Employee stated that employee was tramming a scoop along the travelway and as employee was making a turn through a crosscut a 20' joint of 4" waterline caught the operator's compartment deck and sprung upward and hit upper portion of employee's back close to left shoulder.

Employee was using a chain hoist at which time the chain hoist slipped out of the clevis causing the handle of the chain ratchet to strike employees LEFT pinky finger causing a laceration.

Employee stated was setting up the bolter to install a resin bolt close to the left rib when a portion of draw rock measuring 24" x 21" and approximately 4" in thickness fell hitting EE's lower right side of EE's back.

Employee stated employee was carrying an acetylene bottle when tripped over the corner of a top structure causing employee to fall and twist back and hip.

Employee stated was walking by a rail supply car when a portion of rib rock measuring 24" in width, 18" in thickness and 20" in length fell hitting employee's lower left leg around the calf area.

EE has been diagnosed with Occupational Pneumoconiosis

Employee has been diagnosed with disorders (repeated trauma) involving employee's back. The operator is reporting this instance as a code 26.

A certified electrician was load testing a 6 volt battery on a 3-wheeler with a volt meter when a cap blew off from the center cell, exposing the electrician to liquid from the battery, toward their face and eyes.

Contractor employee was pulling a rubber tire supply car with a scoop. To avoid hitting a dewatering pump ee decided to set the scoop over against the right rib. As ee proceeded to tram the scoop, the back of the scoop deck undercut the rib causing some rib rock 30" x 14" and approximately 5" in thickness to dislodge striking ee on the back of ee's hard hat and shoulders.

Employee stated EE was installing a 6' resin bolt when a section of draw rock fell from the mine roof striking the tip end of EE's left hand ring finger. The rock measured 18" x 18" and approximately 3 inches in thickness.

Employee has been diagnosed with Occupational Pneumoconiosis.

The D-10R dozer was being disassembled in preparation to be moved. The push-arms that had been removed were sitting on the ground. Employee stepped over one of the push-arms and it fell, striking right leg causing a fracture.

EE was helping 2 other employees lift a shuttle car speed reducer on to a 4-wheeler when EE felt something pop in right knee. EE works a 4 on 4 off schedule and was scheduled to return to work on 6/26/2017 in which EE's doctor stated was unable to return to work.

Employee stated was removing a rock off of the left side continuous miner measuring 12" in length 13" in width and 10" in thickness and as EE was going to roll it into the conveyor chain the rock shifted and rolled back onto left hand.

IE was operating Roof Bolter in 5 Right location in face. In Process of installing second row of bolts. When piece of draw rock fell from the roof between ATRS and drill canopy. The rock struck employee's hard hat and cut employee between hard hat and safety glasses. Rock measured 21 inches wide x 19 inches long x 6 inches thick.

Employee was lapping up approximately 50 feet of line curtain when they felt a sharp pain in their lower back. The curtain was covered with mud and water at the time of the incident.

EE (contractor) was assisting 3 co-workers to hang a section of belt and structure to a belt chain. As ee was lifting ee felt a sharp pain and something pop in lower back.

Employee has been diagnosed with Occupational Hearing Loss. Brooks Run South Safety Department was notified on April 20th, 2016. Employee was terminated on 11-10-14.

Employee has been diagnosed with Occupational Hearing Loss. Brooks Run South safety department was notified April 15th, 2016 of the diagnosis. Employee retired on 12-18-15.

Employee was operating # 1 S.C. traveling up #3 entry, coming from the feeder, back to the left miner, when employee ran through a dip in the haulage way causing a pain between shoulder blades, in upper back. Employee visited local hospital due to strain to upper back.

EE was moving the miner cable out of the roadway when the bolt machine was backing up the bolter cable caught the employee's left leg and caused it to bend backwards injuring EE's left knee. This employee did not seek medical attention at the time of the incident. EE went to the doctor on 3/14/2016 in which it was determined EE could not return to work pending on an MRI.

The injured employee was being assisted by a co-worker loading top structure into a scoop bucket and as EE turned to load the last piece of structure EE felt some pain in upper back between shoulder blades.

Employee changed the water filter on the continuous miner and was putting the shield back in place with help from a coworker. As they dropped the shield down employee caught right finger between the shield and frame.

Employer was notified on 2/8/16 by the compensation administrator that the employee was diagnosed with hearing loss on 1/28/16. EE's employment was terminated from the mine on 11/10/14.

Employee had changed out the collector bag in the roof bolting machine dust box. Employee slammed the door shut on the box and caught the middle and ring fingers of left hand between the door and frame.

Employer was notified by the workers compensation administrator on 3/21/16 that the employee had a diagnosis of pneumoconiosis on 1/7/16.

Contractor employee was installing resin bolts in the face of No. 8 entry. Employee was pushing the bolt over with the palm of employee's right hand and as employee was aligning the pin wrench to finish installing the bolt employee engaged the rotation causing the bolt to spring striking employee's right hand.

Employer was notified on 1/8/16 that the employee was diagnosed with hearing loss on 12/17/15. His employment terminated on 12/30/14.

Employee and a coworker were installing a bottom belt roller. One end of the roller was put in the hanger. To install the other end, the coworker lifted up on the belt using channel locks while injured lifted the end of the roller into place. Some weight of the belt was still on the roller and he felt pain in his back as he lifted it.

Employer was notified on 11/20/2015 that the employee was diagnosed with pneumoconiosis on 9/2/2015. The mine was closed in March of 2015.

Employee received a diagnosis of hearing loss on 11/3/15. The mine closed in March 2015.

Employer was notified on 10/6/15 by the compensation administer that the employee was diagnosed with pneumoconiosis on 8/20/15. The mine is non-producing and the employee is no longer employed at the mine.

Employer was notified by the workers compensation administrator on 11/16/15 that the employee was diagnosed with hearing loss on 10/6/15.

Employer was notified on 01/14/16 that the employee had a diagnosis of pneumoconiosis on 09/30/15.

Employee was positioned next to the center section of the scoop near the operators compartment when the scoop inadvertently moved causing an injury to his right lower leg.

Employee was positioned underneath the offside of the Fairchild scoop when it inadvertently moved causing an injury to the employees left ankle and shoulder.

Employer was notified by compensation administrator that the employee had a diagnosis of pneumoconiosis on 9-14-15.

Employee was in the process of unloading trash from bucket of scoop. The scoop was loaded with mine trash and was parked near the dumpster on the surface of the underground mine. The employee was lifting trash into the dumpster. He pulling on piece of curtain it was stuck, resulting in twisting of lower back. Continued to work rest of shift then went for medical attention.

After completing the cut in #4 heading near spad #3839 the employee was in a squat position. Upon standing and taking two steps, stepped onto the miner cable and waterline and twisted his right foot. Employee is unable to return to his regular duties.

Employee was diagnosed with pneumoconiosis on 7-21-15.

Employer was notified on 7/10/15 by the compensation administration that the employee was diagnosed with hearing loss on 6/16/15. The employee is currently off work from an injury on 4/1/15 not related to this hearing loss diagnosis.

Employer was notified by the compensation administrator that the employee had a diagnosis or pneumoconiosis on 7/10/15. He is no longer employed at the mine and the mine is no longer operating.
