While replacing a screw conveyor at the Kiln Baghouse, an employee sustained a lower back burn from alkali powder exposure. The injury worsened over two days despite first aid and was reported to supervision and Health & Safety. The employee was later treated at a clinic, received a tetanus vaccine, and was diagnosed with a second-degree chemical burn.

Employee was getting ready for lunch when they felt something in their right eye, the employee was to the bathroom to flush eye out but was not able to get the material out of their eye. the employee was taken to a clinic where the material come out on its own. The employee was prescribed eye drops.

Employee was using a hammer to drive a pin out of a piece of equipment and smashed their right index finger while striking the pin.

MSHA Inspector was exposed too feed dust during conversation with Cemex employee, MSHA inspector stopped Cemex from preforming their job duties.

Employee was adjusting the forks on a forklift when they came loose and struck their left arm and left back

Miner while doing material removal at ground level of Finish Mill 2 building, employee reported shortness of breath and throat felt like it was closing off. EE was wearing a dust mask N95 together with a neck gaiter.

Employee was entering a silo by way of a work platform and as EE entered the silo EE heard a pop in EE's left knee.

At approximately 10:50 PM, two employees were cleaning beneath the screw conveyor when one employee came into contact with the other employee's tool, resulting in an injury to the affected employee's right eye.

Employee was ascending the stairs in Office Building keeping three points of contact. While approximately on the fourth step, the employee lost balance and fell backward down the stairway landing on the lower floor level and reported pain and discomfort to the hand and tailbone area. No slip, trip hazard, obstruction, or defect with the stairway was identified.

EE rolled EE's ankle taking a step down from an elevation. After an MRI is showed a small tear to EE's ligament.

Walking under a coal belt looking down due to ground been slippery from rain, EE hit EE's head on a conduit bracket and fell onto EE's back. Incident was reported to supervisor and miner said EE was feeling well. Next day shift EE reported that EE's neck was sore. EE was taken to urgent care for evaluation. The CT scan came out clean, and EE was diagnosed with a concussion.

A miner performing water blasting activities in the preheater tower riser area when handling the water lance and when the water supply valve was opened got a kickback force causing the gun section of the lance to strike on EE's right ear, knocking EE to the floor. EE was transported to the hospital, where EE received eleven stiches in the ear and was treated for a concussion.

Employee was changing shaker springs on Shaker C in Plant. While installing the new spring, the shaker deck dropped and pinched left middle finger resulting a laceration that required 13 stitches to close wound.

Employee was walking backwards pulling a water hose and twisted EE's right knee after EE tripped on a 10-inch rock. Taken to doctor for evaluation and diagnosed with knee sprain. Placed on restricted duty.

While assisting a contractor that was repairing a scraper at A03 in the A Frame, a material handler miner got hit by a piece of pipe previously removed fell down the working platform while EE was standing in the underneath catwalk and resting EE's right hand on the fix ladder cage. This struck caused EE an open wound fracture to EE's right pinky.

A maintenance employee slipped on the 3rd step from the bottom landing rolling their left ankle.

Employee asked another employee to release bucket on skid steer. employee raised bucket tilting it down. employee stood in front of bucket (in the line of fire) released the pins and the bucket slid off striking employee in their left foot. EE sustained fractures to 4 bones in their foot.

The employee responded to a high-temperature alarm on the impactor crushers displayed on the HMI. As part of normal correction, the employee proceeded to the impactor building to activate the air horns for each impactor unit. The employee then proceeded to BIC2 and opened the air horn valve. A loose hose end became pressurized and struck the employee in the left eye.

Employee sustained a laceration to EE's right knee while cutting with a hand tool during a maintenance task.

Contractor was assisting repairs of coal mill rollers during outage. When shaft was being lowered onto sawhorse the injured who was assisting the moving of the shaft, pinched their right handed middle finger between the shaft and the sawhorse causing a laceration in their finger. Contractor Supervisor took the injured to get medical attention. Receiving 10 sutures.

Elevator got stuck with EE inside at 5:41. Service contractor notified. MSHA called at 6:21 ref. 04978221. Service contractor arrived at 7:05. Stuck EE out at 7:33. MSHA notified. Service contractor cleaned dirty ground floor switch.

Around 2:30 pm, a mechanic while working in the FM1 separator at the time of positioning a come-a-long above their head, dust got loose and fell into their left eye. They flushed their eye and were taken to Urgent Care where medical evaluated and diagnosed with a corneal abrasion of the left eye, without a foreign body.

The employee stepped onto uneven ground and fell forward, after getting the MRI results during a follow up visit on 2-6-2026 EE was diagnosed with a torn hamstring

EE was lifting a full oxygen bottle into a UTV and during this task the bottle slipped pinching their right ring finger in between the bottle and tailgate causing a laceration and fracture of the finger.

