Employer was notified on 10/6/15 by the compensation administer that the employee was diagnosed with pneumoconiosis on 8/20/15. The mine is non-producing and the employee is no longer employed at the mine.

Employer was notified on 01/14/16 that the employee had a diagnosis of pneumoconiosis on 09/30/15.

Employer was notified on 7/9/15 by the compensation administer that the former employee was diagnosed with pneumoconiosis on 4/6/15. The mine is shut down and the employee was terminated on 5/27/2013.

Employer received a notification on 1/8/15 from the compensation agent that the employee was diagnosed with pneumoconiosis from an examination on 10/16/14. The mine is inactive and the employee terminated employment on 3/28/13

Employer was notified by the compensation provider on 11/4/14 of a pneumoconiosis diagnosis for the individual on 8/20/14. The employee shows termination of employment on 1/15/2013.

Employer was notified on 11/18/14 by the compensation provider of a diagnosis of pneumoconiosis for the employee on 10/30/14. The mine is idle and the employee was terminated from the mine on 11/9/12

Employee received a diagnosis of pneumoconiosis on 7/1/14

Employer received a notification on 7/30/14 that the employee had a diagnosis of pneumoconiosis on 4/16/14. Individual was last employed at the mine on 5/27/13.

Employer received notification on 7/18/14 that the employee had a hearing loss diagnosis on 4/16/14. Individual was last employed at the mine on 5/27/13.

The employee was walking near the NE Mains power center and fell striking his head and falling to the ground onto his left hand injuring his hand and shoulder. Injured did not miss work until 1/14/2014

The employer was notified on 9/16/2013 that the employee was diagnosed with pneumoconiosis.

Employee has been diagnosed with simple pneumoconiosis.

Occupational illness

A roof fall was found by fireboss in airway of 11 north, left return at 6cc of #1 entry (2 breaks outby spad 856). 20' wide X 70' long X 6-8' thick.

Fall of roof in inactive area of mine. 18'X18'X30" at spad 1rt956. No one injured. Rerouted alternate lifeline. Dangered off, timbered off all approaches.

Confirmed OP diagnosis on 2/13/13.

Injured was hanging fly pad back when he slipped and twisted his right knee causing bruising and swelling.

EE was standing on a 4' ladder to set a spool when he slipped & fell to the ground. EE struck his left elbow on the bottom crushing & bruise felt. EE was holding a metal roof strap & when he fell he received a small 3/8" laceration to the left index finger at the first joint. Bleeding was very minimal. EE exited the mine under his own power and refused treatment and/or ambulance.

Employee was shoveling tail piece when a rock came off of the belt hitting the employee on the top of his left foot

Fall of roof. NO injuries! A fall occurred at spad 11R234 between #2 & #3 entry across 50cc of 11 right. The fall was 6ft. thick at the level of anchorage. No persons were affected and ventilation is not affected. This is an inactive area of the mine. All approaches to the site were breakered off (8 each) and dangered off. The incident was reported to hot line at 2:10pm.

Employee was examining stopping with inspector. Fan pressure pushed the stopping down as the employee approached. Air pressure pushed the employee 50' away along the mine floor with debris from the stopping. Employee struck right elbow on debris. Employee continued to work at full capacity until first missed shift on 4/2/2013 and is currently still off work.

Employee was throwing feeder cable out of roadway when he felt a pain in his lower back.

The employee misplaced a 6x8x16 solid cinder block on a stopping which fell striking the employee on the gloved left thumb. Employee finished his shift, sought medical treatment and returned to work. He did not miss any scheduled shifts.

Using utility knife to cut belt when knife slipped. Employee cut finger on his other hand.

Employee was riding mantrip in when it was struck from behind by another mantrip.

Employee has been declared eligible for Part 90 status.

Continuous miner cut into unknown water well.

Emp was cleaning around tailpiece when he felt a pull in his lower back.Employee finished shift.Emp continued to work until 5/18/12 and took 5 days vacation time through 5/25/12 during which time he scheduled an MRI. His 1st date of missed work was 5/29/12 awaiting results of MRI. We received notice of a positive MRI on 6/14/12. Employee remains off work at this time.

