Mine No 1 Coal

On May 26, 2015 this mine was notified that this employee has been diagnosed with pneumoconiosis. This employee worked from 12-05-1990 to 03-04-2013 when he was terminated.

As employee was descending the front steps of the truck his hand slipped from the handrail and he fell backwards to the ground, landing on his hips.

Employee was diagnosed with occupational hearing loss. Employee last worked December 18, 2013. Company was notified on 11/10/2014 of the claim.

Employee was checking oil levels in hydraulic tank. When he bent over and raised up, his foot slipped causing him to fall from the deck of the bulldozer, landing on the ground.

Employee was operating a 785 rock truck when the boom of the 992 G loader struck the side of the 785 rock truck's bed, shaking the employee causing back and shoulder pain.

Employee slipped and fell on black ice while walking to guard shack to begin his shift.

Installing engine in D10T, it had been raining all day & was installing rear mounts, drive line & hoses in hole & standing on angled belly pan & slipping & right knee popped and overextended.

Employee has been diagnosed with a degree of Black Lung. This employee worked from 10-5-1998 to 9-30-2010 when he retired. On 11-21-2012 this mine was notified that he was diagnosed with pneumoconiosis.

Employee was getting out of the rear of a service truck when he had one foot on the bumper and the other on the rear step. He slipped & fell striking his head on a concrete pad used to service equipment.

Employee was diagnosed with pneumoconiosis on 3-5-2014. Company was not notified of this event until 6-20-2014.

Employee was attempting to turn on his water valves on the side of his dozer engine when his finger came in contact with the fan belts resulting in the amputation of the right index finger at the 1st joint.

Chain slipped causing puncture to right hand, requiring 16 stitches. Employee still off work

Working on installing a power link pin in the bucket at RH200 shovel. He had a 1400# power link pin secured to boom of his service truck & was swinging the bin with the boom on his service truck to the power link & he got his lt little finger caught between the pin & power link of machine while guiding the pin in the hole of the power link & mashed the end of lt little finger.

EE used an 8lbs hammer and struck a ratchet to break a volt loose. When he did he felt pain in his right shoulder. EE worked until 8/27/10 and had surgery for a torn rotator cuff. It was initially diagnosis as a shoulder sprain. I am classifying this as an occupational illness due to the report provided by the treating physician. See attached.

While trying to set an anchor pin for the highwall miner, the pin shifted, pinning the miner's hand between the pin and a box on the miner.

EE released a chain ratchet which had tension on it. When the ratchet released it resulted in a piece of metal striking his left index finger.

Employee was spooling his cable back on his trucks boom, when he used his left hand to guide the cable. He got his left index finger caught with the cable, causing a skin amputation to the tip of his left index finger.

EE was in the process of giving his pre-shift to the fueler, when he fell to the ground from his dozer. He received 3 fractured ribs and a laceration to the right side of his head, requiring 2 staples.

Employee was doing his normal duties working on hooking beams together when he noticed a catch in his back and he had trouble straightening up in an upright position.

Unhooking hose from bin, hose flopped around, hitting EE's face.

EE was cutting trees on a slope. While walking between trees, he slipped on an unseen rock. He fell onto the chain of his saw, saw brake was engaged and chain was not moving, cutting his left arm.

Employee cut a tree. As the tree was falling, it hit another tree breaking a limb out of the cut tree. When the limb hit the ground, it came back and hit him in the arm.

Employee was striking a wear plate on the shovel bucket. When he struck the plate a piece of metal broke and struck him in the right upper leg.

Had removed the dozer hood to take the radiator out, when the heat shield slid down the exhaust stack and cut right thumb.

Employee cut a tree. When the tree fell it kicked up a stick that came back and hit him in the face shield. The face shield cracked and hit him in the eye.

Employee's foot slipped between push arm and track. The weight of his body pushed him to the ground with his leg between track and push arm.

EE WAS CHANGING GEAR PACKAGE (HIGHWALL MINER HEAD) BY STRIKING IT WITH A HAMMER AND A PIECE OF METAL FLEW OFF AND EMBEDDED ITSELF IN EE'S LEFT LEG.

EMPLOYEE WAS CUTTING TREES ON A STEEP SLOPE. WHEN THE TREE BEGAN TO FALL A LIMB BROKE OUT OF THE TOP OF THE TREE. EMPLOYEE RAN TO ESCAPE THE FALLING LIMB CAUSING HIMSELF TO FALL DOWNHILL APPROXIMATELY 20 FT.

PUSHING ON LARGE ROCK, BLADE SLIPPED FROM ROCK CAUSING JOLT TO NECK

EE was servicing a DIIR dozer. While climbing down from the machine, ee's foot slipped on right push beam causing him to fall to the ground. First aid given & EE was taken to hospital.

