EE was working on the #1 West Section (2) breaks outby the feeder loading crib blocks on the scoop. While loading the crib blocks, the corner rib rolled out measuring 5'x 3'x 1x1/2' thick striking the employee on the right foot resulting in (5) broken bones.

While rib bolting off of the continuous miner in 7 South mains between 4 and 5 entry at 10.5 xcut, a piece of the rib fell from above the rib bolt striking the employee on the right leg causing a fracture.

Employee was operating the continuous miner in 7 South mains. A piece of rib fell out while mining between 4-5 entry at 10.5 X-cut and struck the employee pushing employee to the ground. The employee suffered 2 fractured ribs as a result of the incident.

Employee pulled Roof Bolting machine into the #4 Left to bolt cut. When rib rolled out from behind curtain striking EE in the left shoulder, knocking EE into the controls.

Employee was rib bolting on the tube side of the 1B Miner when a piece of coal fell from the rib and struck the effected employees right knee. Employee finished EE's shift. On 11/17/23 MRI results determined the employee has a partial quad tendon tear.

Employee was side bolting off the miner at approximately 7+00 survey station in the H3 section. The upper portion of rib became dislodged striking the employee in the head and shoulder area. The rock measured 3ftL x 26inW x 7inThick.The employee suffered a fractured vertebrae and 5 staples.

While bolting on the side of the miner a small piece of rib scaled off of the rib and struck the employee on the left index finger on 3/29. Employee continued to work all required shifts, EE continued to have discomfort in the index finger and went for an x-ray 4/25/23 and a fracture was discovered in EE's knuckle. Employee continues to work without restrictions.

Mains, #1 Section, at Section Feeder, at Spad 692. While performing maintenance on the feeder, the rib rolled out and the employee was injured.

Employee was rib bolting from the right side of the miner at approximately 80+20 survey station. The upper portion of the rib became dislodged striking the employee on the neck. The rock measured 9'longx34' highx8' thick. As a result, the employee suffered a compression fracture to the C3 vertebrae. No compliance issues were found. No other part 50 criteria apply.

Employee was roof bolting from right side of the miner at approximately 80+20 survey station. The upper portion of the rib became dislodged striking the employee on the back. The rock measured 9'longx 34'highx8' thick. This action caused swelling in the employees back. No compliance issues were found. No other part 50 criteria apply.

Employee was carrying tools from the stage loader on the tight side, when a rock fell from rib striking EE's left foot. Rock Measured 25" L x 5 ½" w x 4" thick. EE finished EE's shift and worked for two weeks before being diagnosed with a fracture on 12/3/21. Section was idle when accident occurred.

Employee was rib bolting when a piece of rock fell from the rib and hit right leg / ankle. EE was diagnosed with a fracture to lower right leg.

Employee tripped while walking to the miner. EE grabbed the rib to catch balance and a rock fell striking EE's left thumb. This resulted in 6 stitches. No lost work days or restrictions from this injury.

While working along the left side of continuous miner in the H1TG section #1 entry. Employee was in the process of supplying miner when a rock slid from where the rib and roof meet striking the injured in the back of the head. EE was diagnosed with a concussion.

Employee was rib bolting on the right side of the 4B miner. While drilling a piece of rock / coal came out of the rib and struck the employee. Employee received seven sutures to the left side of head.

Employee was side bolting along the continuous miner in the G2 Section #2 Entry, when a rock fell from the rib protector and struck employee in upper back/neck. Employee moved away from that area when a second rock struck employee on top of head. Employee was diagnosed with concussion like symptoms on 1/10/2020.

Employee was handling mononrail and tripped and fell and grabbed the rib. A piece of rib rolled out and hit EE on the ear causing injury.

Employee finished installing a roof bolt located in the 8 north section, when a rock fell from the rib, striking employee, causing employee to fall to the ground. Employee received six sutures in the right forearm.

While stitching a short block in the E31 CM section, the employee went to add a drill steel when a rock fell from the ATRS of the rib bolter, striking the back of employee's right hand. EE was diagnosed with a fracture to the right ring finger.

Employee was in the panline on the F-27 Longwall face hooking the panline up to shield when a rock slid out of the face striking employee on the left lower leg causing right lower leg to strike the eiko track. On 2/28/2018 employee was diagnosed with a non-displaced fracture of the fibula & a partial tear of the lateral collateral ligament in right leg and began missing work.

A piece of rock fell from the rib and struck employees right leg. This caused a fracture to the right heel bone.

