Employee was roof bolting when a piece of rock rolled out from the rib approximately 47" long X 7.5" thick X 14" wide and struck him on the left leg. He was transported to MARH hospital where he received 15 stitches. Employee had a follow up visit with the dr. on 5-1-12 and returned to work on restricted duty on 5-2-12.

The EE was scooping up a roll of belt when the deck of the scoop hit the rib causing the rib to roll out hitting the operator.

EE stated that he had hung a curtain 40 ft. inby spad 639 in #2 entry when a piece of rib apx. 4" thick X 18" wide X 24" long fell and struck him on the right leg. Employee was transported to MARH hospital where he was diagnosed with both bones fractured in his lower leg.

EE was helping change a hydraulic hose on the roof bolter when a rib rolled off and struck him in the hip and leg area. EE was treated and the mine and transported to Middlesboro ARH however he was airlifted to the University of Tennessee hospital rather than MARH. He had surgery for a fracture in the hip and he also had a compression fracture of the L2 vertebrae.

EE had walked to the continuous miner to talk to the operator. When he squatted down beside him, the coal rib rolled off and struck him on the back and lower legs. This resulted in a hairline fracture to the right lower leg and three fractures to the left lower leg.

Employee was working on stopping in the return when a piece of rock fell off of rib hitting employees left foot, employee became a lost time on 6/1/10 - 6/7/10 with bruised left foot. Employee has been released to reg. duty with no restrictions.

EE was helping on the continuous miner when a coal rib rolled off and struck a timber that was set against the mine roof. The timber struck EE on the right side of the head and shoulder.

EE was helping on the continuous miner in the #6 entry of the 002-0 mmu, when a piece of draw rock fell from between the side bolt and coal rib and struck him on the right side and hip. The rock was approximately 54" long x 30" wide x 2"-4" thick.

EE was operating a continuous miner in the #5 right crosscut and a piece of draw rock fell between the roof bolts and rib and struck him on the head.

Putting up belt curtains and a rib rolled off and struck his right foot. Started missing 5-4-2009.

Employee was tramming roof bolter to left break in the belt entry, when a rib rolled off hitting him on the left side of his face. Employee received sutures to left side of his forehead, eye, and nose. Employee was placed on restricted duty until 6-24-08, employee has returned to regular duty with no restriction. Safety talk was held with employee on safe work habits.

EE was cleaning belts and a rock rolled off of the rib and struck him. Started missing work 6/23/2008.

A piece of rock fell from the rib and hit ee on the left ankle. He has a fractured ankle and is working at a restricted duty status.

EE was setting the ATRS on the roof bolter when a coal rib rolled and struck his right leg, trapping it against the roof bolter. This occurred in the right crosscut in the #6 entry of the 003-0 mmu. Rib roll measured approximately 10' long x 2.5' high x 10" thick. The result was a fracture to the lower right leg.

EE was shoveling ribs in the #6 track entry 11 breaks outby #3 belt drive and a brow fell and struck him on the right side, injuring his right arm and head. Brow was approximately 3' long, 2' wide, and 6" thick.

EE stated that he was moving a continuous miner cable over to the rib and had turned, the rib rolled off and struck him on the right leg in the calf and knee area. EE's knee was pinned against a parked shuttle car by the material from the rib. EE was transported to the hospital where it was determined that he had a fracture to the tibia near the knee.

EE was shoveling #3 belt on the off side of belt when rib rolled off and hit him in the back.

Employee was pulling loose material from the rib when material rolled off and struck him on the left foot. He continued to work the rest of the shift and went to the doctor that night. There were no fractures and he was told to take the next 2 days off. He was released to come back to work on regular duty on 01/19/07.

Employee was struck by a rib roll and suffered a fractured ankle. Started missing work 11-27-06.

Employee was setting timbers in the #6 entry of the 002 section and a coal rib rolled off and struck him on the back and hips.

EE was operating a continuous miner in the #6 entry of the 004 section and a coal rib rolled off and struck him on the right side of his body, injuring his back, hips, and leg.