Employee was installing an anchor bolt when his rubber glove adhered to the drill steel twisting his hand.

Injured was trying to move a rock out of the walkway when he felt a sharp pain in his left shoulder. Employee finished his shift and did not seek medical treatment for an extended time.

Injured received a chemical burn on his buttock when the contents from an aerosol can of "duster" came in contact with his pants.

Employee was struck by a piece of coal 2'X 2'X 1'on the rt ankle.Employee received 1st aid,transported to surface to waiting ambulance.Employee was treated at Logan Reg Med Ctr and referred to orthopedist. Employee sustained fracture of ankle bone which required placement of screw during outpatient surgery. Employee remains off work at this time.

Event was called out as a roof fall at south 25c/c,3-4 entry BUT was actually 22c/c,3-4 entry. This was ONLY a continuation of a previously reported fall. The area had already been cribbed off AND dangered off and reported to the proper agencies. No injuries or fatalities are associated and no ventilation or escapeways are interrupted. No inundations are observed or detected.

An unintentional roof fall in the return entry of E panel, at 24 X-cut. The fall measured 19' x 100' x 4 to 5' thick.

An unintentional roof fall occurred in the #3 entry face and crosscut right o 11 right E panel sub-mains. The fall measured 19 wide by 55 feet long and 2 feet to 4 feet high.

An unplanned roof fall has occurred, above permanent anchorage, on the 11 north miner section. This fall exists in the #4 belt entry, one crosscut outby spad #566 and measures approximately 50'X 20" X 10' high.

An unintentional roof fall occurred on the 11rt A belt at 19 c.c. The fall measured 45' X 20' X 8'.

The employee was installing a plug onto the end of a 120 VAC cable. He was using a screwdriver to tighten the screw when his screw driver slipped and went into his hand.

EE was assisting the motormen in putting on a wrecked rock duster back onto the track. As the jack was being let down it creeled to the side and the bar struck the ee in the left cheek resulting in contusions and a laceration.

While helping set bits on the continuous miner the ee started down the #3 entry and was struck by a piece of rock that fell from between the bolts and pushed the ee to the floor striking him on the shoulder and left leg. The injury was not known until 11/9 due to a previous injury, when an MD read a MRI revealing a fracture to the tibia.

EE was roof bolting top when a small piece of top rock broke loose and struck him on the left forearm resulting in a laceration requiring 4 sutures to close.

An unintentional roof fall occurred in the face of #4 entry, it crosscut on the 11 north mains section. The fall occurred in an unbolted 20 foot cut and pulled two roof bolts in the last row of permanent support. Fall 20' x 19' x 2'.

An unintentional roof fall occurred in the #2 entry of 2 left pillar section. The fall came past the breaker timbers and measured 70'X5"X20'. The area was dangered off and breakered before the fall occurred.

An unintentional roof fall occurred in the #2 entry of 1 left pillar section. The fall was approx 80' X 20' X 8' and started with a normal pillar fall and came outby over the riding of the Breaker timbers. No one was working in the area at the mine.

An unintentional roof fall occurred on the 1 left pillar section at the pillar line in the the #3 entry. Two Fletcher MRS's were trapped by the fall. The area was cribbed and timbered off. The immediate area was resupported and the MRS's were retrieved.

The employee was loading a hydraulic pump into a scoop bucket when the pump slipped and caught his index finger between the pump and the bottom of the scoop. The accident resulted in a severed tendon in his right index finger between the knuckle and the first joint.

A fall measuring 15' long x 19' wide x 5' thick was discovered while firebossing the #2 belt at 22 x-cut south mains between #3 & #4 entries at spad #154. The fall will not be rehabilitated and all entrances are breakered off.

Anomalous gas readings indicated the presence of methane and carbon monoxide behind new seals installed in the west mains of the mine. It was found by MSHA that the glue used to couple the seal gas testing tubes emitted fumes which the detectors read as methane and carbon monoxide. Air samples were pumped from behind the seals and results indicated no methane or carbon monoxide

Due to abnormal amounts of rainfall an inintentional inundation occurred causing water to collect from crosscut #2 to crosscut #11, in all entries, on the South Mains, resulting in the inability of 7 men to exit the mine.