Employee was walking by E29 longwall head drive motor when the mesh tore and rocks fell out, striking the left hand. They received 16 sutures.

Employee was changing bits in E29 Longwall on the tailgate drum when a rock fell from face and landed on left foot.

Piece of rock fell from the roof/rib (corner) and struck employee's finger between the rock and the rock deflector on the miner. This caused a fracture to left middle finger.

Employee was rib bolting off of the Continuous Miner, while drilling a hole a piece of rock fell from the rib striking the employee on the right elbow causing a contusion. On 9-22-15 the employee began missing work due to this occurrence.

Employee was rib bolting off of the continuous miner in the 7 North Mains Right Section. When preparing to start drilling a bolt hole a piece of rock fell from the rib striking the employee in the left ear, causing a laceration requiring 12 sutures to repair.

Employee was operating the miner in 3 North Right Mains, when a rock fell from the face and struck the slider tube. The slider came down on top of the employees hard hat and caused a laceration to his forehead. He received 6 sutures.

Employee was struck on the left ankle by a piece of falling rib.

Employee was struck on both legs by a piece of rib that fell out of the face while he was helping bolt.

Employee was struck by a rock while rib bolting off of the miner. The rock struck his right leg. The employee worked all scheduled shift until being taken off of work on 7/30/2014.

employee was scaling down roof getting ready to set-up an air drill, he was in the panline when the face rolled in on him.

A piece of rock fell from where the rib meets the roof and struck the employee on the head and neck.

He was bent over shoveling on the 7B Longwall Headgate area when a rock dislodged from the rib or roof and struck him in the back. He remained working without absence until medical evidence indicated a fracture of the L-4 Vertebrae. He began to miss work due to his injuries on 10-1-13.

He was walking to the miner when a piece of material became dislodged from the rib and struck him in the right ring finger. The result was two fractures and a laceration requiring two sutures to the right ring finger.

Individual was bolting on the left side of the Alpine Voist Miner when struck by a rock from the top of rib line struck the left shoulder. Individual experienced pain to the shoulder.

While working as a rib bolter in the BMX Mains Right Section a piece of Rib measuring 56" X 30" X 14" struck the employee on his left side and ankle. This caused a fracture to the employee's left ankle.

He was rib bolting on the right side of the 8B miner. A piece of stone rolled out of the rib and struck him on the front of his right knee causing a contusion. He began missing work on 4/25/13.

He was working near the rib next to the stage loader on the 7B Longwall section. The bottom part of the rib rolled out and struck him on the back of both legs. He complained of pain in his hips. He began missing work on 4/17/13 as a result of his accident.

The injured was installing a eye bolt for the messenger line in the No.3 entry 30'inby brk. No.66 when a piece of rib rolled off measuring 30"x24"x10" striking him on the left foot, causing a fracture to his left big toe. The accident happened on 21 Rt. development panel.

Employee was struck in head with a piece of stone from the rib. Employee received a cut to his head requiring stitches and a hairline fracture to his C-2 vertebrae.

Individual was operating the shearer on the 18 w longwall when a rock blew out at the face area striking the individual on the right ankle.

Rib rolled on the 6B longwall headgate area. The rib pushed him into the stage loader causing pain in his lower back. *We dispute the allegations in Citation #8051196 and submit this report solely to terminate the citation.

While working at BMX in the 1 A Tailgate section employee was filling the rockduster hopper on the loading machine in the #2 entry, when he was struck in the lower left leg by a piece of rib measuring 4' long X 2' wide X 12" thick.

Employee was rib pinning on the right side of the miner in the section. He was drilling the first hole for his rib pin on the corner. The rib rolled out striking him on the right shoulder and behind the right ear.

While roof bolting off the left side of miner in BMX section, a piece of black rock from the top of rib struck the employee on left hand and forearm.

Individual was a rib bolter on the 18w section fall #1 entry. The corner of coal pic rolled out striking the right side of face, pinned both legs thighs and right ankle. Individual suffered an abrasion to the right temple area, and bruises to the thigh area.

Individual was retrieving a roof strap towards the back of the miner when a piece of corner rib fell striking the outside of the left foot. Individual received a fracture to the fifth metatarsal.

While working on the tailgate ranging arm a piece of material fell out of the face causing the employee to fall striking his forehead on the shearer resulting in a laceration requiring sutures.

EE was cleaning the walkway along the 15-W Longwall stage loader, when a large rock rolled from the top of rib striking EE and pining him beneath the rock. EE sustained fractures to both sides of the pelvis and a cracked scapula on the left side.
