EMPLOYEE WAS TAKEN TO SUMMERSVILLE HOSPITAL AND REFERRED TO CAMC HOSPITAL. HE WAS TAKEN INTO SURGERY TO CLEAN INFECTION OUT OF HIS HAND ON 7/15/2011. EMPLOYEE WAS RELEASED ON 7/18/2011.
Employee filed for Black Lung then the following month quit. Letter received on 11/1/11 in HR department. Part 50 audit at Maple Coal revealed that this needed reported.
Coming down hill to prop plant was about 75-100 ft from bottom of hill truck broke lose could not stop hit bank and over turned.
While employee was operating the continuous miner in the #3 face a piece of rock was thrown back off the cutter head and struck the ee in the RT eye and nose. It was reported that the ee had his safety glasses on and they were knocked off and/or broken.
Employee was crossing the #5 belt when he slipped and started to fall. He had a hold of a belt chain with his left hand and while trying to catch himself he pulled his left shoulder. The mine was idle and the belts were all off.
Employee was cutting side boards off #2 s/c he was up in the boom of the s/c and step backwards causing him to slip. His leg went between a brace across the end of the boom and the conveyor chain, this cause a bruise to the left leg.
EE was watching miner cut in the #5 entry. While observing the miner, a small piece of rock was thrown back off the cutter head, striking him in the right eye. Rock went under and around by the side of his safety glasses.
While cutting a pin out of a D10 dozer track, a piece of hot metal popped into employee's ear causing a burn. Employee didn't go to the Dr until 10/21/10 at which time he was given a prescription for medication and returned to work.
EE was trouble shooting in miner panel when right elbow came in contact with 1 phase of 990 volts. He continued to work for approx 1 hr when he said he felt funny and was going to hospital and get checked out.
EE was using a razor knife cutting car cable splice open when the knife slipped off of the cable cutting EE's right leg above the knee.
EE was helping put a lid on Power Center and his finger got mashed between the box and lid.
Piece of rock fell from roof hitting employee on the left hand and arm causing bruise rock measured 1"x3'x5".
Employee was hit with a piece of fly rock while a single hole was being shot. The employee along with 3 other employees were positioned behind a D11 dozer while shot was being shot. The piece of rock measured 4" x 2" and either was a glancing blow or just brushed his back.
Employee was stepping off the #2 bolter onto the mine floor and twisted his right knee.
EE was hauling concrete up to the scale house with fork lift, when the fork lift ran out of fuel, lift started to roll backwards down the road. EE said he started hitting buttons trying to set park brake when lift didn't stop, EE took off seat belt and stepped off lift. EE then fell backwards hitting his head and lower back.
While getting off D11 dozer, employee put all of his weight on right arm causing a burning feeling and knotted up. Didn't say he slipped, just step down causing all his weight to be pulled against right arm.
Employee was wearing safety glasses and a scarf around face while grinding on tailpiece. He had quit grinding. With scarf & glasses still on, he coughed blowing debris from scarf under glasses into his eye.
The coal truck was coming down the hill and met another truck coming up the hill. The driver got over too far and ran into the ditch causing the truck to turn over against the bank. As the driver was pulling himself out of the truck, he hurt his shoulder.
While lowering the canopy on the roof bolter, EE had his right hand on top of the lever that lowers the jack. The canopy was lowered down on top of his hand. EE was looking away while lowering the canopy.
EE was walking from power center towards the face, when he tripped on a piece of rock laying on bottom and fell. rt knee was bruised.
Employee was getting in scoop. Stated he had hold of scoop canopy and was stepping into deck of scoop. Felt pain in Rt shoulder as if it dislocated.
EE was putting hand up on cable that was hung from roof. Said he felt rock and looked up and was hit with a small piece of rock in the face.
The operator left the cab area, walked down the stairway toward the lower compartment of the Hitachi Excavator. He hit his head on a piece of metal causing a cut to his head requiring stitches.
The 988B operator was going around a parked D & S truck. As he started around, a tractor-trailer was coming the other way. The back end of the trailer was about to hit the back of the loader. The loader operator steered the loader so he would not get hit. The bucket then went into the side of the parked D & S truck ripping off the hydraulic tank, steps & put a hole in the bed.
While bolting top in the #6 entry, a piece of rock approx 2' X 1' X 5" thick fell and hit the operator side of the canopy. The piece of rock glanced off the canopy and hit the off side bolter operator in the back, causing a bruise.
While the employee was rolling up fly pad, the fly board broke and hit him across the nose cutting it and possibly breaking it (not likey).
Miner was in 6 Rt, while cutting middle band of rock, the continuous miner started to bounce. The miner boom was bouncing up and down, causing the load-lock retrieve valves to release and let the boom fall into the S/C. This caused S/C to bounce and throw EE, the S/C operator, up into th top of the canopy.
Pulling belt with scoop, belt pulled against rib. He got caught in belt and hurt ribs.
Lifting up small cable hoist with plant hoist. Small hoist rolled over pinching right hand pinky finger against plant hoist hook.
While working on right drill pot, employee had hand laying on bolter sissor arm when right hand bolter man was told to move head causing employee's finger to be pinched between the bolter arms and bolter frame.
Employee was cutting in right hand x-cut when he for some reason advanced inby permanent roof support and was struck with a piece of falling roof.
While hanging belt chairs using a 6' step ladder ee fell from about 3 steps up ladder when ladder started to turn over from being set on loose material.
Pulling miner cable over out of roadway.
Lifting flight bar out of belt transfer, felt pain in abdomen/left side.
Backed miner up to clean up cut. Rock fell from between coal rib and roof bolts striking top of head. Knocked to ground - had four knots on head, knot & bruise on left shoulder. (Rock was 3' x 3' x 6" in thickness.)
Lifting bucket of concrete mix overhead, felt pain in left arm and shoulder.
Laying interlocking block for seals & working on knees. Spilled concrete mix that got in employee's clothing and caused chemical burning on knees and shins.
Man slipped & fell striking hand on rock. Dislocated middle finger and small laceration.