K2 Elevator stopped between 6&7 floor w/ contractor inside @ 13:24. MSHA called 13:55 ref. 04940914. Service contractor notified & arrived 15:25. Stuck person out at 16:03. MSHA notified 16:04.

Employee was assisting in troubleshooting an ammonia pump when a mist of aqueous ammonia came from the work area and made contact with the side of their face and eye.

K2 elevator stopped moving with contractor inside. Contacted Control Room approx 12:25pm. Attempts to get elevator to move unsuccessful. Contracted elevator service company called. MSHA notified of entrapment approx 12:55pm. Elevator service contractor tech arrived. Stuck person out approx 1:50pm. Contactor wheel was replaced. MSHA notified that person got out at approx 2:00pm

In occurrence while EE was assisting an EE, this EE stepped backwards onto EE's left foot causing a closed displacement fracture of the third metatarsal bone.

A laborer working in the Finishing Mills was cleaning a gypsum spillage. The employee was filling and carrying two five-gallon buckets; the employee stated their back was hurting and had trouble standing and walking with a sharp pain. They were treated for a back spasms.

While attempting to remove a filter cover using a pipe wrench, the cover broke loose and the pipe wrench lost grip and fell onto EE's right great toe. The injury has been identified as a right great toe contusion resulting from a falling object. Employee had medical evaluation and was released for light duty.

EE was performing a drag resocket. EE experienced a laceration to the distal phalanx of the right-hand little finger when a small chip of metal that was embedded in the sledgehammer's handle cut the glove and finger. EE received stitches.

EE was lifting a mill bearing cap and the chain hoist hook stretched causing the cap to fall and graze EE's hard had and right cheek bone.

Employee was adjusting the drag cable on the drum when EE's finger became pinched between the wire rope and the drum. This resulted in a fracture with stitches to the left index finger.

On November 20, 2025, at about 3:00 p.m., the Maintenance Clerk was repositioning a 24x20-inch picture frame when it broke. Falling glass struck the employee's left kneecap, causing a one-inch cut. The incident was reported immediately. The employee received four stitches; no restrictions were issued.

Victim was working on unloading material that had been cleaned from the silo. Routine work for USA. They took a guard off saw a rock was lodged in a rotary gate, and decided to save time and effort used a finger to dislodge the rock instead of picking up the bar. The rock had moved allowing the rotary gate to rotate, and they couldn't move their finger fast enough.

Subjective c/o generalized back pain after transferring 1/2-full 5-gal buckets ~3-4' from 3'H ledge to coworker; no lifting, stooping, or twisting required. Mandatory weekend shift; EE wanted to leave early, complaint is suspect. EE refused post-incident drug screen, admitted THC/marijuana use. Terminated for cause per zero-tolerance drug/alcohol policy.

For unknown reason, haul truck veered over the berm and into quarry lake.

While cutting belt at tail pulley the blade slipped out and hit employees left arm. Left arm was positioned in a bad area when this happened. The cut was on EE's forearm 3 inches past base of thumb. Cut got fatty tissue and some mussel.

A Dozer Operator with 24 years of experience, was dismounting the dozer before EE's lunch break. Employee stepped from the track down to the first step, left shoulder popped as EE descended

Miner was working in Kiln 1 ballroom doing routine housekeeping, EE went to lift by self a 3x3 door (weight 50 pounds) used to return cleanup material to the silo. About an hour later EE felt a sharp pain in EE's groin and a dull pain in EE's lower abdomen. EE continued to work and finally reported the incident at 4:50pm

EE was lifting vactor hose and felt back pain.

On 10/03/25 at 8:35 am, the coal mill operation began. At 9:26 am, the control operator noticed a fire on the coal mill line. Employees secured the area and tried to extinguish it. When it persisted, the Hygiene Fire Dept. was called at 9:44 am and arrived at 9:56 am. MSHA was notified the same day and visited on 10/06/25 to review the area and follow up on the investigation.

EE was inspector bucket elevator and slipped on mill ball on ground.

EE was assisting with a drag rope endo when the eye on the tugger cable snapped. The employee was contacted by the drag rope, resulting in two lacerations that required stitches.

This is a personal issue on this employee from pre-existing condition. EE was preparing to gather EE's tools when EE slipped and felt a pop in EE's groin area.

EE was in the process of ascending on to a closed cab skid steer. As they attempted to open the door of the cab of the skid steer, the EE lost footing, pivoted to the side as they held on and then fell on to the ground resulting in a laceration to the upper part of the EE's head.

EE was replacing drive belts and caught left ring finger, resulting in the loss of nail.

EE felt right knee pain while ascending stairs on the shuttle wagon.