An Unintentional roof fall was discovered ~8:30 AM in the South Mains Primary Escapeway. The fall was located between #103 and #104 crosscuts, # 7 entry and measured ~ 50' x 20' x 5'. The area was supported with 48" fully grouted resin rods.

Employee was operating shuttle car when he got against the rib and a piece of rock or coal came into the canopy and caught his hand against the grab handle he was holding onto resulting in the partial/full amputation of all four fingers on the left hand. Employee was flown from the mine site to St. Mary's Hospital by Healthnet Aeromedical service.

The employee was operating a 4 wheel emergency vehicle when the wheels hit a rock in the roadway causing the steering wheel to turn suddenly, hitting his little finger resulting in a fracture to the little finger on his left hand.

Dusting belt line - got close to belt, snagged clothes on belt - belt slammed to ground.

Roof fall occurred on 10left section entry #3 and #5 at spad no.10LTA907, 4500' from the active face, size of fall, length-150' width-18' height-6 to 8'. All entrances to fall area will be breakered off on evening shift 8/27/2008. There were no injuries, no equipment involved, area will not be rehabilitated.

EE was moving a shuttle car cable anchor when the cable took-up and hit him on the heels. The EE was rolled backward landing on his back and hands. The EE did not miss work until 8/11/08 when he went to the DR and has not returned.

Employee was operating a scoop in last open break. When he backed up he caught the canopy on the rib and a section fell into the deck striking his left shoulder. He was complaining of pain in his left shoulder and arm. When he realized he was against the rib instead of pulling up and repositioning he tried to continue in reverse.

EE was using a slate bar to pull pin from a rubrail on C/M. When the pin came out, the rubrail struck him in the bend of his leg (ankle/foot), knocking him to the ground. He was complaining of pain in his right leg, foot/ankle. The rubrail was bent just slightly making the pin difficult to pull.

The employee was bolting the roof using two pieces of stack steel to drill the hole. When he started to remove his steel the top piece stuck in the hile momentarily. When it fell out of the hole, it struck the machine drill head and bounced up striking the employee in the mouth

The employee was operating a shuttle car in the #2 entry of the 10 left section when he hit a hole causing him to be thrown around inside the cab and felt a pain in his lower back and pelvis.

The employee was walking across the section when he slipped on wet and muddy floor. He caught himself on the rib. He felt pain in his back. He worked until 6/27/08 when he left for vacation. He did not return from vacation due to his injury. His first missed day was 7/7/07.

The injured was bolting top when he moved the head of the bolter to put up an inside bolt. As he was drilling the hole a sliver of rock fell behind him and cut through his leather boot into the flesh above his right ankle resulting in a laceration requiring sutures to close.

An unintentional roof fall was discovered at 1:49 pm at 50 x-cut #3 to #4 entry measuring 8' long x 5' high x 20' wide. The fall will be timbered and dangered off.

Employee stuck his head out from under the shuttle car canopy and was struck by the cable taking up.

An unintentional roof fall was discovered at 2:20 pm. An area approx. 10' x 10' had fallen from around bolts near the mine drift mouth in the primary escapeway.

The employee was using a cinder block to knock a ventilation hole in a stopping when his left index finger became caught between two blocks.

The employee was starting to experience pain in his right groin and was diagnosed with a rupture of hernia as a result of repetitive trauma.

There was an unintentional roof fall on North Mains 39 x-cut inbetween #5 & #6 entries. This is an inactive area and travel is not regularly required. The area measures 30' long, 18' wide, 5' high.

An unintentional roof fall occurred at 39 x-cut 10 left track entry. The area is inby active workings and is used only for access to Bleeder evaluation points and seals. Two travelways remain open to the area.

The EE was exiting the scoop when he slipped on wet belting in the roadway, his left leg went underneath the scoop, resulting in a twisting motion to his knee.

The employee was using a knocker type splice tool when a piece of metal flew from the knocker and struck him in the right index finger and imbedded itself. The finger developed an infection requiring treatment prior to removal.

An unintentional roof fall 30'x20'x4' occurred on the 11 left section #6 intake entry at spad #282.