Employee was turning a right break when a rib rolled off striking him on the left foot. He continued to work and on 6/20 he went back to the doctor and a simple fracture was found in a bone in his left foot. At that time he was taken off work and the accident became reportable.

EE was getting an air reading behind the line curtain in the #4 entry of the 002-0 mmu when the coal rib rolled and struck him on the right hip and leg. He continued working until the end of his regular shift. On Saturday, 5/20, he went to the E.R. where he was treated and released.

Piece of sandstone rolled rib striking employee on back. Rock measured 72" long, up to 24" wide, by 1" to 5" thick. The employee started restricted duty on 4/01/06.

As employee was installing a roof bolt, a rock fell from the rib and rolled onto employee's left foot and ankle.

Employee was struck by rib roll on the right arm while installing roof bolts.

Rock fell off rib onto employee's back.

He was going to work on conveyor chain on the miner and rib rolled out on his right foot.

Employee alleges he was struck by a rib roll while chocking the scoop. He was struck on the right leg.

RIB ROLLED OUT ON MID BACK

EE WAS WATCHING THE MINE ROOF FOR THE CONTINUOUS MINER ON PILLAR WORK. A COAL RIB ROLLED OFF, DISLODGING A 9' TIMBER, STRIKING EE ON THE SIDE OF THE HEAD.

EMPLOYEE ALLEGES HE WAS STRUCK BY A RIB ROLL WHILE OPERATING THE CONTINUOUS MINER. HE WAS STRUCK IN THE RIGHT LEG, KNEE AND THIGH. (NOTE: HE DID NOT SEEK MEDICAL ATTENTION UNTIL 1/12/04)

EMPLOYEE WAS DRIVING A SCOOP AND RIB ROLLED OFF AND ROCK STRUCK LEFT HAND. BROKE RIGHT FOURTH FINGER AND RECEIVED 6 SUTURES TO LEFT PALM. NO LOST TIME.

OPERATING A REMOTE CONTROL UNIT FOR C/M WHEN RIB POPPED OFF AND STRIKING EE.

EE WAS MOVING MINER CABLE WHEN RIB ROLLED OFF STRIKING HIM IN THE BACK KNOCKING HIM FACE FIRST INTO MINE FLOOR CAUSING CUTS AND BRUISES TO HEAD AND FACE.

EMPLOYEE WAS STRUCK FROM BEHIND BY A RIB ROLL & SUFFERED A FRACTURE TO HIS LEFT LEG. HE BEGAN MISSING ON 12/2/02.

EMPLOYEE WAS TRAMMING A SCOOP WHEN A RIB ROLLED OFF AND STRUCK HIM CAUSING A LACERATION TO HIS HEAD.

WHILE SHOVELING LOOSE MATERIAL ROCK, COAL, ECT EE STRUCK LEFT HAND OVER RIB, CAUSING INJURY.

PIECE OF COAL/ROCK FELL OFF RIB, STRIKING EMPLOYEE IN THE LEFT SHOULDER WHICH CAUSED HIM TO MOVE FORWARD STRIKING HIS FACE AGAINST THE CANOPY OF THE SCOOP.

PULLING PIECE OF ROCK AND WHEN PULLED, ROCK RIB FELL HITTING HIM IN LEFT LEG.

A RIB ROLL OCCURRED AND STRUCK EE.

RIB ROLLED ONTO RIGHT FOOT AND BRUISED FOOT.

COAL RIB FELL STRIKING EE ON RIGHT ELBOW AND FOREARM CAUSING MINOR BRUISING.

A RIB ROLL OCCURRED AND STRUCK EMPLOYEE ON THE LEFT ANKLE.

EMPLOYEE WAS STRUCK BY A RIB ROLL ON THE LEFT LEG WHILE INSTALLING ROOF BOLTS.

EMPLOYEE WAS STRUCK ON THE BACK AND LEFT FOOT BYA COAL RIB ROLL WHILE SETTING UP THE MACHINE TO ROOFBOLT.

A RIB ROLL STRUCK THE EMPLOYEE ON THE BACK.

EMPLOYEE WAS STRUCK BY A RIB ROLL ON THE LEFT FOOT.
