb decided to seek medical attention, medical provider x-ray negative, set up MRI, results read March 25, out patient surgery on March 28 released to restricted duty March 29.
Employee was going to truck and started jogging and fell on rock.

Employee was cleaning out inside of portable screen deck and material fell and got into left eye.
Employee was working on Snubber Pully on conveyor. When last bolt was removed the pulley rolled foward and pinched right index finger between impact wrench and angle iron resulting in a cut and broken right index finger.
Employee was working on a conveyor belt; wrench slipped causing him to strike his hand against the conveyor resulting in a broken little right finger.
Employee was tightening nut on bolt at plant using cheater pipe in late January 07. Went to dr, neg xray, regular duty. Continued pain, back to dr on 2/20/07. Findings were lumbar strain, bruised rib, restricted duty.
Employees lifting I-Beam to place on lowboy. Beam bumped track of trackhoe and pushed end around knocking employee on ground against berm and beam caught foot. Steel toed shoes, hard hat in place. No damage to mine equipment.
Employee pulled his back while attempting to unclog the feed chute in Unit One.
EMPLOYEE WAS DISMOUNTING FROM A DOZER DURING RAINING CONDITIONS HE HAD PLACED HIS RIGHT FOOT ON THE GROUND AND HIS LEFT FOOT WAS ON THE BLADE. HIS RIGHT FOOT SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL RESULTING IN HIM FRACTURING HIS RIGHT WRIST.
INJURED PARTY WAS BLADING A HAULROAD WITH A TRACTOR, BELOW AN INCLINE. A ALOADED HAUL TRUCK TOPPED THE HIL AND DIDN'T SEE THE TRACTOR IN TIME TO STOP, RUNNING INTO BACK OF TRACTOR, CAUSING THR EE BROKEN TOES TO THE OPERATOR OF THE TRACTOR.