The employee was installing the last bolt in the face, he placed the glue and bolt into the hole and hit the fast feed on the droll pot. This caused the canopy to drop 3 to 5 inches and strike the ee in the head. The canopy had been raised by the drill pot pressure on the floor and was not defective.

Employee was shoveling between the top and bottom belts when the shovel got caught between the top belt and the top of the top belt roller. The shovel flipped around and struck the employee on the right forearm.

The employee was trimming the longwall belt with a utility knife when the knife slipped and he raked it across his right thigh resulting in a laceration.

The inj EE was in process of assisting with the loading of a roll of 54" belt into a scoop bucket.As he was standing next to scoop, a rib (corner) 24"x40"x18" fell & struck the EE on rt leg.

The employee was in the process of installing a coupling onto an 8" water line. The employee was using a crib block to knock the coupling onto the water line when his left little finger was caught between the coupling and the crib block resulting in a laceration requiring 7 sutures.

The employee was changing a hose for the water discharge sump. At the completion of his task he hopped or jumped down from on top of the continuous miner, landing on his right foot resulting in a fractue of the 5th metatarsal bone.

The injured employee had walked up into the entry to check the positioning of the MRS he was two feet inby the corner standing next to the miner operator when a rib rock 62" x 12" x 14" popped off of the right rib striking him on the right shoulder and upper torso.

Employee and a co-worker were in the process of drilling holes into the mine floor to install anchor pins for a new belt drive. As the employee was attempting to re-position the drill, his right index finger was caught between the drill steel and the chuck of the drill, resulting in a laceration requiring 7 sutures to close.

The EE was loading a trapping shoe into a scoop bucket when it rolled over onto his right index finger resulting in a laceration requiring sutures.

Employee slipped on water line, fell and struck his left arm and shoulder on the pipe.

The employee was walking around a flat car to place blocking under the equipment on the car. He slipped and fell striking his left knee.

Employee was moving the continuous miner cable when he felt pain in his back and left pectoral muscle.

Employee stepped down onto slick bottom (mud) when his feet flew out from under him causing a hyperextsion of the knee.

Employee was walking across wooden bridge when a board broke causing him to fall.

Employee was helping make a belt splice - slipped and fell on knife cutting his left hand.

Employee was building stopping when the stopping fell over and cinder block struck him in the side of neck and back.

The ee was hooking a chain to pull a cable sled when the sled shifted in scoop bucket and caught the injured rt hand between the sled and scoop bucket.

Using a crib block to knock a spline on sizer reducer, he caught left ring finger and middle finger between spline and crib block.

Several employees were installing cribs along the 9LT cross belt, when a piece of draw slate approximately 42" x 50" x 0-3" fell from between the bolts and struck the injured on his right side.

Employee was recovering belt structure, he pulled on a bottom roller with the roller hanger. The hanger slipped off of the roller resulting in the hanger striking employee above left eye.

Employee was riding #26 Jeep. The Jeep started to back up & employee hit his head on the 73' belt guard.

Injured was riding on mantrip when a board slid down from the rib. The board went into the operator's compartment and caught injured's left ankle against the seat.

Unintentional roof fall approx. 160' x 20' x 8' fell in old 11 RT (idle) belt entry. Inby spad # 11 R 79.

Unintentional roof fall, #5 entry (neutral) 70'x 18'x8'.

An unintentional roof fall approximately 100' x 18' x 6-10' occurred on the return near the "Dead Mens Branch" bleeder fan.

Roof fall above anchorage was found during normal examinations of the bleeder system. This fall does not impeed travel of air or men. All entrances have been breakered off and area will be rehabilitated. Measurements: length 80', width 18' to 20', height 6' to 8'.

While pulling together a broken belt, the employee was struck in face when chain broke.

Unintentional roof fall 37 x cut north macas. #4-#5 entry. Fall measured 50'x 19'x 12.

Employee working on long wall move piece of rock fell between 2 cribs striking him on neck and shoulder.

Injured employee was holding a 2" hose with channel lock pliers. Another employee was striking a coupling with a hammer. The hammer slipped from his grasp and struck the victim in the mouth.

