The employee responded to a high-temperature alarm on the impactor crushers displayed on the HMI. As part of normal correction, the employee proceeded to the impactor building to activate the air horns for each impactor unit. The employee then proceeded to BIC2 and opened the air horn valve. A loose hose end became pressurized and struck the employee in the left eye.

EE reported, while changing a bearing on the 24 drive elevator in the Kiln area that they were lifting gear housing into position when an aerosol can with Knocker Loose fell from top of the drive elevator where it had been placed. When aerosol can fell they claimed that it hit floor, then ruptured and struck their calf and thigh, resulting in a scrape and bruising.

Employee was trouble shooting an air valve to a bag house when it popped off striking them in the mouth/nose area.

The employee was opening a lid on a tanker truck to begin filling it with CKD and the lid blew open due to pressure build up blowing the dust into his eyes, burning them.

EE was walking down the stairs on the bottom of the spray tower when water busted out of the bottom of 78 screw. The water flowed off the deck and caught EE on the stairs covering him with muddy water. Water blisters developed on his right calf/ankle.

THE INJURED EE AND THE WITNESS WERE REMOVING AN OBSTRUCTION INT HE DISCHARGE CHUTE FROM E126 ELEVATOR TO THE ALKALI TANK. THE END OF THE AIRLANCE THEY WERE USING WAS PLUGGED AND BROKE LOOSE, B LOWING ALKALI DUST OUT OF THE INSPECTION HOLE THEY WERE WORKING IN. THE DUST ENTERED THE INJURED EE LEFT EYE. BOTH EE WERE WEARING SAGETY GLASSES ONLY.

EE WAS APPROACHING LEFT OUTSIDE TRAILER TIRE TO INSPECT SLICE IN TIRE. AS HE PUT HIS LEFT HAND UP TO THE TIRE, THE TIRE BLEW OUT IMPACTING HIM AND THROWING HIM BACK TO THE GROUND 7 FT FROM THE TIRE.