EE WAS STANDING ON THE BUMPER OF THE DUEL TRUCK. WHEN HE ATTEMPTED TO PULL THE HOOD UP, HE SLIPPED AND FELL BACK ONTO LEFT HAND.

WHILE DOING REPAIR WORK ON HIGHWALL MINER, EE SLIPPED AND FELL HURTING HIS LEFT HAND.

EE STUCK FINGER IN AC VENT FAN CUT END OF FINGER

WHILE USING AN AIR POWERED GRINDER TO CUT OFF A PIECE OF EXHAUST TUBING ON SERVICE TRUCK, GRINDER WHEEL CAME APART AND MATERIAL CAME OVER TOP OFSAFETY GLASSES AND WENT INTO RIGHT EYE.

BENT OVER TO OPEN DRAIN VALVE ON ENGINE OIL ON THE GEN SET AT THE HIGHWALL MINER.

THIS RECEIVED NOTICE ON 2-4-03 GRANTING 5% AWARD FOR OCCUPATIONAL PNEUMOCONIOSIS.

WHILE BACKING UP TO BE LOADED, EE BACK INTO SHOVEL BASKET, CAUSING PAIN THE LOWER STOMACH AREA

BACKED INTO LARGE ROCK CAUSING A JOLT TO BACK AREA.

EMPLOYEE WAS GETTING OFF THE DOZER WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL. HE CAUGHT HIMSELF WITH HIS HAND.

THE EE WAS DISMOUNTED A D-9N WHEN HIS FOOT PIVOTED TO THE RIGHT AND HE FELT A BURNING SENSATION TO HIS LOWER BACK.

WHILE DRIVING A ROCK TRUCK, EMPLOYEE RAN OVER A HOLE IN THE ROAD. HE TOLD SUPERVISOR HE WENT TO THE BATHROOM AND PASSED SOME BLOOD. HE LEFT TO SEEK MEDICAL ATTENTION.

EE WAS HELPING TO REMOVE WHEEL UNITS FROM 992G USING A PIPE AND BREAKER BAR TO GET TOP BOLTS AND NUTS LOOSE. SOCKET POPPED OFF AND FELL, PIPE FHHIT EE IN TOP OF HEAD. PREMIUM ENGERGY EE DRIVE OUR EE TO HOSPITAL.

OCCUPATION PNEUMOCONIOSIS, 11-21-02 5% AWARDED.

EE WAS BEATING A PIN IN PLACE WITH A SLEDGE HAMMER AND STARTED FEELING PAIN IN HIS BACK.

ON DECEMBER 13, 2001 NOTIFIED THAT EMPLOYEE WAS GRANTED 1.1 % AWARD FOR HEARING LOSS.

WHILE CHANGING TIRE, EE STRAINED HIS STOMACH MUSCLE. STARTED MISSING TIME ON 2/14/02. CO NOTIFIED 2/20/02.

WORKING ON D10N DUEL TILT VALVE, WAS REMOVING SPOOL AND IT WAS TRUCK AND SPOOL RELEASED WITH PRESSURE PINNING HAND BETWEEN SPOOL AND HOSE ON END OF VALVE.

LIFTING A SPRAYBAR OUT OF COMPANY PICKUP AND FELT PAIN IN GROIN AREA AND UPPER ABDOMEN.

HE TOOK PLUG OFF HYDRAULIC MOTOR. HYDRAULIC OIL HIT HIM IN THE EYE.

LOADING LARGE ROCK WITH 5230 HYDRAULIC SHOVEL WHEN SPROCKETS ON SHOVEL SLIPPED ON TRACK CAUSING A JOLT TO THE BACK.

EE WAS GUIDING A PUSH ARM IN PLACE WITH BOOM AND LEFT HAND. THE PUSH ARM MOVED SUDDENLY AND BEFORE EE COULD LET GO HE FELT A SHARP PIN IN THE BACK AND LEFT LEG.

EE WAS CLEANING TRUCK DUMP AREA WITH A DOZER. WHILE OPERATING THE DOZER HE BACKED INTO A LARGEROCK, GIVING HIM A JOLT TO THE LOWER BACK.

EE STARTED MISSING WORK AGAIN ON 8/28/00 10/2/00JUST NOTIFIED BY WC

WHILE DRIVING A ROCK TRUCK, A ROCK ROLLED ONTO REAR OF TRUCK GIVING THE DRIVER A JOLT TO THE NECK AND BACK.

WHILE DRIVING TRUCK, BEGAN TO HAVE LOWER BACK PAIN.