In the outby section of 12 RT. Section, in the #18 crosscut Track Entry a roof fall above anchorage was found to exist.

Employee struck the rib with the scoop he was operating. Employee's head struck scoop canopy.

Employee was lifting 60" belt structure roller with belt with bar.

Employee reached over a carton of grease weighing 8 lb to get a 50 lb bag of dust. He should have moved the carton of grease.

Employee was bolting ribs. was struck by a 3'x 18' x 6' Rib.

EE was struck by a 2" piece of pipe when it was uncoupled. The pipe was in a bend and flew through the air. (end of pipe)

Employee was removing the terminal end for a locomotive battery. The negative lead was cut and layed to the side. As the employee cut the positive lead, the negative lead came into contact with the wire cutters resulting in an arc.

EE was resting his hand on the scoop blade, as the operator withdrew the blade, the EE's hand was caught between the back of the bucket and blade.

The employee was exiting a scoop when his foot was caught in the deck, causing him to fall forward onto his hands.

THE INJURED EMPLEE WAS INSTALLING ROOF BOLTS WHEN A PIECE OF TOP ROCK FELL AND STRUCK HIM ON RIGHT FOREARM.

Employee was holding a 6" x 16" roof plate in line and caught his right index finger between plate and the roof.

Employee was removing the cover from an electric track switch and caught his right index finger between the cover.

He was getting plates off the bolter for the roof bolts when the other operator moved the machine and ran over his left foot.

The employee was pulling a cable and lost his balance. The employee fell to the ground on top of the plug. Employee started losing work days on 3/17/06.

Employee was seated inside mantrip waiting to go up hill on surface track. A second mantrip exited the mine on the same track and struck the first trip. Injury not reported as lost time until 3-1-06.

THE EMPLOYEE WAS UNCOUPLING A 1/2" WATER HOSE AND FAILED TO RELEASE PRESSURE ON THE LINE. WHEN UNCOUPLED, THE HOSE STRUCK THE EMPLOYEE ABOVE THE LEFT EYE (LACERATION - 9 SUTURES).

The injured employee stepped onto a 6" discharge pipe and lost his balance. His right hand grabbed for the belt structure and was pulled into the bottom roller. The employee was able to free his arm.

The employee was hanging a ventilation curtain at the long wall head when a rock fell and struck him on the right leg.

Employee was in process of installing roof support cribs, for shield removal. A piece of top rock fell and struck him in the lower back with a glancing blow. The employee continued work until 2-7-06 before starting to miss.

Employee was stacking block for metal doors on 14rt. said shomething hit him on upper back.

While plastering stoppings got b-bond on skin.

INJURED WAS ATTEMPTING TO REMOVE THE GUARD FROM THE MASTER CONTROL PANEL OF THE 14CM15 MINER. THE GUARD SLIPPED AND PINCHED HIS LEFT RING FINGER.

Employee was stepping into the face conveyor, to attach a cable when he struck the front ok his knee on it.

Employee was building stoppings on 11 section when block fell striking thumb on right hand.

Rock fell from top and struck the injured on the left side of the face & nose.

Employee was building support cribs at #11 BT entry when he slipped lacerating his elbow on left forearm.

While installing 6ft resin bolt using a RRII Fletcher bolter, the bolt came out of drill pot on machine striking employee in the left arm and causing a contusion on the left arm.

Unintentional roof fall 18 x 70 x 8. #41 Cx North Mains spad #0414. 6,000 ft outby face.

Employee was loading mill car when rail shifted and pinched middle finger on right hand.

EMPLOYEES RT MIDDLE FINGER WAS CAUGHT BETWEEN A BRAKE HEAD AND THE FRAME OF THE SHUTTLE CAR RESULTING IN AN EVULSION REQUIRING 8 SUTURES TO CLOSE

EMPLOYEE WAS UNHOOKING A ROCK DUSTER FROM THE SCOOP BUCKET AND RECEIVED AN INJURY TO HIS LEFT ELBOW. THE TENSION ON THE CHAIN HOLDING THE DUSTER IN PLACE WAS TOO TIGHT CAUSING THE EMPLOYEES ARM TO HYPER EXTEND.

