Working on installing a power link pin in the bucket at RH200 shovel. He had a 1400# power link pin secured to boom of his service truck & was swinging the bin with the boom on his service truck to the power link & he got his lt little finger caught between the pin & power link of machine while guiding the pin in the hole of the power link & mashed the end of lt little finger.
EE used an 8lbs hammer and struck a ratchet to break a volt loose. When he did he felt pain in his right shoulder. EE worked until 8/27/10 and had surgery for a torn rotator cuff. It was initially diagnosis as a shoulder sprain. I am classifying this as an occupational illness due to the report provided by the treating physician. See attached.
While trying to set an anchor pin for the highwall miner, the pin shifted, pinning the miner's hand between the pin and a box on the miner.
EE released a chain ratchet which had tension on it. When the ratchet released it resulted in a piece of metal striking his left index finger.
Employee was spooling his cable back on his trucks boom, when he used his left hand to guide the cable. He got his left index finger caught with the cable, causing a skin amputation to the tip of his left index finger.

EE was cutting trees on a slope. While walking between trees, he slipped on an unseen rock. He fell onto the chain of his saw, saw brake was engaged and chain was not moving, cutting his left arm.
Employee cut a tree. As the tree was falling, it hit another tree breaking a limb out of the cut tree. When the limb hit the ground, it came back and hit him in the arm.
Employee was striking a wear plate on the shovel bucket. When he struck the plate a piece of metal broke and struck him in the right upper leg.
Had removed the dozer hood to take the radiator out, when the heat shield slid down the exhaust stack and cut right thumb.
Employee cut a tree. When the tree fell it kicked up a stick that came back and hit him in the face shield. The face shield cracked and hit him in the eye.