A ROCK APPROXIMATELY 24" X 17" X 1-2" FELL FROM THE TOP AND STRUCK THE VICTIM ON THE LEFT HAND WHILE HE WAS INSPECTING A HYDRAULIC TAKE-UP PUMP AT 31 X-CUT ON 11 LEFT NORTH.

An unintenitional roof fall occurred 9 x-cut, #3 entry 10 lt north maijs, the fall measured 20'x20'x4-5'.

INJURED WAS CLEARING LOOSE COAL FROM AROUND THE ULTRA-TRAC PIN WHEN HE WAS CUT ON A SHARP PIECE OF METAL

EMPLOYEE TRAVELING TO SURFACE OPERATING #8 MANTRIP. TROLLEY POLE CAME OFF OF WIRE AND CAUGHT HIS HAND BETWEEN POLE AND WIRE

UNINTENTIONAL ROOF FALL, 80' X 19' X 4'. #2 ENTRY, SPAD #117, 2500' OUTBY FACE, 10 LEFT SECTION.

THE INJURED EMPLOYEE WAS MOVING A 42" BELT, DRIVE ROLLER, WITH A SLATE BAR. THE BAR SLIPPED & THE ROLLER ROLLED BACK ONTO HIS FOOT.

Unintentional roof fall. 50' x 50' x 8'. #4 belt entry. Spad #278 1200 feet outby face. 1 left section.

Unintentional roof fall 20 x 18 x 4. #10cx, #1 entry spad 15. 3800 ft outby face, 15 right section.

An outby laborer received a laceration to his right foot when two mantrips collided, at a switch on the mine yard.

EMPLOYEE WALKING OUT OF LOAD OUT TUNNEL STRUCK HIS HEAD ON CROSS BEAM CUTTING FOREHEAD

UNINTENTIONAL ROOF FALL #10 ENTRY #10 CUT. INTAKE ON NORTH MAINS 10' X 18' X 7'

An ee's hand was caught between the frame of the scoop bucket and the ejector blade.

Unintentional roof fall. #9 entry #49 xcut return on west mains area. 19' x 40' x 7'

Emp. was getting ready to change bit on hammer. Driller asked if his hands were clear of connection point. Emp said yes. Driller proceeded to begin connection which pinched end of emp's middle finger of left hand between hammer and bit.

A general laborer twisted his left knee when he slipped while carrying a belt structure. He did not start missing work until 5-19-05.

EE struck his lt. thumb with a 3 lb. hammer while attempting to remove a broken bit from the continuous miner head.

Unintentional roof fall #1 entry, 23 x-cut, return, 15 rt section 005-0 100' x 18' x 6'.

EE WAS WALKING IN HIGH INTERSECTION THEN STARTED INTO PART WHICH WAS LOWER STRIKING A ROOF BOLT PLATE, PUNCTURING HIS EYE, EE WAS WEARING SAFETY GLASSES.

Employee was using a high-pressure water hose to wash out underneath the stageloader. He lost control of the hose, was knocked down and struck on the right wrist the hose nozzle.

AN UNINTENTIONAL ROOF FALL 25' X 15' X 6' OCCURED #10 CROSSCUT. #3 ENTRY, 15 RT NORTH MAENS. 100' IN BY SPAD 514.

The motor operated by the injured employee had a broken coupling pin and mechanical problems preventing it from independent operation. It was chained to another motor pulling it. The coupling was being held in place by a jack bar. The pull motor stopped causing the coupling to make contact and pinch the injured's left hand between the bar and motor.

EE WAS LOADING BELT STRUCTURE INTO A SCOOP BUCKET, EE LEFT LEG BECAME TANGLED IN THE STRUCTURE. EE LOST BALANCE, FALLING FORWARD STRAINING LEFT KNEE.

Park brake leaked off manbus causing it to run away striking rib when it derailed throwing ee around inside bus.

The injured ee jumped from a mantrip after it started rolling. He struck the ground with his hands and received a laceration to his left wrist requiring 5 sutures.

AN EMPLOYEE WAS STRUCK BY A PIECE OF TOP ROCK MEASURING 54" X 31" X 1/2" TO 12" THICK.

WHILE WALKING TO HIS WORK SITE, THE EMPLOYEE SLIPPED ON WET, UNEVEN BOTTOM AND STRUCK HIS LOWER BACK. HE CONTINUED TO WORK AND DID NOT START MISSING WORK UNTIL 10/11/04.

An ee was struck in the back with a piece of top rock measuring 40" x 28" x 0-4" thick.

EMPLOYEE CLEANING BELT, REMOVED A PIECE OF METAL FROM UNDER BELT LINE, GETTING A SMALL PIECE OF METAL IN HIS LEFT INDEX FINGER.

EMPLOYEE WAS SPLICING A STAGE LOADER CHAIN. THE CHAIN SLIPPED AND CUT HIS RT. HAND.

AN UNINTENTIONAL ROOF FALL OCCURRED ON 3 RIGHT SECTION, 19 CROSSCUT #3-#4 ENTRY. THE FALL MEASURED 80' X 20' X 6+ FEET. THE AREA WAS MINED OUT 1500" OUTBY AND SEALS HAD BEEN PLACED BETWEEN 19 AND 20 CROSSCUT.

THE EMPLOYEE WAS CHANGING WATER SPRAYS ON THE SHEAR DRUM. A CHIP CAME OFF OF HIS HAMMER AND LODGED IN THE TOP OF HIS RIGHT HAND.

EMPLOYEE STATES A PIECE OF ROCK FELL FROM ROOF STRIKING HIM ON HIS HEAD, BACK & LEFT LEG WHILE HANGING CABLE.

THE EMPLOYEE WAS CHANGING TRAM HOSES ON A STAMLER FEEDER WHEN HE FELT A PAIN IN HIS LOWER BACK. HE STARTED MISSING WORK ON 7/21/04. RULED AS OCCUPATIONAL INJURY ON 9/9/04 WVA WORKER'S COMP.

A PIECE OF TOP ROCK ABOUT 3' X 2' & UP TO 3" THICK FELL AND STRUCK THE INJURED ON THE HEAD, NECK AND RIGHT HAND RESULTING IN A LACERATION REQUIRING TEN SUTURES.

MULTIPLE ROOF FALLS OCCURED ON THE 3 RT. SECTION.

An unintentional roof fall 60' by 20' by 5' occurred on the 13 left section between #4 and #5 crosscut, #1 entry return.

Employee stepped over track to reach jacket, twisted his right ankle.

AN UNINTENTIONAL ROOF FALL 35' X 4 1/2' TO 5' X 20' WAS DISCOVERED AT 81 CROSSCUT, #8 ENTRY OF NORTH MAINS.

EMPLOYEE BUILDING CRIBS IN RETURN AIRWAY, OTHER EE'S PASSING CRIB BLOCKS TO EACH OTHER & ONE FELL HITTING EE ON RIGHT WRIST.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN #10 ENTRY RETURN OF NORTH MAINS. FALL MEASURED 45' X 20' X 3-6'.

THE INJURED WAS WALIKING DOWN THE SECTION ROADWAY WHEN A RIB 4' X 3' X 10" - 14" FELL AND STRUCK HIM ON THE LEFT LEG.

The injured employee was assisting with the removal of a tail roller bearing. Another employee was using a hammer and chisel to remove the outer bearing race. A piece of the back chipped off and struck the victim in the hand. The employee did not start missing work until 6-30-04.

An intentconal roof fall occured ont he longwall 7 rt. section headgate. the fall measured 70' x 16' x 6 1/2'.

The employee was struck on the right leg by a piece of 65 lb track rail being loaded on a low-boy which was moved by a continuous miner.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE FACE, #3 ENTRY, 10 LT SECTION. THIS UNIT IS IN NON PRODUCING STATUS. THE FALL MEASURED APPROX 60' X 19' X 6'.

THE VICTIM WAS BOLTING THE ROOF, PREPARING FOR LONGWALL RECOVERY, WITH A MICON DRILL. HE TURNED TOWARD THE SHIELDS & WAS STRUCK IN THE RT.LOWER BACK & HIP BY A 3'X 9'X 3-18"ROCK. INJURED DID NOT CEASE WORK UNTIL 5/7/04.

The injured ee was tightening bolts on a belt discharge roller bearing, his ratchet slipped causing him to strike his chin on the discharge roller, his upper teeth bit through his lower lip.

Getting off a 236 Cat Skid Steer - tripped on a "rut" - sprain or brake - left ankle/foot

Employee felt a buring sensation in his lower back, Employee stated he was suffered from back problems for many years.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BETWEEN 7 & 8 CROSSCUT, #1 ENTRY, NORTH EAST MAINS, MEASURING APPROX. 60' X 10' X 4'.

EE WAS CLIMBING UP STEP LADDER ON WET FLOOR TO CHANGE LIGHT BULBS. THE LADDER SLIPPED, CAUSING EE TO FALL FROM LADDER.

EMPLOYEE CARRYING BUT BUCKETS ACROSS FACE, SLIPPED AND FELL ONTO PONTOONS OF SHIELDS.

AFTER ATTACHING TWO CHAINS, FROM AN OVERHEAD HOIST, TO R 14 CM15 CONTINUOUS MINER, THE VICTIM'S LEFT THUMB BECAME CAUGHT BETWEEN THE TWO CHAINS WHEN TENSION WAS APPLIED.

UNINTENTIONAL ROOF FALL, 25' X 20' X 5'-6'. #8 CROSSCUT, #6 ENTRY ON 2 RT SECTION.

UNINTENTIONAL ROOF FALL 19' X 19' X 5-6'. #37 CROSSCUT, #9 ENTRY OF NORTH MAINS SECTION.

THE INJURED AND ANOTHER MINER HAD JUST PICKED UP A 4' X 10'. WHEN THEY STARTED TO WALK THE INJURED FELT A PAIN IN HIS KNEE.

EE WAS CUTTING NYLON BAND ON CINDER BLOCKS WHEN HIS KNIFE (UTILITY TYPE) SLIPPED & CUT HIS LEFT LEG.

EE WAS BUILDING CRIBS IN ENTRY WHEN HE PICKED A CRIB BLOCK UP & TURNED AROUND STRIKING THE CRIB HE WAS BUILDING & BREAKING HIS LEFT RING FINGER.

The ee was walking down the longwall return when he felt a distinct pop in his right knee. Injury was not reported to management until 3-8-04. EE started missing work on 4-26-04.

UNINTENTIONAL ROOF FALL OCCURRED, 40 CROSSCUT, #6 ENTRY, NORTH MAINS. 80'X 19'X 7-8'.

EE WAS HANGING PINNER CABLE WHEN THE CABLE FROM THE PINNER PULLED LOOSE, STRIKING EE AND THROWING HIM INTO CONTINUOUS MINER MACHINE.

EE CLAIMS TO HAVE BEEN STRUCK BY A RIB ROLL ON 11-5-03, HE CONTINUED WORKING UNTIL 11-18-03 AND REPORTED THE ALLEGED INJURY ON 12-9-03.

EE ATTEMPTING TO SECURE SET OF STEPS TO OVERCAST WHILE STANDING ON THEM. THE STEPS SLID DOWN OVERCAST HURTING EE'S LEFT ARM.

EE WAS STEPPING ONTO RAIL CAR FROM MINE FLOOR, WHEN HE PUSHED UP HE FELT PAIN IN HIS LEFT LEG.

AS THE VICTIM WAS ATTEMPTING TO BACK THE ROOF BOLTER FROM UNDERNEATH A ROCK WHICH HAD LODGED THE ATRS, A PIECE OF 4' DRILL STEEL WAS LODGED IN A HOLE FLEW BACK AND STRUCK THE VICTIM ON THE LEFT SHOULDER.

AN UNINTENTIONAL ROOF FALL OCCURRED ON 150' X 18' X 7'-8' ON NORTH MAIN 2 RIGHT AT 77 CROSSCUT BETWEEN #8, #9, AND 310 LAST OPEN CROSSCUT.

EMPLOYEE WAS OPERATING MANBUS, PULLED UP TO AIRLOCK DOORS, OPENED DOORS TO GO THRU, STARTED MANBUS THRU DOOR WHEN DOORBLEW SHUT STRIKING EE'S RIGHT LEG.
